No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

What is Florida Complete Care?

Florida Complete Care (FC2) is a Miami-based Medicare Advantage special needs plan under contract H9986, owned and operated by Independent Living Systems. It serves Medicare members who live in a nursing home, need nursing-home level care at home or in assisted living, or are dual eligible with Florida Medicaid.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Is Florida Complete Care the same as Florida Community Care?

No. Both plans belong to Independent Living Systems, but Florida Community Care is the Florida Medicaid managed care plan, covering managed medical assistance and long-term care, while Florida Complete Care is the Medicare Advantage special needs plan with Part D drug coverage.

A dual eligible member on the Duals VIP plan may hold Medicare through Florida Complete Care and Medicaid through a plan such as Florida Community Care.

Florida Complete Care, written FC2 on much of its paperwork, is a Medicare Advantage special needs plan sold only in Florida under Medicare contract H9986 and run by Independent Living Systems, a privately owned Miami company founded in 2001. Addiction treatment is paid for under Medicare Advantage rules here, inside a plan designed around nursing-home level of care and dual eligibility rather than around a broad commercial network.

Three products sit under that contract. Florida Complete Care (HMO I-SNP, plan 001) enrolls Medicare members living long term in a nursing home or skilled nursing facility. Florida Complete Care In The Community (HMO-POS I-SNP, plan 002) enrolls members living at home or in assisted living who meet nursing-home level of care on Florida’s 701B or 701T assessment. Florida Complete Care Duals VIP (HMO-POS D-SNP, plan 004) enrolls full benefit dual eligibles, QMB Plus and SLMB Plus members, and holds a Florida Medicaid contract alongside its Medicare one.

The I-SNP service area runs to 22 Florida counties, benefits differ across three service-area segments, and the 2027 listing for Duals VIP shows a narrower 11-county footprint. This guide sets out which levels of addiction care are covered, where prior authorization applies, what each plan’s cost share looks like for 2026 and 2027, how medication is handled, and how to work out whether a program is in network.
