About Cottonwood Pathways – Farmington
Cottonwood Pathways operates from 653 West Arrington Street in Farmington. The site sits in San Juan County, which shapes which public funding routes apply. Nearby communities include Crouch Mesa, Napi Headquarters and Shiprock. Distance matters here, and the nearest option is not always the one that fits.
Intensive outpatient and outpatient care are delivered here, alongside care for co-occurring mental health conditions. Which level fits depends on the assessment rather than on what is available, so the useful first question is what that assessment involves. Ask what the assessment involves before you commit.

Treatment Services at Cottonwood Pathways
Intensive outpatient and outpatient care run to a set weekly schedule, with evenings and weekends spent at home. That suits people holding down work or caring responsibilities, though it asks more of someone managing early recovery alone. Ask what the assessment involves before you commit.
Trauma-specific services run alongside, which matters where the two are tangled together. Co-occurring mental health conditions are treated in the same building rather than split across providers. Assessment decides the level of care, so ask what that involves and how quickly it can happen.

Cottonwood Pathways Address, Location and Contact Information
The address is 653 West Arrington Street, Farmington, NM 87401, in San Juan County. The catchment reaches Crouch Mesa, Napi Headquarters and Shiprock. Confirm what a program covers before traveling in, since the level of care listed is not always the level with capacity.
Ask what a first appointment involves and how long the current wait runs before making the trip. Opening hours shift with staffing, and an evening slot one month may not exist the next. Waiting times vary with capacity, so call ahead rather than arriving unannounced.

Cottonwood Pathways Reviews and Ratings
Patients remember things about Cottonwood Pathways that no register holds: who answered the phone, how the first day went, whether family were included.
If you have that kind of knowledge, a review in this section passes it on.
This place doesn't help if your self referred for mental health. They will give you the run around and schedule you months out. You have to be addicted to a substance to be seen or heard here. Wouldn't recommend
Everyone was dreading going here, because of their reputation, and past issues no one was looking forward to being managed by cottonwood. But then the group got Shianne as a counselor and Darryl as peer support, and… each day got better and better. Everyone together is making progresss and moving forward. We didn’t feel like we got a jaded counselor that was reading from a book. Or someone that had a cookie cutter approach to treatment. Things looked positive, acupuncture was provided along with other helpful tools. That’s why it was so disappointing when “whoever is in charge” couldn’t care less about how we feel. Every week for the past couple Months someone else has quit or is no longer there. Acupuncture gone, this person gone, that person gone, The team we started with is 1/3 gone now as well most sent back to prison because they didn’t stay sober. How sad that NM is still sending people to prison because of a substance abuse problem or addiction. What does that say about your program cottonwood??? That it’s ineffective, doesn’t work, it’s a waste of time and money for at least 59% of the people ordered to go there. This is a failure from the top. You can’t keep staff, you’re inconsistent. Oh and today some guy waltz’s in to let us know the coffee pot is no longer allowed. Even for the MAT group who all dose and take methadone before class. While we are told cottonwood is the only one that has a group for MAT lol…well you treat it just like all your other groups so you’re doing nothing special. Except now you want to treat us like we’re idiots who don’t even deserve coffee, no snacks. “No eating or drinking unless it’s a clear water bottle or you buy it from our vending machine”. How disappointing you were so close to doing an incredible job and you messed it all up and made it all about the numbers and failed miserably. Everyone let whoever made you go here know how cottonwood made you feel. Shianne now is all we have left. Darryle no longer works here. Hopefully you treat her well:
The staff and counselors are amazing there not just textbook counselors they actually know what its like to live that life and how to turn your life around. They've inspired me so much that I've chosen to be a substance… abuse counselor myself and to try and give the help back that was given to me. I would highly recommend checking them out if you want recovery or help turning your life around. Thanks Cottonwood
Horrible staffing throughout all locations wouldn’t recommend at all!
Every staff member I have encountered has been so amazing and kind. I was given a chance to turn my life around. When you want it, things start to happen for the better. I am self referred and while the sobriety life… has its challenges, I thank Cottonwood for being apart of my journey to sobriety. Thank you Cottonwood from the front staff to the Counseling, ERS, UA area, and Group sessions, for making this place feel welcoming. I was scared and nervous in the beginning. I knew I needed help. I didn't feel judged and that was important to me. Thank you for help saving my life that I never knew I needed.
The staff is great and very friendly, especially if you have Cliff to talk to about your substance use, just be honest and communicate with the staff, they’ll be helpful as they can
Self-referred individual who was looking for mental health counseling. VERY DISAPPOINTING services at Cottonwood. Took 2 hours of paper questionnaires just to get an appointment over a month away. First meeting was… another 2 hours of repetitive substance use focused questioning. Second appointment I was locked out of the building and had to wait over 10 minutes past my scheduled appointment that lasted only a half hour and was uninformative. Yet, double standard applied when I was late and even called ahead to advise it, was denied and told to reschedule...though when I arrived, I could see my "counselor" outside doing nothing. She never answered or returned calls and responded to most everything with sarcasm. No compassion. No follow up when I disappeared. After 3 visits, over two months, and nearly 5 hours invested into the place, still hadn't received any mental health counseling. Apparently, they're only interested in substance use services. Total waste of time, energy, and hopefulness. Shameful.
Yeah stay away from this place entirely. I had to jump through hoops just to get ahold of my therapist in order to switch to a different one. She's never there and never returns my phone calls. I have physically drove… to their facility and each time she is never there. Or when I do have an appointment she cancels last minute abruptly and I have to wait for weeks on end to get seen. Today I threw my hands up entirely and decided to search elsewhere, Nevermind that I came to them while going through a full on crisis and am still going through it.
I love how much I've learned from the staff and tools they give me! Thanks guys!
They are nice and polite . paying attention and answer yr questions. They are doing their job! Good job! Keep it up! I love you!
Friendly and knowledgeable staff. Do a lot of court mandated counseling and substance abuse help. Like for DUIs, they have sober support groups. I found the counseling goal oriented, educational, and productive.
They charge 10 bucks to use a clean restroom for you to pee in. Friendly staff...so friendly they follow you in. Lol
