Rehabs That Take Physicians Health Plan

Physicians Health Plan is a name used by more than one regional health plan, and treatment centers in Indiana, Michigan and North Carolina list it. Coverage can include drug and alcohol treatment, with benefits varying by plan, network and level of care.Listed facilities name Physicians Health Plan on their own websites. Because regional plans with this name are separate companies, check that the plan on your member card is the one the facility accepts. Before admission, call the plan to ask whether the center is in network, which levels of care are covered, whether prior authorization or a referral is needed, and what you may pay.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

Does Physicians Health Plan cover rehab?

Yes. Fully insured HealthChoice plans from Physicians Health Plan of Northern Indiana cover diagnosis and treatment of substance use disorders the same as any other medical or surgical condition, including inpatient care, partial hospitalization, intensive outpatient and outpatient visits. Prior authorization is required for inpatient, partial hospitalization and IOP.

If your member ID card shows your employer's name rather than the PHP logo, the employer is self-funded and benefits come from the employer's own plan document.

Private Insurance Acceptedhttps://rehabseekers.com/insurance-accepted/private-insurance-accepted/
Does Physicians Health Plan cover inpatient rehab?

Yes. Inpatient confinement in a participating hospital or another facility authorized by PHP is covered when ordered by a provider, with a semi-private room as standard and a private room only when medically necessary and arranged in advance. Every inpatient admission needs prior authorization.

Medical Management handles admission review at 260-432-6690 ext. 12 or 1-800-982-6257, using InterQual criteria together with PHP's own medical policy database.

Inpatient Rehabhttps://rehabseekers.com/programs/inpatient/
Does Physicians Health Plan cover residential rehab?

No, residential treatment is not listed as a covered benefit on the 2026 Indiana HealthChoice certificate, and a general exclusion rules out services given in or billed by a residential setting, ranch program or halfway house. Ask the plan about your own group's terms.

Customer service at 1-800-982-6257 ext. 11 can tell you whether a licensed residential substance use program would be payable under your employer's plan.

Physicians Health Plan of Northern Indiana, known as PHP, is a not-for-profit health maintenance organization based in Fort Wayne that sells employer group, level-funded and self-funded coverage across northern Indiana and six counties of northwest Ohio. For anyone arranging addiction treatment, that means the benefit arrives through a workplace plan, the network is regional, and authorization runs through the plan’s own medical management staff rather than an outside behavioral health company.

The name causes trouble. A separate plan in Lansing, Michigan, also called Physicians Health Plan, was renamed University of Michigan Health Plan and ended all coverage on December 31, 2025, with select employer groups running to January 31, 2026. A program in Michigan that lists “Physicians Health Plan” is almost certainly describing that closed plan, which was never related to the Indiana company.

This guide covers what the HealthChoice certificates pay for at each level of care, where the residential exclusion sits, which services need prior authorization, what a member is asked to pay, how addiction medication sits on the 2026 formulary, and how to find a program inside the Freedom or Options networks.
