Unprofessional & dishonest staff. Request not to have Tracy or Allison as clinicians. Tracy closes services w/o informing the client & puts in referrals for other services, w/o first discussing so or making… recommendations to the client. I signed up for services with Family Continuity. My daughter & I were assigned Tracy for IHT & Allison as a therapeutic mentor, TM, for my daughter. After some time, Tracy began canceling appts & wasn't returning my calls/texts. I informed Allison, & was told she would reach out to Tracy & have her contact me. This never happened. A few weeks pass, when I'm contacted by management. Management states if my daughter wanted to continue seeing Allison, other services needed to be in place. (I was confused, bc as far as I knew, we had other services, Tracy was just MIA). A week or so later, I'm contacted by a clinician to set up services for my daughter. I state "I never requested these services, but that my name should be on the wait list. (Allison recommended I see an individual clinician at FC so herself, Tracy, & the clinician could communicate w/ eachother, to which i agreed). This employee told management that I denied services, services that I never requested, nor were they ever recommended. Again, I spoke with management, & I'm told my name is not on the wait list, yet Allison told me more than once she put me on it. Even with all this, I continued services. I spoke with management about putting other services in place, since Tracy stopped showing & requested a copy of the discharge summary, since i was informed that Tracy closed services. Should this not have been discussed with the client first? I am baffled by the things Tracy wrote on the summary. Tracy states "mother did not agree w/ treatment goals & would not participate in services." Really Tracy? I have ALL of our texts from 9/2020. Texts you didn't respond to, texts of me reaching out to you bc you didn't call for our scheduled appts, texts about canceling "bc you were exposed to covid," texts about canceling bc "your car was totaled by a deer." Do you not recall these? Also, I'd like to know what treatment goals I didnt agree with, since treatment goals are created by the client, not the clinician?!? I created goals I didn't agree to 🤔 ? Tracy states "person did not return/was non-responsive to outreach attempts." When? Show me texts I didn't respond to, calls I didn't pick up/return? Oct. 14, 2021 we scheduled a phone appt (bc Oct.13, you texted, saying you were in an accident & your car was totaled bc you hit a deer). Do you not remember this? I can resend you the texts if you'd like? It really isn't professional for a master's level clinician to stop showing up for appts, yet blame the client for not showing, when the appts were in my own home!! Tracy states "family was informed regarding no show policy & case closing details, family did not respond to notification." Tell me Tracy, what notification did I not respond to?? Show me the proof bc you're a LIAR! Tracy states "client was mailed a copy of transition/discharge summary." How convenient, since there is no way to prove you didn't mail it. Wouldn't Christopher have questioned why I requested one if you "mailed" one? I think so. Tracy states "outpatient referral for child has been made, waiting for acceptance from parent." Why are making referrals that were NEVER previously discussed/recommended? Tracy states "last contact with client 11/3/21," yet in a different section, states, "last contact 9/23/21." Which one is it Tracy, 11/3/21 or 9/23/21 ? As if all of this wasn't unprofessional enough, both Tracy & Allison also spoke with outside personnel, without a signed release. This is a violation of HIPPA, and I will be contacting the licensing board, as well as the office of human rights.Management at Family Continuity seems very professional & respectful. I spoke with Christopher, & he agrees both Tracy & Allison did not follow protocol & is looking into matters. Brendan is also very professional & helpful. Sent from Yahoo Mail on Android
