Anthem’s 2023 expansion took its individual plans from nine southeast counties to 68 across the state, at every metal level. BadgerCare Plus members in Brown and Outagamie counties can pick Anthem among nine HMOs. Rehab centers in Green Bay and Oshkosh treatment programs appear in the listings.

Anthem BadgerCare Plus covers Eau Claire, Chippewa and Dunn counties alongside Group Health Cooperative of Eau Claire and Security Health Plan, and Anthem’s Medicare Advantage and employer plans also serve the region. Eau Claire addiction treatment and Menomonie rehab services serve the Chippewa Valley.

Can I use Anthem for rehab in a different state?

Sometimes. Anthem’s 2025 Wisconsin POS contract limits out-of-state services to emergencies and urgent care unless authorized in advance, and Anthem’s individual HMO certificates from other states pay non-network substance use care only in an emergency. Our sources do not describe BadgerCare Plus coverage outside Wisconsin, so Medicaid members should ask Anthem before any treatment across the state line.

How much does rehab cost with Anthem?

Anthem costs depend on the plan. Individual plans follow their Bronze, Silver or Gold deductible and coinsurance, and employer plans follow the employer’s design. Anthem’s Medicaid members pay small BadgerCare Plus copays whose monthly total cannot exceed 5 percent of household income, though Medicaid does not pay for residential room and board.
