Prior authorization and medical necessity review sit with the carrier that underwrites the coverage: Humana on a Honor plan, Devoted Health on its own plans, or the TRICARE regional contractor for military coverage. Humana’s authorization rules for substance use levels of care on the Honor plans were not published in the sources reviewed here, so the useful step is a call to the number on the card asking three things: whether detox, residential, partial hospitalization or intensive outpatient care needs authorization, how continued stay is reviewed, and what the appeal route is if a request is refused. Medicare Advantage denials carry a defined appeals process with expedited review where delay would jeopardize health.

How Does a Service Member or Veteran Get Rehab Covered?

Through TRICARE or VA, not through USAA. TRICARE covers medically necessary substance use treatment including emergency and non-emergency inpatient care, detoxification, partial hospitalization, intensive outpatient programs, medication-assisted treatment, office-based opioid treatment and opioid treatment programs. Aversion therapy and treatments TRICARE does not recognize are excluded, and some services carry special rules, which the regional contractor explains.

In crisis. The Veterans Crisis Line is reached by dialing 988 and pressing 1, around the clock. It is listed on Humana’s veterans pages and is open to Veterans, service members and families whether or not a Medicare Advantage plan is in force.

A Medicare Advantage plan does not replace VA coverage. Humana describes the Honor plans as complementing it, so a Veteran enrolled in VA community care keeps that route open and can use the Honor network for civilian care instead.

How Much Does Rehab Cost With USAA?

On a Humana USAA Honor Giveback plan the premium is $0 with a Part B giveback, the deductible on H5216-278-1 is $0, and the in-network maximum out of pocket on that plan is $4,700 for 2026. Inpatient psychiatric care runs $375 a day for the first seven days, in-network therapy is $0, and out-of-network care is 50% coinsurance.

The figures move by state and plan year. For 2027, the Arizona plan H5216-436-1 shows a $5,150 in-network maximum out of pocket with inpatient psychiatric care at $340 a day for days 1 to 6; the Nevada plan H5216-216-0 shows $6,500, $330 a day for days 1 to 5 and 40% out of network; the Idaho plan H5216-427-4 shows $9,150, $450 a day for days 1 to 4 and 35% out of network. A USAA Life Medicare Supplement works differently: it pays Original Medicare copays, coinsurance and deductibles at any provider that accepts Medicare, with an early-enrollment discount of up to 50% at age 65 declining with age, and a household discount of up to 5% for Texas policyholders.

Those are the sums a program bills without coverage. Under a Honor PPO the number that matters for a long admission is the in-network maximum out of pocket, because daily charges stop counting once it is reached, and the gap between a $4,700 cap and a $9,150 cap is the difference between two plans sold under the same name.

Does USAA Cover Suboxone, Methadone and Other Addiction Medication?

USAA publishes no formulary, because it does not underwrite drug coverage. Humana states that Honor plans cover opioid treatment for in-person visits. The Honor Giveback PPOs listed above carry no Part D, so pharmacy coverage comes from the Honor Giveback with Rx version or a separate prescription drug plan, and formulary placement is checked on that plan’s drug list.

Where the Prescription Is Filled

Medication given during an inpatient stay or inside an opioid treatment program is paid as part of that service, which is why it never shows as a pharmacy fill. A buprenorphine or naltrexone prescription collected afterwards falls under Part D, and the tier and any step requirements were not published for the Honor with Rx plan, so check the drug list before discharge rather than at the counter.

Medicare Supplement and TRICARE

A USAA Life Medicare Supplement adds nothing Medicare excludes; it pays the gaps on Medicare-covered care, including Medicare MAT delivered through licensed opioid treatment programs. TRICARE separately covers medication-assisted treatment, office-based opioid treatment and opioid treatment programs for members with military coverage.

Which Rehab Centers Are In-Network With USAA?

There is no USAA network, because USAA contracts no providers. A program is in network when it holds a contract with the carrier behind the plan, which means Humana’s network for the specific Honor plan, Devoted Health’s network, or TRICARE authorization for military coverage. That status is settled in the carrier’s provider search and by the plan name on the member card.

An admissions team that says it accepts USAA is usually describing a Medicare Advantage plan sold through USAA, or confusing the brand with TRICARE or VA benefits. The question that resolves it is simple: which carrier will you bill, and is this program in that carrier’s network for my plan number. Humana customer care staff received training from USAA specifically for questions from members holding veteran benefits.

Is USAA the Same as TRICARE or Humana?

No to both. TRICARE is the Department of Defense health program, and USAA is a private reciprocal inter-insurance exchange founded in 1922 with no government endorsement; its Medicare Supplement products are not connected with the federal Medicare program. Humana is the insurer behind the co-branded Honor plans, an alliance that dates to 2005 and that put the USAA name on every veteran-focused Honor plan from the 2024 plan year.

Before you call a program: have the health insurance card in hand, know the carrier and plan number printed on it, and ask the admissions team whether that plan is one it contracts with. Those three answers settle cost and authorization before anyone discusses admission dates.
