EHS Inc – Buffalo Address, Location and Contact Information
You will find this provider at 206 South Elmwood Avenue, Buffalo, New York, 14201. Erie County covers this area. The site also serves the surrounding area, and anyone traveling in should confirm the route before setting out. Public transport, parking and the length of the journey all affect whether someone completes a course of treatment, and they are worth settling before the first appointment rather than after.
It is worth calling first to confirm access, parking and the shape of a first visit. Remote appointments may be possible where travel is a barrier. Reception hours vary, so a call that goes unanswered once is worth repeating rather than treating as a dead end.

If I could rate lower I would. Absolute garbage place. They don't coordinate between departments and their mental health receptionist went out of her way to make me uncomfortable. Would not give me information, would… not help with finding resources. Just rude and unhelpful. I will be reporting them to the BBB and DOH If you're trying to get any kind of help your better off going to the ER or ECMC psyche department.
ABSOLUTELY NO JUDGEMENT here..Everyone is kind ..caring and compassionate with hearts of gold..I highly recommend Evergreen Health to everyone..To everyone there..thank you for showing everyone who walks thru your doors… much needed compassion..
Im highly disappointed by the unprofessionalism in my recent visits as I really like this practice and have had great experiences in the past. After not receiving my sti results I messaged the office and asked for an… update, none was given and I was told I would need to make another appointment to be tested. I make the appointment and go. I was informed at this visit my vaginal sample from my previous visit was discarded/not tested. The doctor stated it was probably because my chart states male(as I am transgender) and someone ASSUMED it was incorrect for a vaginal sample to be tested. This is ABSURD. As a practice that specifically deals with queer people, I was appalled at the lack of attention to detail of my chart. NEVER at any other doctor’s office I have been to has done this, EVER. I have been transitioned for over a decade and gone to many specialists, different primary care doctors and planned parenthood, AND NEVER HAD THIS HAPPEN. I am disappointed in the care i received that day and the lack of attention to detail in both my message AND the physical sample. I am unable to recommend Evergreen for transfolk. They are good for short term solutions but FIND SOMEWHERE ELSE if you plan on taking care of yourself in the long term.
The lady that answered the phone was rude
I spoke with schedulers Ray and Destiny and a nurse Lisa on the phone. They were all extremely compassionate and willing to get me the help I need.
This is the first time in my life i have felt validated and heard by an OGBYN. I was treated like a real person with real pain/discomfort, rather than just a babymaker. They are all so kind here.
Been my primary since I moved here and they haven’t disappointed at all in 7yrs. Dr Atwal, Dr Gabor and others have been absolutely amazing to work with!
EHS has been my medical provider for several years. Overall, the care has been excellent with staff being attentive, accommodating, and respectful. However, I have experienced some ups and downs, especially with turn… over in providers at least three times. This has resulted in broken rapport and differing approaches in treatment. My other gripe has been extended wait times even with scheduled appointments. I do appreciate the integration with The Pharmacy at Evergreen, which delivers medications to your home. EHS is a good choice for persons looking for a new provider that takes a person-centered approach and specializes in medical care for the LGBTQ+ community.
The offices are clean with disability and impairment accommodations and amenities for their patients, and they offer great free services for people in need, but their doctors and employees are very unprofessional. Their… security guards casually talk about one-night stands and other inappropriate gossip in public and the doctors like to play mind games and toying with people's wellbeing. My therapist refused to add a medical note to my recent IQ test to document that I was sleep deprived during the testing and he mocked my results by belittling and patronizing me. My psychiatrist who prescribes my medications constantly does power plays to assert her dominance; she refuses to answer my social worker's messages and inquiries about prescriptions to force me to wait months for each appointment just to receive a response which is hindering my mental health recovery process: this is done as a display of power. I already cycled through doctors around 5 times or more due to negligence and malpractice between this facility and Best Self Behavioral.
My name is Joshua Noble-Donahue, and I am writing this review because I left my experience with Evergreen Health feeling dismissed, unheard, and ultimately abandoned as a patient. When I first came to Evergreen Health… about two years ago, I was seeking help for several ongoing health concerns, including severe sleep problems, restless legs symptoms, nerve-related symptoms, emotional regulation, focus issues, and questions about whether ADHD could be contributing to my difficulties. Instead of feeling like I had a clear, consistent treatment plan, I often felt like I was being passed from provider to provider and receiving conflicting information. As a patient, that was confusing and frustrating. As time went on, I became increasingly concerned about my overall treatment plan. I felt my concerns were not being fully addressed, and despite continuing to struggle with my symptoms, I wasn't receiving clear explanations or collaborative discussions about my care. I spent time researching my symptoms, documenting my concerns, and came to appointments prepared to have thoughtful conversations about possible treatment options. My goal was never to tell my providers how to practice medicine, but to advocate for myself and better understand my own health. After adjustments were eventually made to my treatment plan, I experienced noticeable improvements in several of my symptoms. That experience made me feel that some of the concerns I had raised deserved more attention earlier in my care. The most upsetting part of my experience came after I began asking more detailed questions and advocating for myself. Instead of feeling supported, I felt as though the relationship with my primary provider changed. My future appointments were canceled, and I was left feeling like I no longer had a provider willing to work with me. I understand that patients and providers will not always agree on every aspect of treatment. However, I believe every patient deserves respectful communication, clear explanations, and collaborative care. No one should feel punished or abandoned simply for asking questions about their health or wanting to participate in decisions regarding their care. I am not writing this review out of anger or to attack anyone personally. I am writing because I am exhausted and deeply disappointed. I came to Evergreen Health looking for help during a difficult time in my life, but I left feeling dismissed, confused, and unsupported. Every patient deserves to feel heard, respected, and included in conversations about their own healthcare. Based on my personal experience, I cannot recommend Evergreen Health.
