Common questions about Methadone Clinics

Answered from what is listed here.

Which North Dakota cities have methadone clinics?

Grand Forks comes first for methadone clinic listings in North Dakota, followed by Minot, Fargo and Bismarck. Opioid treatment programs in the state cluster in and around those cities.

Rural residents often travel to one of these larger cities for care, since Grand Forks, Minot and Fargo are among the biggest population centers. Daily dosing in the early weeks of treatment makes the drive from home a practical factor when choosing a program.

Is methadone treatment covered by North Dakota Medicaid?

Nearly all of the methadone clinics in North Dakota record accepting North Dakota Medicaid. Coverage details can depend on how a person is enrolled, so checking with a program's admissions team before the first visit is sensible.

For Medicaid Expansion members, benefits run through a managed care organization operated by Blue Cross Blue Shield of North Dakota. Expansion members can confirm their enrollment and behavioral health coverage with that plan before dosing begins.

Which methadone clinics in North Dakota are close to a river?

Nearly all of the methadone clinics in North Dakota lie within 25 miles of a river, with a median of about 4.6 miles from listed facilities to the nearest river. The most-named rivers nearby, in order, are the Red Lake River, Des Lacs River and Maple River.

How do methadone clinics work in North Dakota?

A methadone clinic in North Dakota is an opioid treatment program (OTP) that dispenses methadone on site, usually alongside counseling. To do that it needs federal certification, a DEA registration and a state Opioid Treatment Program License. Methadone for opioid use disorder can’t be filled at a regular pharmacy, so doses come from the program’s own dispensing window. In North Dakota, programs are listed in Fargo, Bismarck, Grand Forks and Minot, and much of the rural west is a long drive from any of them.

Every listed North Dakota methadone program sits within 25 miles of a river, a median of 4.6 miles away. The Red Lake, Des Lacs and Maple rivers are among the nearest (see our rivers guide). The closest airports are Grand Forks International, Minot International and Hector International in Fargo, a median of 3.5 miles from the programs (see the airports page).

Programs can arrange guest dosing with another clinic while a patient travels, or a full transfer after a move, but both need advance notice and records sent between programs. Buprenorphine is another medication option. Office-based prescribers can provide it, and some offer it by video, so it doesn’t require daily clinic visits. See Suboxone clinics in North Dakota and telehealth options.

How do you start methadone treatment, and can you begin the same day?

Starting methadone involves three steps:

  1. an intake assessment
  2. a medical exam
  3. a first dose, once the program’s medical staff approve it

Some North Dakota clinics can give the first dose the same day or the next day, but none can promise that before intake. Bring:

  • photo ID
  • insurance or Medicaid card
  • a list of current medications
  • contact details for any prescribers

Dosing windows usually open early in the morning so patients can dose before work. Under the federal rules revised in 2024, admission is based on a diagnosis of opioid use disorder rather than a set length of opioid use.

Pregnant people have priority under federal block grant rules (45 CFR 96.131), and programs receiving that funding admit them ahead of others. North Dakota has no separate state priority rule on record. See rehab for pregnant women in North Dakota.

When can you get take-home doses of methadone in North Dakota?

Take-home doses in North Dakota follow the federal schedule in 42 CFR Part 8, as revised in 2024. Program medical staff decide based on stability and the patient’s ability to store doses safely. Any added conditions under the state’s opioid treatment rules at NDAC 75-09.1-10 also apply.

Time in treatmentFederal maximum for take-home doses
Days 1 to 14Up to 7 days of take-home doses
Days 15 to 30Up to 14 days at a time
Day 31 onwardUp to 28 days at a time

The federal limits are maximums, not automatic steps.

Safety with methadone

Dose changes with methadone are made gradually and supervised by the program’s medical staff. Higher doses can slow breathing, and the risk rises when methadone is combined with alcohol, benzodiazepines or other sedatives. Take-home doses belong in a locked box, out of reach of children and visitors.

Naloxone options in North Dakota:

  • Pharmacists can prescribe it.
  • North Dakota Medicaid members can get it at no cost from enrolled pharmacies under a state standing order.
  • The Behavioral Health Division mails free two-dose kits through hhs.nd.gov/opioids or (701) 328-8920.

Does North Dakota Medicaid, Medicare or private insurance pay for methadone?

North Dakota Medicaid, Medicare Part B and many private plans can pay for methadone treatment, but only if the specific program is enrolled with that plan.

Medicaid. North Dakota expanded Medicaid on January 1, 2014. Blue Cross Blue Shield of North Dakota manages benefits for Medicaid Expansion members. Our Medicaid page has more.

Medicare. Since 2020, Medicare Part B has covered opioid treatment program services, including methadone, counseling and testing (see Medicare rehab in North Dakota).

Private insurance. Private plans may require prior authorization, so call the number on your card first.

Self-pay. Many programs charge self-pay patients a weekly fee. Ask what the fee includes.

Public funding. The Behavioral Health Division uses federal block grant funds for treatment, and the State Opioid Settlement Fund supports treatment awards; see state-funded rehab in North Dakota.

Who licenses and oversees methadone clinics in North Dakota?

The State Opioid Treatment Authority, within the Behavioral Health Division of the North Dakota Department of Health and Human Services, oversees methadone clinics in the state. The same division issues the separate Opioid Treatment Program License under NDAC 75-09.1-10. Licensing a new program starts with a certificate of need approved by that authority.

Federal requirements apply on top of state rules. Each program needs federal certification, which depends on accreditation from an approved body such as CARF or the Joint Commission, plus a DEA registration.
