What is a sober living home and how is it different from rehab?

A sober living home is alcohol- and drug-free shared housing, and in Tennessee, as elsewhere, it is not treatment. Residents pay rent, share chores, follow house rules and usually get counseling or attend meetings somewhere else.

Residential treatment is a licensed clinical program with medical and counseling staff. For those programs, see residential rehab in Tennessee. Correctional halfway houses are different again: they are placements for people leaving prison or living under supervision.

Tennessee’s Office of Licensure also issues a license called Alcohol and Drug Halfway House Treatment Facility. A Tennessee program called a halfway house may therefore be a licensed treatment setting rather than a sober home. Many residents combine a home with outpatient rehab or an intensive outpatient program.

From January 1, 2026, Tennessee certifies recovery residences through outside organizations the state selects, so ask any home which group certified it. Get the house rules, testing policy, deposit and refund terms in writing, and ask how the home handles medication for opioid use disorder.

Are sober living homes in Tennessee certified or regulated?

Tennessee certifies recovery residences through outside organizations chosen by the state. Under Tenn. Code Ann. 33-2-1401 to 33-2-1404, the Tennessee Department of Mental Health and Substance Abuse Services selects Recovery Residence Certification Organizations for three-year terms, starting January 1, 2026. The named organizations include:

Certification comes from these state-level groups, not from NARR directly. To look up a home, ask which organization certified it. Then search for the home with that organization or on the department’s recovery housing page. If a court or agency is referring you, ask whether the referral has to go to a certified home.

What do the NARR levels of support mean?

NARR describes four levels of support, from peer-run houses to housing inside a treatment program. The level tells you how the home is structured. It is not a ranking of quality.

How much does sober living cost in Tennessee, and who pays?

Residents usually pay the rent in a Tennessee sober home themselves. No state source sets a typical price. Ask for every cost in writing, including:

rent and the deposit
drug testing fees
what is refunded if you leave early

Plan on paying rent yourself unless your TennCare plan or another payer tells you in writing that it covers housing. Health coverage generally applies to treatment, not rent (see Medicaid options).

What should you ask before moving into a sober home?

Before you sign a lease at a Tennessee sober home, get the house rules in writing. Useful questions to ask:

How often is drug testing done, and who pays for it?
What are the curfew and guest rules?
Are meetings or work required?
How long do residents usually stay?
What is the refund policy?
What exactly happens after a return to use?

Some homes ask a resident to leave after any use. If so, ask whether staff will help arrange a safe next step, such as detox or a treatment bed, rather than a same-day exit with nowhere to go.

Can you live in a sober home while taking medication for opioid use disorder?

Policies on buprenorphine, methadone and naltrexone differ from home to home. Ask directly, get the answer in writing, and ask how medication is stored. TennCare’s BESMART network lists buprenorphine providers. Our Suboxone clinics and methadone clinics pages show options by city.

Naloxone, safety and resident rights

Ask whether the house keeps naloxone and where it is stored. After time without opioids, tolerance drops. A return to use then carries a higher overdose risk, especially just after leaving a home.

Many Tennessee pharmacies dispense naloxone without a prescription.
TennCare covers up to two naloxone products a month, with a copay of no more than $5.
Tennessee’s Good Samaritan law offers some legal protection to people who call for help during an overdose.

Federal fair housing law can protect people in recovery from housing discrimination. It generally does not cover current illegal drug use.
