Harbor – Toledo (123 22nd Street) Address, Location and Contact Information
123 22nd Street, Toledo, Ohio, 43604 is the address held. This address falls in Lucas County. People travel in from the surrounding area, and confirming the route beforehand saves difficulty. Public transport, parking and the length of the journey all affect whether someone completes a course of treatment, and they are worth settling before the first appointment rather than after.
A quick call before you go will settle directions, parking and what to expect first time. Remote appointments may be possible where travel is a barrier. Reception hours vary, so a call that goes unanswered once is worth repeating rather than treating as a dead end.

It is a good place for my brothers love this place❤️❤️❤️
Don't bother going here for an autism assessment. Their diagnostic criteria is out of date. They will gaslight you and tell you that you have PTSD and "a borderline low IQ". They will also tell you your autism symptoms… are "just anxiety". Go to a place that actually assesses autism and has the lastest up to date information on it. Edit: I was diagnosed with high functioning autism on February 24th, 2026 by a psychiatrist who was with me more than two minutes. Center For Solutions In Brief Therapy us where I was diagnosed. Shame on you harbor for failing autistic women in such a monumental way.
Very disappointed in our experience with Harbor. I chose to bring my kids here for testing and medication management due to the fact that they are well established in the area. I was never informed that though I would… be going into the office, the entire process would be through telehealth. Truthfully, I would've likely chosen a different provider had I known this. It seems very impersonal, and just not the experience I was looking for. But, we stuck with it. I definitely thought there was a lack of communication or understanding in staff when I'd have questions, and phone calls were not always returned. Our last appointment, I took time off work (as I have a full time job), pulled my child out of school, drove the 30 minutes one-way to the office for our appointment that we had been waiting months for to get back on medication, just to be informed that they scheduled the wrong amount of time, and we would need to reschedule again for another couple of months. They seemed unapologetic and unbothered by this, as if it's a regular occurrence, and once I looked at the reviews, I saw why - because this is a systemic issue they are not addressing. I called to file a complaint after realizing this. I informed the operator that I had a complaint, and I left a voicemail with the woman in charge of quality. It's now been over a month, and to this day my phone call still has not been returned. At first I thought perhaps they were out sick, but this is inexcusable. I know that there are some amazing individuals at Harbor, but there are clearly issues with the culture here being indifferent to client concerns, starting at the phone operators/schedulers, to the nurse practitioners, all the way up to the quality department. We will not be returning.
Couldn't get an appointment for 2 months out! Mental Health should be readily available 24/7. Called and was told someone will call me back within 1 day, they called me back, told them My daughter needs to be seen ASAP,… told me to leave a message and someone would call me back in 1 day. Going on day 3!! I even told them she's talking about hurting herself!!! Mental Health Should be Available 24/7!! NOT 2 MONTHS LATER!!! WHAT IS WRONG WITH THIS CIRCUS!!??? THIS PLACE NEEDS TO BE LOOKED INTO!! WHY ARE OUR APPOINTMENTS BEING CANCELED AND IF U WANT AN APPOINTMENT YOU HAVE TO WAIT 1-3 MONTHS!!?? WHAT A JOKE!!!!
Absolutely horrible. If you cancel or miss so many appointments they will discharge you but they cancel appointments all the time. If you want your child to get help, this is not the place to go. They assigned John… Davis to my son and he saw him once and something always comes up and he has to cancel the appointment. Katina was wonderful but her skills are being wasted here and deserves a much better place to work. If you're not going to help or children then why have the doors open? Completely unacceptable! Thank you for showing NW ohio that you can't be trusted to treat children with mental health. Been with them almost a year and nothing has changed. You have canceled your last appointment on my child.
Their staff are friendly, caring, helpful, children can go here and get the help they need. John has been very helpful and kind to me and my son, he seems to be concern on what going on, we need more men as role models… for children.
They make you wait months and months only to tell you they can't test you. Don't seek help through them you just give them money and never get answers.
This is nothing more than a mental med. Dispensary! My last 2 appointments were canceled and they seem to have the need to charge me. I stopped all my meds they had me on. Im so much better than being under Harbors… care. They almost ruined my life. No Therapist available for 3 months out.. Now they're advertising on a local tv channel. Seriously?
Good luck getting testing, they'll forget to schedule the appointment they told you that you'll have next month and you won't be told until you come in and tell them you're ready for it.
