What is STAR Plus?

STAR+PLUS is the Texas Medicaid managed care program for adults age 21 and over who have disabilities and for adults age 65 and older. The Texas Health and Human Services Commission runs it and contracts health plans by service area, so each member joins one plan that delivers Medicaid care and long-term services.

Service coordination is the program's main feature. A coordinator meets each new member within 30 days to assess needs and build a service plan.

Does STAR Plus cover rehab?

Yes. Texas Medicaid covers substance use disorder treatment, including assessment, withdrawal management, outpatient counseling, residential treatment and medication assisted treatment, and STAR+PLUS health plans deliver those benefits. Admission starts with a face-to-face multidimensional assessment by a qualified credentialed counselor, and residential care must come from a licensed chemical dependency treatment facility.

Does STAR Plus cover detox?

Yes. Withdrawal management is covered at three levels under Texas Medicaid: outpatient for mild to moderate symptoms, residential when outpatient is unsafe, and inpatient hospital for severe withdrawal or serious medical or psychiatric conditions. STAR+PLUS health plans deliver that benefit, and several require authorization first.

Superior HealthPlan requires prior authorization for inpatient admissions including detox and for outpatient detox; its behavioral health authorization line is 1-844-744-5315.

Does STAR Plus cover residential rehab?

Yes. Residential substance use treatment is a covered Texas Medicaid benefit and must be delivered by a licensed chemical dependency treatment facility. Community First Health Plans lists detoxification, residential treatment and inpatient rehabilitation among its STAR+PLUS benefits, and Community Health Choice lists inpatient treatment for substance use and chemical dependency.

Superior HealthPlan requires prior authorization before a residential admission, so the center usually submits the request before the stay begins.

Does STAR Plus cover intensive outpatient treatment?

Yes. Community First Health Plans lists intensive outpatient programs and partial hospitalization among its STAR+PLUS behavioral health benefits, and Community Health Choice lists partial hospitalization. Superior HealthPlan covers day treatment, intensive outpatient and partial hospitalization when prior authorization is obtained first.

No referral from a primary care provider is needed for mental health or substance use services with Wellpoint, Community First or El Paso Health.

Does STAR Plus cover Suboxone?

Yes. Buprenorphine with naloxone, sold as Suboxone or Zubsolv, falls under Texas Medicaid's medication assisted treatment benefit, but it needs clinical prior authorization: an opioid use disorder diagnosis within 730 days, age 16 or older, and no opioid pain medication claims in the previous 30 days.

Authorization lasts 180 days under criteria effective October 1, 2025. Buprenorphine without naloxone also requires pregnancy or documented naloxone intolerance.

Yes. Which health plans offer STAR Plus?

Seven organizations hold the STAR+PLUS contracts that took effect September 1, 2024: UnitedHealthcare Community Plan, Molina Healthcare of Texas, Superior HealthPlan, Wellpoint, Community First Health Plans, El Paso Health and Community Health Choice. Texas has 13 service areas, and each area offers only two or three of them.

Is STAR Plus the same as STAR?

No. STAR is the Texas Medicaid managed care program for low-income children, pregnant women and families, while STAR+PLUS serves adults 21 and over who have disabilities and adults 65 and older. STAR Kids is a third program, for children and adults age 20 and younger who have disabilities.

Can I have Medicare and STAR Plus?

Yes. Individuals with both Medicare and Medicaid can enroll in STAR+PLUS. Medicare stays the primary payer for acute and behavioral health care, including addiction treatment, and members keep their Medicare providers, while STAR+PLUS mainly covers long-term services and supports.

STAR+PLUS is the Texas Medicaid managed care program for adults who have a disability or are age 65 or older, run by the Texas Health and Human Services Commission rather than by an insurance company. Drug and alcohol treatment is covered through the contracted health plan named on the member card, and that plan sets the network, the authorization rules and the phone number that answers questions about a planned admission.

HHSC divides the state into 13 service areas and contracts two or three health plans in each. Seven organizations hold the contracts that took effect September 1, 2024: UnitedHealthcare, Molina, Superior HealthPlan, Wellpoint, Community First Health Plans, El Paso Health and Community Health Choice. The line-up changed in nine of the 13 areas on that date, and Cigna’s HealthSpring was not awarded a contract. A listing that names STAR Plus is saying it works with the program; which plan it contracts with is the next question.

