Nobody enrolled in Psychcare; the coverage came from the health plan that hired Psychcare to manage the behavioral side of it, and that plan is still the one that decides whether a rehab stay is paid.

The name also no longer trades. Psychcare, which was called Psych/Care, Inc. until February 2006, was acquired on February 1, 2011 by Beacon Health Strategies, a Boston firm established in 1996; the deal is recorded in PitchBook rather than in any company announcement. Beacon merged with ValueOptions in 2014 to form Beacon Health Options, Anthem completed its purchase of Beacon on March 2, 2020, and the business was renamed Carelon Behavioral Health on March 1, 2023 under Elevance Health, the company formerly known as Anthem.

So when a Florida treatment program lists Psychcare among the names it works with, it is describing a benefit manager, usually an older one. Does Psychcare Cover Drug and Alcohol Rehab?

Coverage comes from the health plan behind the card, not from Psychcare. Substance use services were one of Psychcare’s listed service lines, alongside utilization review and third-party administration, but the levels of care paid for were set by the Medicaid, Medicare or employer plan that hired it. The benefit belongs to Humana; the review belongs to the vendor.

What Levels of Care Does the Behavioral Health Manager Review?

No Psychcare-era benefit document listing levels of care survives. The successor’s Florida provider manual, issued in June 2021, names inpatient mental health and substance use services, outpatient behavioral health treatment, intermediate treatment, partial hospital program and intensive outpatient program, applied across Florida Medicaid managed care, Florida Healthy Kids and commercial and exchange plans.

Medical Detox

Detoxification sits on the inpatient side of the review list, which means medical detox is authorized rather than excluded. The admissions team files the request, and under the Humana Healthy Horizons arrangement an inpatient request is taken on a round-the-clock line with the decision made during that call.

Residential and Inpatient

Residential treatment and crisis stabilization are both named among the inpatient services reviewed for Florida Medicaid members. Length of stay is settled by the level-of-care criteria the manager applies, which the June 2021 manual describes as its own published criteria rather than a named external set, so ask the program how continued stay at a residential rehab is reviewed.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient are named in the Florida manual, along with outpatient behavioral health treatment and intermediate treatment. On the Medicaid side, selected outpatient services, including psychosocial rehabilitation, targeted case management and therapeutic behavioral services, carry their own authorization requirement.

Which name is really on the benefit? Read the card first. The health plan issuing it, a Medicaid managed care plan, a Medicare Advantage plan or an employer plan, decides the benefit. The behavioral health organization printed beside it, or named when you call, decides the authorization. Psychcare was the second kind of name, and its work now sits with Carelon Behavioral Health.

Does Psychcare Need Prior Authorization for Rehab?

Yes for inpatient care under the plans its successor manages. Inpatient mental health, detoxification, residential treatment and crisis stabilization all require authorization, and a short list of outpatient services does too. Utilization review was a core Psychcare service, so authorization was always part of this name rather than an afterthought added later.

For a Florida Medicaid member whose plan uses Carelon, inpatient requests go by phone and are decided in the call, while outpatient requests are faxed on a form. The curious detail is that the fax number listed for Psychcare in a 2007 vendor profile is the same number Humana now publishes for outpatient behavioral health authorization requests to Carelon in Florida, which is the clearest trace of the old Miami operation still in service.

Which Florida Plans Still Use a Behavioral Health Carve-Out?

Fewer than once did. Molina Healthcare of Florida moved behavioral health and substance use management in-house on February 1, 2022 for its MMA and Comprehensive members, and Aetna Better Health of Florida ended its Beacon contract on March 1, 2022, moving to an integrated model with Behavioral Services Network for the provider network across MMA, LTC and Florida Healthy Kids, with continuity of care honored for 60 days.

Member lines for those plans are Molina Healthcare of Florida on 866-472-4585, and Aetna Better Health of Florida on 1-800-441-5501 for MMA, 1-844-645-7371 for Long-term Care and 1-844-528-5815 for Florida Healthy Kids. A third-party billing payer list still shows PsychCare as a Florida Medicaid behavioral health payer name pointing to Molina, which is why the old name keeps appearing on Florida Medicaid paperwork years after the company stopped trading.

If a program says it takes Psychcare. Treat that as shorthand for the behavioral health side of a Florida plan. Give the admissions team the plan name from the card, not the Psychcare name, and ask them to settle network status and authorization under that plan.

How Much Does Rehab Cost With Psychcare?

There are no Psychcare copays to look up, because Psychcare set none. Cost sharing was always written by the health plan, and that is still true of the plans its successor manages. For Florida Medicaid managed care the plan’s own schedule applies; on an employer plan the deductible, coinsurance and out-of-pocket maximum decide the bill.

No Psychcare medication policy or formulary was ever published under that name. Where the Prescription Is Filled

Medication given during an inpatient or residential stay is billed as part of the program, while a prescription collected afterwards runs through the plan’s pharmacy benefit with its own tiers.
