Highmark plans may pay for the clinical care at a luxury program in Pennsylvania, such as detox, residential treatment, therapy and medication, if the stay meets the plan’s medical necessity rules. You pay the deductible, any coinsurance and every extra the plan does not cover, so the bill splits into two parts.

Highmark Behavioral Health Services, the company’s own unit, handles utilization management and case management for mental health and substance use treatment in Highmark programs. Some employer groups buy behavioral coverage from another company, so benefits differ from one member to the next.

Does Highmark require prior authorization for inpatient rehab in Pennsylvania?

Yes, Highmark requires authorization for all inpatient behavioral health services in Pennsylvania, including inpatient detox and inpatient substance use rehabilitation, and requests go through Availity or by phone. County authorities

Each Pennsylvania county has a Single County Authority that directs publicly funded treatment. A private stay is arranged with the program’s own admissions team instead.

Single case agreements

If a program is out of network, its admissions team can ask Highmark for a single case agreement, which treats one stay at in-network benefit levels. A member can also ask a Highmark case manager whether one is possible.

Good faith estimates

A self-pay client can ask a Pennsylvania program for a good faith estimate, which federal rules require when the stay is booked at least three business days ahead. If the final bill lands $400 or more above it, the client can dispute the difference.

Before booking: tell Highmark the level of care, tell the program the plan name on your card, and keep both replies in writing. Authorization rules changed for outpatient levels in March 2026, so check the date on anything you were told earlier.