Does STAR+PLUS Cover Drug and Alcohol Rehab?

Yes. Texas Medicaid covers substance use disorder assessment, withdrawal management, outpatient counseling, residential treatment and medication assisted treatment, and STAR+PLUS health plans deliver those benefits. Admission follows a face-to-face multidimensional assessment by a qualified credentialed counselor, which sets the level of care. Authorization rules differ from one contracted plan to the next.

Plan wording follows the same benefit. The Community First handbook lists outpatient mental health and substance use services, detoxification, residential treatment, inpatient rehabilitation, intensive outpatient programs and partial hospitalization. Community Health Choice lists inpatient, outpatient and partial hospitalization care along with treatment for chemical dependency. UnitedHealthcare lists mental health services and substance use treatment as 100% covered with no copay, Superior lists behavioral health as a core benefit, and Wellpoint places mental health and substance use services inside its acute medical care benefits.

Which plan serves your area? Harris (Houston): Community Health Choice, Molina and UnitedHealthcare. Bexar (San Antonio): Community First, Molina and UnitedHealthcare. Dallas: Molina, Superior and UnitedHealthcare. Travis (Austin): Superior and UnitedHealthcare. El Paso: El Paso Health and Molina. Lubbock, West Texas, Central Texas and Northeast Texas kept their earlier plans through the 2024 contract change.

Medical Detox

Withdrawal management is covered at three levels: outpatient for mild to moderate withdrawal, residential where outpatient care would be unsafe, and inpatient hospital for severe withdrawal or where serious medical or psychiatric conditions sit alongside it. Superior requires prior authorization for inpatient admissions including detox and for outpatient detox, so the admitting team usually files that request. Residential and Inpatient

Residential substance use treatment is covered and has to be delivered by a licensed chemical dependency treatment facility, a license issued by HHSC. The Community First handbook also lists psychiatric hospital care for members age 21 and over. Ask the admissions team which STAR+PLUS plans hold its contract and whether residential rehab has been authorized before a bed date is agreed.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient care appear on plan benefit lists, and individual and group counseling is covered, with groups of up to 16. State fee-for-service policy sets 135 units of group and 26 hours of individual outpatient counseling a year, with more available by prior authorization, and a plan may apply that differently. Policy says abstinence should not necessarily be required to take part. Tobacco use disorder as the primary diagnosis sits outside the benefit, and Community First lists virtual behavioral health visits as covered.

Who Can Enroll in STAR+PLUS?

Adults age 21 and over who receive SSI, who need STAR+PLUS home and community based services, who are in Social Security Exclusion programs or who live in a Medicaid nursing facility, plus members of Medicaid for Breast and Cervical Cancer. Those on other waiver services, residents of IDD community facilities with Medicare, anyone without full Medicaid such as QMB or SLMB coverage, children in state foster care and anyone under 21 outside the Breast and Cervical Cancer program enroll elsewhere in Medicaid.

Does STAR+PLUS Need Prior Authorization for Rehab?

It depends on the health plan, because there is no single STAR+PLUS authorization list. Superior requires prior authorization for inpatient admissions including detox, outpatient detox, partial hospitalization, intensive outpatient and day treatment, and residential treatment. Under state policy the request is completed and signed by a qualified credentialed counselor, and going past the annual outpatient limits also needs authorization.

A referral is a separate matter, and it is usually not required: Wellpoint, Community First and El Paso Health all state that members can use mental health and substance use services without a referral from a primary care provider. Behavioral health authorization for UnitedHealthcare STAR+PLUS runs through Optum Behavioral Health on the provider line 1-877-614-0484, Superior takes behavioral health authorization requests on 1-844-744-5315, and Community First points members to Member Services for its list. Superior also allows in-network providers up to 8 hours of psychological or neuropsychological testing per member per calendar year without authorization.

How Does a Service Coordinator Fit In?

Service coordination is described as the main feature of STAR+PLUS and is open to every member. The plan’s service coordinator meets a new member within 30 days of enrollment to assess needs and build a service plan, and the HHSC handbook says anyone with a history of behavioral health conditions or substance use disorders receives at least monthly contact from the plan plus face-to-face contact. The coordinator is the right first call when a treatment admission is being arranged, and the plans also run non-emergency medical transportation to covered appointments, through MTM Health on 844-867-2837 for Wellpoint, SafeRide on 1-855-932-2335 for Community First and Access2Care on 1-844-572-8196 for El Paso Health.

