The 2026 Align Senior Care California plan names inpatient psychiatric care, partial hospitalization, intensive outpatient program services, outpatient substance use services and opioid treatment program services among its covered categories.

Who the plans serve shapes what that looks like in practice. Enrollment in an Institutional Special Needs Plan requires Medicare Part A and Part B, residence in the service area, and either more than 90 days in a participating nursing facility or an institutional level of care in the community. The care model pairs the plan with on-site primary care where the member already lives, which is why coverage follows Medicare rules rather than the commercial pattern of traveling to a residential campus.

What Does an Align Senior Care Plan Pay For at Each Level of Care?

The plans pay the Medicare ladder of behavioral health services, with the level decided at clinical assessment and most inpatient care gated by prior authorization. Cost sharing changes by state and by plan, and the inpatient day bands below come from the 2026 summaries of benefits.

Medical Detox

Medically managed withdrawal falls under the acute inpatient hospital benefit, which is covered and requires prior authorization. The summaries do not use the word detox as a separate benefit line. Inpatient hospital cost sharing for 2026 runs at $0 per stay on the California plans, $100 per day for days 1–6 on Florida Premier Care and $230 per day for days 1–6 on Michigan Premier Care. Residential and Inpatient

Inpatient psychiatric care is covered with authorization. California plans charge $0 per day for days 1–3, $100 per day for days 4–10 and $0 per day for days 11–90. Florida and Michigan Premier Care apply the same per-day figures as their inpatient hospital benefit for days 1–6, then $0 through day 90. Skilled nursing care in California needs authorization, with the first 7 days after an acute stay approved automatically and the first 5 days of skill-in-place approved automatically in network.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient program services are both named as covered categories on the California plan, as are outpatient substance use services and opioid treatment program services. Outpatient individual and group therapy carry a $0 copay in California, and $20 individual with $10 group on Florida and Michigan Premier Care. No session limit for outpatient therapy is stated in the 2026 summaries.

Does AllyAlign Health Need Prior Authorization for Rehab?

Yes for several substance use services. On the 2026 California Advantage Care plan, authorization is required for outpatient substance use services, opioid treatment program services, partial hospitalization and inpatient psychiatric hospital care. It is not required for intensive outpatient program services, outpatient psychiatric services or mental health specialty services.

Michigan Premier Care publishes a different list for 2026: prior authorization for inpatient hospital, inpatient mental health, outpatient hospital services, diagnostic tests and radiology performed outside the nursing facility or physician office, genetic testing, non-emergency ambulance, some Part B drugs and durable medical equipment. The Florida, Iowa and Perennial Advantage lists are not published in the same form, so a member in those states asks Member Services which services on the treatment plan need review.

The plans do not name the medical necessity criteria they apply, and no authorization phone or fax number appears on the prior authorization list. Ask the admissions team which clinical criteria set the reviewer uses, who submits the request, and how continued stay is reviewed once treatment is under way.

Do I Need a Referral From My Primary Care Provider?

Generally no. Members choose a primary care provider, and the Michigan evidence of coverage states that in most situations no advance approval from that provider is needed to see an in-network specialist. Authorization and referral are separate questions, though, so a service on the authorization list still needs plan review even when no referral is involved.

Before a bed is held. Align Senior Care HMO plans pay out of network only for emergency and urgent care, for dialysis while traveling, and for medically necessary care the network cannot supply when the plan authorizes it in advance at in-network cost sharing. Michigan Premier Care is an HMO-POS, so certain services can be used out of network with limits and possibly higher cost sharing.

How Much Does Rehab Cost With AllyAlign Health?

Less than the sticker price, and capped by an annual out-of-pocket maximum. For 2026 the California Advantage Care and Premier Care plans carry a $0 premium, a $0 deductible and a $1,900 in-network maximum. Drug coverage carries its own deductible on two of those plans: $450 on Florida Premier Care and $615 on Michigan Premier Care, both applying to tiers 3 to 5. Florida retail tiers run $0 preferred generic, $10 generic, $45 preferred brand, $95 non-preferred and 27% coinsurance on specialty for a 30-day supply. A third-party listing shows Michigan Senior Care at 20% coinsurance for outpatient individual and group therapy, which is not stated on the plan’s own document and is worth checking with Member Services.

The 2026 summaries do not print separate copays for outpatient substance use services, opioid treatment programs, partial hospitalization or intensive outpatient. Those are the amounts a program bills without coverage. Under an Align Senior Care plan the member pays the copay or coinsurance for the service, and nothing in network beyond the annual maximum once it is reached, which is why the authorization step decides the bill far more than the rate card does.

Does AllyAlign Health Cover Suboxone, Methadone and Other Addiction Medication?

Yes. On the 2026 California Advantage Care formulary, buprenorphine sublingual tablets and buprenorphine-naloxone films and tablets sit on Tier 1 with a quantity limit of 120 per 30 days and no prior authorization flagged. Methadone 5 mg and 10 mg tablets and oral solution are also Tier 1, with a 360 per 30 days limit.

Naltrexone 50 mg tablets are Tier 2 with no restriction listed, and Vivitrol 380 mg injection is Tier 5 specialty and excluded from extended day supply. Naloxone injection is Tier 1, while the nasal spray products Kloxxado and Opvee are Tier 3.

Where the Prescription Is Filled

Medication taken during a covered inpatient stay is part of that stay. A prescription filled at a pharmacy runs through the Part D side of the plan, with the tier and any deductible above deciding the cost. Methadone for opioid use disorder is not a retail fill; it is dispensed inside a licensed opioid treatment program, and those program services need plan authorization on the California list.
