At first, usually yes. NIDA states individuals starting opioid use disorder treatment generally attend the program every day or almost every day to receive their medication. Take-home doses for days the clinic is closed, including one weekend day and holidays, are allowed for any patient in comprehensive treatment.

When can you get take-home methadone?

Does Medicaid pay for a methadone clinic?

Yes. State Medicaid programs pay opioid treatment programs through bundled daily or weekly rates. Ohio Medicaid pays $18.47 a day, or $129.28 for a two to seven day week, effective January 2024. Louisiana pays one bundled rate covering dosing, counseling, urine drug testing, exams, case management and lab services.

Does Medicare cover methadone clinics?

Yes. Medicare pays enrolled opioid treatment programs a weekly bundle covering the drug, dispensing, counseling, therapy and toxicology testing. CMS lists the 2026 national payment at $277.29 a week, made up of $44.41 for the medication and $232.88 for non-drug services.

A separate add-on code pays for up to seven additional days of take-home methadone billed alongside the weekly bundle.

Can you get methadone from a regular doctor or pharmacy?

No. SAMHSA states methadone for opioid use disorder cannot be prescribed outside a certified opioid treatment program. Methadone prescribed for pain can be filled at a pharmacy, but that is a different use. A program may, however, operate a medication unit inside a pharmacy where state rules and space allow.

Who accredits methadone clinics?

SAMHSA's approved accrediting bodies are CARF International, Social Current (formerly the Council on Accreditation), The Joint Commission, the National Commission on Correctional Health Care, and the Washington State Department of Health. Accreditation is a peer-review process with onsite visits measuring the program against federal treatment standards.

Certification runs for a term of up to three years, and a new program may hold provisional certification for up to one year while accreditation is pending.

Can you be fired for taking methadone?

Generally no. That single rule shapes the whole experience. What Is a Methadone Clinic?

A methadone clinic is a federally certified opioid treatment program that dispenses medication for opioid use disorder on site, alongside medical and behavioral health services built around an individual assessment and a care plan agreed with the patient. Programs may use the FDA-approved opioid use disorder medications: methadone, buprenorphine and buprenorphine combinations, and naltrexone. Methadone there is given only in oral form.

Louisiana Medicaid policy uses “opioid treatment program” and “methadone clinic” for the same provider type, so the two names describe one thing. Every program has a medical director, a physician licensed in the state, responsible for all medical and behavioral health services delivered. Comprehensive treatment, in the federal wording, means continued medication combined with harm reduction, medical, behavioral health and recovery support services.

Federal rules were rewritten on February 2, 2024, the first substantial update in more than twenty years, with the revised treatment standards taking effect that April. The regulation was renamed Medications for the Treatment of Opioid Use Disorder, the requirement that an adult have a one-year history of opioid use was removed, and the rule that anyone under 18 show two unsuccessful treatment attempts was dropped. Counseling was also unlinked from medication, so declining counseling cannot be grounds for withholding a dose.

MOUD or MAT? Federal guidance now prefers “medications for opioid use disorder” over “medication-assisted treatment”, on the grounds that the older term implies the medication is secondary to counseling. You will still see both on clinic paperwork and on Medication Assisted Treatment (MAT) listings.

What Happens at a Methadone Clinic?

Medication can start after that screening alone. A full in-person physical, including serology and other clinically appropriate tests, follows within 14 calendar days of admission.

Before that dose the practitioner explains how the dose will be raised, how to transport and store medication safely, and the value of keeping naloxone on hand. If a screening was done outside the program, it has to have been completed no more than seven days before admission.

The Care Plan and Counseling

The care plan records the patient’s own goals plus harm reduction steps, education and employment aims, and medical, psychiatric, economic, legal and housing needs, with how often each service will be delivered, and it is reviewed as circumstances change. Counseling must be clinically appropriate, trauma-informed and culturally responsive; cognitive behavioral techniques and motivational interviewing are both named in federal guidance.

Drug Testing and Health Monitoring

Federal rules require no fewer than eight random drug tests per patient per year, using tests with FDA marketing authorization, and they expressly allow programs to hand out legal harm reduction supplies so a person can test their own supply for adulterants. A periodic physical should happen at least yearly. Programs must also counsel on preventing HIV, viral hepatitis and sexually transmitted infections, and treat or refer anyone who tests positive, which is why HIV and hepatitis care turns up so often at these clinics.

Do You Have to Go to a Methadone Clinic Every Day?

At the start, usually yes. NIDA states that individuals beginning opioid use disorder treatment generally attend the program every day or almost every day to receive medication. Any patient in comprehensive treatment can take home doses for days the clinic is closed, including one weekend day and state and federal holidays, whatever their time in treatment.