Out-of-network is where bills come from. The Community First handbook states that if a member sees a provider outside the network, the plan will not pay and the member can be billed. Superior requires prior authorization for all services from non-participating providers. Settle network status and authorization with the plan before the admission date, not after.

How Much Does Rehab Cost With STAR+PLUS?

For covered care in network, usually nothing. UnitedHealthcare and Wellpoint both state there are no copays for covered STAR+PLUS services, UnitedHealthcare lists mental health services and substance use treatment as 100% covered with no copay, and Community First lists no copay for behavioral health visits. The money risk sits with out-of-network care and unauthorized stays rather than with the benefit itself.

Plans add extras on top of the standard benefit, and those differ by service area; HHSC publishes comparison charts so a member can see them side by side. Superior pays a $20 reward for a follow-up visit within 7 days of an emergency room visit for a substance use disorder or after an inpatient behavioral health stay. Members With Medicare as Well

Holding both coverages changes who pays first. STAR+PLUS does not alter how Medicare services are delivered: Medicare remains the primary payer for acute and behavioral health care, and STAR+PLUS mainly covers long-term services and supports. The STAR+PLUS Medicare-Medicaid Plans ended December 31, 2025, and from January 1, 2026 those members moved to integrated dual special needs plans paired with a STAR+PLUS plan, with UnitedHealthcare Connected members in Harris County moving to a UnitedHealthcare HIDE SNP alongside UnitedHealthcare STAR+PLUS on one combined ID card.

Does STAR+PLUS Cover Suboxone, Methadone and Other Addiction Medication?

Yes. Texas Medicaid covers medication assisted treatment for alcohol and opioid use disorders, prescribed by physicians, nurse practitioners and physician assistants or delivered through opioid treatment programs. Buprenorphine and naloxone products carry clinical prior authorization criteria, and the health plan’s pharmacy benefit manager handles the request.

Under criteria effective October 1, 2025, a buprenorphine and naloxone authorization asks for an opioid use disorder diagnosis within the past 730 days, an age of 16 or over and no opioid pain medication claims in the last 30 days, and it runs for 180 days. Buprenorphine without naloxone needs the same diagnosis plus pregnancy or documented naloxone intolerance. Opioid treatment programs are a recognized provider type in state policy, but methadone-specific rules were not published on the pages reviewed, so a member starting at a methadone clinic should ask the plan how that course is authorized and how long an authorization lasts.

Where the Prescription Is Filled

Pharmacy benefits run through the plan, and the manager differs: Navitus Health Solutions serves Community First and El Paso Health, with Navitus reachable on 877-908-6023 for El Paso Health members. Medication dispensed inside an opioid treatment program is part of that program’s service rather than a pharmacy fill, which is why it does not appear on a drug claim. Naltrexone formulary placement was not published on the pages reviewed, so ask the plan which formulations sit on its drug list before discharge.

Which Rehab Centers in Texas Are In-Network With STAR+PLUS?

A program is in network when it holds a contract with the member’s own STAR+PLUS health plan, since each contracted plan runs its own network across its service areas. Residential treatment has to come from a chemical dependency treatment facility licensed by HHSC. Network status is settled in that plan’s provider directory or by calling the member number printed on the card.

Start from the plan name on the member card, then use that plan’s directory. Wellpoint publishes STAR+PLUS directories by service area and an online Find Care search; UnitedHealthcare members search its online directory or call Member Services on 1-888-887-9003, which also answers behavioral health questions around the clock. HHSC publishes county maps of STAR+PLUS plans, useful when a move changes the service area.

Is STAR+PLUS the Same as STAR, STAR Kids or Medicaid?

No to the first two, and partly to the third. STAR+PLUS is one of Texas Medicaid’s managed care programs, so a member holds Medicaid and receives it through a STAR+PLUS health plan. STAR covers low-income children, pregnant women and families, and STAR Kids covers those age 20 and younger who have disabilities. None of these exist outside Texas.

Changing or choosing a plan: new members have 15 days after the enrollment packet arrives to pick a health plan before HHSC assigns one, and changes are available monthly through the Texas enrollment broker on 1-800-964-2777 (TTY 711) or the Your Texas Benefits website.
