Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

What is Partners Health Plan (PHP Cares)?

Partners Health Plan was a Bronx-based not-for-profit that ran New York's only FIDA-IDD plan, PHP Care Complete, combining Medicare and Medicaid benefits for adults with intellectual and developmental disabilities in nine downstate counties. The demonstration started April 1, 2016 and ended December 31, 2025.

It was a Medicare-Medicaid Plan under the CMS Financial Alignment Initiative, run jointly by CMS, the New York State Department of Health and OPWDD.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Is Partners Health Plan still active?

No, the plan closed on December 31, 2025. Its own site and the New York State Office for People With Developmental Disabilities both state that the PHP Care Complete FIDA-IDD demonstration is no longer available, and new service authorization requests are not being processed.

Former members can reach Participant Services at 1-855-747-5483, TTY 711, 8am to 8pm, seven days a week.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Does Partners Health Plan cover rehab?

No, coverage ended with the plan on December 31, 2025. While it operated, substance use treatment sat inside the integrated benefit: the three-way contract defined behavioral health as treatment of a mental illness or a substance use disorder, and inpatient substance use rehabilitation was covered with authorization.

Former members now get substance use treatment through the new Medicare plan they choose and through New York State Medicaid, which continues unchanged.

Partners Health Plan, known by its short name PHP Cares, was a Bronx-based not-for-profit that ran New York’s only Fully Integrated Duals Advantage plan for adults with intellectual and developmental disabilities, and it stopped operating on December 31, 2025. A treatment listing that names it is describing a card that no longer pays for care delivered today.

The product was called PHP Care Complete FIDA-IDD, a Medicare-Medicaid Plan built under a three-way contract between the Centers for Medicare & Medicaid Services, New York State and the plan. Enrollment was voluntary and limited to adults aged 21 and older who were eligible for Office for People With Developmental Disabilities waiver services, dually eligible for Medicare and Medicaid, and living in nine downstate counties: Bronx, Kings, New York, Queens, Richmond, Nassau, Suffolk, Rockland and Westchester.

This guide covers what the plan paid for in addiction treatment while it ran, where prior authorization applied, what a participant was charged, how medication was handled through the pharmacy benefit manager, and what anyone holding that card should do now that coverage has moved back to a separate Medicare plan and New York State Medicaid.
