Common questions

Answered from what is listed here.

What is Positive Healthcare?

Positive Healthcare is the managed care division of AIDS Healthcare Foundation, a Los Angeles nonprofit founded in 1987. It has provided coverage for individuals living with HIV since 1995 and runs two Los Angeles County plans: PHP, a Medicare Advantage HMO SNP, and PHC California, a Medi-Cal plan.

Both plans are open only to members living with HIV or AIDS, and no employer or marketplace products are offered. Member Services answers at (800) 263-0067, TTY 711.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Does Positive Healthcare cover drug rehab?

Yes. On the PHP Medicare plan, outpatient substance use services, partial hospitalization and opioid treatment program services each carry a $0 copay in 2026, and inpatient hospital care is covered with authorization. PHC California members receive drug and alcohol treatment through the Drug Medi-Cal program.

Medi-Cal members are referred to the Los Angeles County Substance Abuse Prevention and Control program rather than treated inside the plan's own network.

Outpatient Rehabhttps://rehabseekers.com/programs/outpatient/
Does Positive Healthcare cover alcohol rehab?

Yes, under both plans. PHP Medicare covers outpatient substance use visits at $0 and alcohol misuse counseling as a free preventive service. For PHC California Medi-Cal members, alcohol treatment is delivered through the Drug Medi-Cal program run by Los Angeles County public health.

Medications for alcohol use disorder, including acamprosate, disulfiram and oral naltrexone, sit on PHP's Tier 1 with no prior authorization.

Positive Healthcare, written in some directories as Positive Health Care, is the managed care division of the AIDS Healthcare Foundation, and it runs two small health plans built only for anyone living with HIV or AIDS. If that is the card in hand, addiction treatment is a covered benefit, but the door into it depends entirely on which of the two plans the card names.

One is PHP (HMO SNP), a Medicare Advantage chronic condition special needs plan with Part D under contract H5852, rated 4 out of 5 stars for 2026 and holding close to 700 members. The other is PHC California, the Medi-Cal managed care plan the foundation created for the state in 1995, with about 770 members in 2024. Both serve Los Angeles County and nowhere else. The Florida Medicare plan closed after December 31, 2023, and the Florida Medicaid HIV specialty contract moved to another carrier in 2018–2019, so a Florida listing naming this plan is describing a contract that has ended.

This guide sets out what each plan pays for at each level of care, where authorization applies, what a 2026 member actually pays, how medication for opioid and alcohol use disorder is handled across two different pharmacy systems, and how to find a program that holds a contract with the right plan.