This place is awful. My daughter was coming for weekly visits then out of no where her therapist couldn’t see her for 6 weeks. Then my daughter got sick and had to miss her appointment so we had to wait another 6 weeks.… They said this was because another therapist left so ours got their caseload and I guess those patients were more important. I tried to get her in to see another therapist and unless my 5th grader could miss class every week for an appointment they wouldn’t be able to see her weekly anymore. I also asked for help in getting an iep and they were beyond useless in guiding us or responding to the school. Don’t bother starting any therapy here. They have no idea what they’re doing.
smells horrible and doesn’t keep up with appointments. canceled my appointment then wouldn’t give me my meds. and then gave me a 500 dollar bill after saying everything should be covered.
Last year we were in desperate need of help for our son. Not only did Harbor get us connected to a wonderful counselor (Robert), but they also connected us with Amanda Chagolla who has always been just an email or call… away when it comes to questions and handling our son’s medication. Mental health and healthcare in general is such a tough area of work to be in, but they do the best they can and treat us with respect which I really appreciate. Our son is back to straight A’s, enjoying school, and has wonderful support system now which means the world to us as his parents.
This is by far the most unorganized health center I’ve ever been to. I went in for a mental health assessment to give to my attorney, they wouldn’t give me a copy of proof that I did it, I signed a release for him to… get his own copy and they just don’t know who he has to speak to get it. It’s been a run around for quite some time. Steer clear of this place, go to Unison.
One Star because Clare our intake person was nice. The nurse practitioner was nice. The people that greet you need work. Especially the rude lady at the second window. I was desperate to get my son help as I had been… trying places for 9 months. The first red flag was when the rude lady at the 2nd window misgendered Clare calling her a him, and she was clearly a "she" doesn't matter if she used to be a he. They signed us up for services that I thought were a part of the "whole" therapy experience. When we showed up for the first therapy appointment., the rude lady doesn't greet us, she just blurts out that we have a balance for the prior weeks' meeting with the social worker. No statement for the charges, yet expecting a payment. After a phone call to my insurance company, the therapist my son was seeing informed me that she was not on their list. My son said the woman talked to him like he was a 5yr old and probably should work with grade school kids not teenagers. When we weren't sure if she was covered, she declined to even meet with me after his session. The other woman up front was supposed to follow up with me for a different therapist and it's been two weeks and she still hasn't. The people there do not return calls nor do they follow up. I would stay far far away from this place!
I will never go bk too this angency. I rather drive an hr or 2 out than go bk here very unprofessional the child physc she was rude!!! If I could it be 0 stars as far as a rating.
There are far better options. Save yourself the time and headache.
New to Harbor and have several issues. One, Hippa not always followed. Two, patients and staff are talking in common areas where other guests can hear. Three, staff has been overheard speaking negatively about patients… while my wife and son visit there. Finally today the last straw, our N.P. Brittney had an unexpected schedule issue(understandable) and was unable to meet with us. Upon arrival to the 3:30 appointment, we were told it had just happened and it was decided to still have us come down. After several calls and several unprofessional conversations with Joanie, Lori and Brittney herself, we find out the N.P. notified the office at 10:58 but the office didn't feel the need to call us to see if the 3:30 appointment would still serve our child's needs with the N.P. not attending thus proving our time and needs are not as important as our billable occurrences.
Complete joke of a place. They treat their patients like a number at the DMV. I'm on 3 weeks of waiting for a call back to call my sons prescription into the right pharmacy. They do not care whether your kid has the… medications they need or not. I do not recommend sending your kid there for any mental health treatment. I am now looking for a new doctor for my son to see, since harbor cant get there act together.
They aren’t very hands on. Therapists tend to be hard to book appointments with because they’re short staffed also hard to get ahold of anyone- no one picks up the phone!
I went there for only one day and as soon as I got there I knew it wasn’t going to give me the right treatment needed. It is in an old apartment building first of all and we had an appointment for 8:30 and the… “therapist” didn’t show up until 9. She looked very unprofessional to start off with. When we went to her “office” it was a very small room with old toys scattered around and she had a curling iron on her desk. While doing the online paperwork she asked me my sexual orientation and I said that I was gay and she thought I was lying and she wrote “claims to be ‘gay’ ”, then continued to ask me if I dressed as a boy (which I do not). Then she asked me about my initial problem and I told her that I was there for an anxiety disorder which I have struggled with for a long time and she said I don’t seem like the “person to have anxiety” and said I don’t fit the general description of someone with anxiety and said it seemed shady. She then googled, on her phone, the symptoms of anxiety disorders and then asked me if I had any of the symptoms even tho I just told her what my symptoms were. Overall I don’t believe this business takes what they do seriously and I don’t believe they can help kids as they should.
I love harbor but the doctor accused me of doing drugs when I have been sober three whole years because of discharging done. I was pregnant and the meds they gave me did not work so she lowered the medicine. Why lower… it when it's not working and my depression is getting worse. Never will go back.
Its hard to get regular therapy appointments for kids
Bad experience...only received medications...no additional services offered..
