My girlfriend currently goes here, and they have helped her tremendously. We are so grateful for the staff, nurses, and for her beautiful therapist, Angie. Thank you for never judging her and always wanting the best for… what she’s doing in life. This has been a huge change from where she was going before. These people at this clinic, actually care about their patients and you’re in and out in 15 minutes. They don’t make you wait around in a queue and have to wait hours just to get your medicine. If you’re looking for help and need a reputable and good place to get your medication from, I highly recommend Clean Slate.

I’ve been a patient at this location for many years now. Clean Slate has seen me through many trials and tribulations. If you have an addictive personality like I do and want to get a handle on it and your life,… schedule an appointment. You won’t be disappointed if you follow through with the instructions the provider gives. I had to remove a star because they don’t have enough locations and I have to drive like an hour and a half to reach the closest center.

I am so lucky to have found this place. From the women at the front desk, the nurses, Dr. Tucker, and my counselor, Angie, I couldnt be more lucky to have them caring me. Even while experiencing some side effects from a… med, the Dr was available to speak to immediately over the phone. Theyre always helpful and want us to succeed. Couldn't recommend this place more. They all deserve raises.

I am so grateful for Clean Slate! I really needed to find a new Suboxone clinic and they got me an appointment immediately, two days after I called to become a patient. Staff is super friendly, Velma Is wonderful and… she has helped me tremendously and talking with her is so easy. You don’t feel judged whatsoever and you can tell that they actually care about you. I used to go to CMS and that was a whole different dynamic. Clean Slate is a doctor’s office, it’s not like going into a methadone/Suboxone clinic and they don’t make you wait hours to get dosed. They fill your prescription at the pharmacy so an appointment with them is like 10 minutes tops out of your day once you’re an established patient. So grateful that my friend referred me to this place. I’m coming up on 10 months clean, if I can do it, so can you. If you’re looking for help or want to change your life and get sober, look no further because this place is amazing and 10/10 recommend.

I'm cash pay. Pay everytime I come in and have some lady harassing me saying I owe money. I pay every appointment. Always chasing money its gross.......

I first went there for my initial appointment and after I told them my whole life story and that I used alcohol occasionally I was told that they could help me. Then after that appointment I did a virtual meeting… followed up by another appointment a couple of months later when my prescription from the previous place I was going to had expired. I was required to leave a urine sample at this appointment and then after talking to the nurse practitioner she told me that alcohol was not tolerated at all. She only gave me 1 weeks supply of the Suboxone I was taking for 5 years which left me 1 week to find another place to help me. This place (or person) was not there to help me. They wasted my time is all they did. I'm glad I didn't go there for help 5 years ago when I started Suboxone or I would of been dead by now

Everything has been great, my doc listens and makes it easy to talk to and go through the process! But it seems that there blood tests and urine tests are not always correct! My husband had his blood drawn there and… they told him he has hepatitis C, which is BS, we had his blood drawn again but at Sonora Quest and they said he’s 100% hepatitis C free , there’s no way he could have it! Then they tell me my last urine test was Positive of having the metabolisms of fentanyl, which absolutely impossible and wrong cause I don’t do fentanyl I didn’t do any drug! I’ve been going for 6 months always a clean urine test and all of a sudden they say this and l had two test right after that and both are negative so they are very wrong I will continue to fight that cause now that sets ne back with them and it’s not fare and now they are gonna think I’m lying and used which is incorrect! I’m so upset over this!

Amazing staff and treatment. Not only do they help with therapy but other treatments as well.

Its great. Everyone there are so nice and repectful. My Dr. Truly wants to help me and thats a rare thing these days.

The people there come across like they don't judge you for being there. They are decent and respectful ladies that help you by prescribing medicine. That way you can stop your dependency on drugs by replacing them with… medicine (like Suboxone). They also have a counselor available to talk to there at the office. It's a place that doesn't make you feel like your in a trap. They are there for you if you want help.

Very understanding staff, they make it clear that they are there to help fix my issues. My transition has been smooth and comfortable. Thank you CleanSlate. I glad I found you all.

No on two different occasions I had to come to them or last minute visit the first time I was a new patient this time. I am a returning patient after my doctors office closed permanently, but I will say they are… incredibly efficient at that office. It’s probably the only doctors office I’ve ever been to where you are going to see the doctor at the time of your appointment every time in my opinion that’s a pretty big deal. Our time is valuable just like there is and they actually respect that.. They are great about sending the medicine almost immediately. It’s just a very smooth process as long as you are there for the right reason.

I have been clean for almost 6 years and I couldn't have done it without Suboxone. The whole staff at the Gilbert location are so caring and nice. They always get me in and out as fast as they can.

I had the worst experince at this unrealible facility. I made an appoitment and they told me to call before hand to make sure the provider is in. So i called at 8 and they told me she was in. Only to drive an hour to… get there they take my picture hand me a clip board with like 20 pages to fill out get my information and then tell me sorry the provider isnt in. Waste of time money and gas! Just thought id share my awful experince so it doesnt happen to anyone else or to see if it has happened to someone else

Absolutely everyone, from the front desk staff to case managers & therapists, are the BEST. Problem solvers, very friendly & professional, its been an excellent experience. We've been to other nearby clinics & this one… is by far so much more efficient & friendly. Thanks ladies I greatly appreciate you all..