Beyond that, the 2024 rule made the 2020 take-home flexibilities permanent. In days 0 to 14, up to 7 unsupervised take-home doses may be provided. From day 15, the supply may reach 14 days. From day 31, it may reach 28 days. Each step is at the practitioner’s discretion, and state rules may be stricter than the federal ceiling.

Who Decides on Take-Home Doses?

An appropriately licensed practitioner or the medical director decides and documents the reasoning. The factors are the absence of active substance use or conditions raising risk, regularity of attendance, absence of serious behavioral problems, absence of known recent diversion, the ability to transport and store doses safely, and any other safety criterion the medical director considers relevant. Rigid time-in-treatment thresholds and strict reliance on toxicology results were dropped in favor of shared decision making and overdose prevention.

Doses go home in child-resistant packaging labeled with the program’s name, address and telephone number, and the education on safe storage is recorded in the chart. Where take-homes fall outside the federal limits, or where a designated person or a facility such as a nursing home collects medication, the program files an exception request on Form SMA-168 through the federal extranet. Every program also keeps a diversion control plan naming who carries out each safeguard.

How Much Does a Methadone Clinic Cost?

Self-pay figures published by operators run roughly $80 to $150 per week. One national operator estimates about $126 per week, or $6,552 a year, for daily visits with counseling; another clinic lists methadone maintenance at $90.00 per week. A 2017 report quoted patients paying between $350 per month and $200 per week. The medication itself is normally bundled into the clinic fee rather than billed separately.

Medicare and Medicaid

Medicare pays enrolled programs a weekly bundle; CMS lists the 2026 national payment at $277.29, made up of $44.41 for the drug and $232.88 for non-drug services, with a separate add-on for up to seven extra take-home days. Medicaid rates are set by each state: Ohio pays $18.47 per day, $129.28 for a two to seven day week and $517.12 for four weeks as of January 1, 2024. Louisiana pays one bundled rate covering dosing, counseling, urine drug testing, physical exams, evaluation visits, case management and lab services. California pays narcotic treatment programs a uniform statewide daily rate on each day a dose is administered. Oregon added a separate code for the medication at $0.18 per 5 mg dose from July 1, 2025. If you have no coverage, ask about a sliding-fee scale tied to income, and about grants, scholarships, charity care and payment plans, including whether anything has to be repaid if treatment stops early.

Directory note. Nearly every methadone clinic records self pay, so a weekly or monthly rate is usually quotable over the phone, and state-funded status is recorded at a majority of them, with sliding scale MAT at a smaller portion.

How Do I Know a Methadone Clinic Is Legitimate?

A lawful program holds four things at once: certification from the federal opioid treatment authority, accreditation from an approved body, a state license, and a DEA registration. Accreditation is a peer-review process with onsite visits measuring the program against federal opioid treatment standards. Certification runs for a term not exceeding three years.

The approved accrediting bodies are CARF International, Social Current (formerly the Council on Accreditation), The Joint Commission, the National Commission on Correctional Health Care, and the Washington State Department of Health. A newly opened program that has applied for accreditation may hold provisional certification for up to a year, granted only after state and DEA approvals; a program renewing with a one-year accreditation may hold conditional certification, which lapses if standard accreditation is not obtained.

What Rights Do Patients Have?

Records are protected by the federal substance use disorder confidentiality rule and HIPAA; the 2024 Part 2 rule has been in effect since April 16, 2024, with compliance required by February 16, 2026, and it allows a single consent covering future disclosures for treatment, payment and operations. Treatment records cannot be used to investigate or prosecute the patient without written consent or a court order.

The Department of Justice states the Americans with Disabilities Act protects individuals taking legally prescribed opioid use disorder medication under a licensed professional’s supervision, including at work: an employer generally cannot dismiss someone for a positive test on medication taken as prescribed unless they cannot do the job safely or another federal law disqualifies them. A medication unit is a fixed or mobile site, geographically separate from the program, where licensed practitioners or community pharmacists may dispense medication, collect drug test samples or provide other services. Mobile units are extensions of the program able to deliver any service where space and privacy allow, and they cut travel distances, which a 2026 study identifies as a real barrier to methadone treatment given near-daily attendance.

What Is the Difference Between a Methadone Clinic and a Suboxone Prescriber?

Methadone for opioid use disorder is available only at a certified program, with supervised dosing at the start and take-homes released over time. Buprenorphine, the active medication in Suboxone, can be prescribed by many physicians, nurse practitioners and physician assistants and filled at a pharmacy, without attending a special clinic. Certified programs may offer buprenorphine and naltrexone too, so one clinic can hold all three options.

Switching between medications is a clinical conversation weighing medical, psychiatric and substance use history, personal preference and what is available locally.