I drove an hour here, waited an hour at least to see the doctor, then I drove another hour home. All while paying my ride $10 an hour. This is ridiculous. Normally I really like the doctor here so I was keeping it when… I should’ve moved it somewhere closer. It’s really hard finding good doctors. A lot of these places are horrible. I’m giving a three star purely becausedr eluid Has always been a really nice guy and tried to help me out the best you can. But I just don’t have time or money for this anymore. Unfortunately, after being here for the last eight years, I’m gonna have to switch.
They refuse to let you change providers without a meeting with a supervisor. I get that not everyone can work Monday - Friday 8am to 5pm daily, but if I try to tell you that I need someone who can work on days where im… available then I should be transferred to someone who is available! You cant make people wait MONTHS for a chance to BEG to switch providers. It is incredibly demeaning. You also only get one online portal account for all of evergreen and it will not integrate with other services. So if you first came to evergreen for the dentist, you wo t be able to use the portal to message your GP or request meds or make appointments anywhere but the dentist. This forces you to call and ask the staff to deliver messages many of which never get a response. The worst part is non of it will change because this "non-profit" has a CEO making millions.
I go here for mental and physical healthcare, and I've never been happier with a doctor's office before. I've never felt so seen, heard, and understood by medical professionals before, and I couldn't possibly recommend… Evergreen enough. In-person, tele-health appointments, every possible accommodation I can imagine. If you're like me and don't have health insurance they'll actually work with you on getting the care you need for a price you can afford, and I promise you that you can afford it. Just talk to them. New patients call on the 1st of every month to get in. Call early in that day, setup the appointments, you won't regret it.
Kyle Switzer has saved my life I was highly addicted to opiates in 2015 and joined the Suboxone program he is always there for me and truly cares how a feel and wants you to succeed . Thank you Kyle Brian Lee Spellburg
They are very helpful however they are not always well-coordinated and are under-funded. They are still the best community healthcare service in WNY and the only one that is truly LGBT+ supportive. They provide much… more than just healthcare with having a wellness program that includes activities like cooking classes and art therapy, a food pantry, and a pharmacy that is nonprofit and does not overcharge or screw customers over the way for-profit pharmacies often do.
I have been going to Evergreen since 2018 and I am a Transgender Male and I have been through 4 different doctors, but I am glad I started going here. I feel so heard and seen. My lastest doctor Sydney Gregory has been… the best doctor I have had since going there. She really listens and does everything she can to help me feel better. I would give 10 stars if I could. I have recommended a few people to switch to Evergreen. A friend of mine has been having issues with her doctor so I suggested asking if she can switch to my doctor. Told her how good she has been to me. I give 10 out of 10 rating. Totally recommend Evergreen.
Really cool people doing God's work with little to no acknowledgment.
Best place to get yourself in treatment for numerous things and also the mental health services are phenomenal to let y'all know if you are looking for a friendly place to go to evergreen is that one place 💯💯 the also… have multiple things that they do for the community also like they have a pantry and other things ect hands down they are good people for sure for sure 💯😊😊💯
This primary care office is one of the worst practices I have ever been to. They suck at communication, the response time of horrible. Trying to get an appointment is impossible. Apparently they only have 1 nurse there… to answer and or return calls, and only they can set up an appointment. The wait time is excessive in testing room and in the exam room. I work at a PCP office and I would be ashamed if my practice ran the way this one does. I do not recommend and will be looking for another PCP.
Good service
I felt the need to write this review.. being a female and in my 40's.. my friend recommended Evergreen because of the staff. He was right! They made me feel so welcome and seen. So understanding and they listen. I… signed on with my primary today, and always nervous if I will feel comfortable talking with someone new. Linda B. made me feel so comfortable and so welcoming. She came in with the biggest kind warmest smile and she is so easy to talk to. I'm so happy with the move I've made. :). The energy and the feel of a place is huge for me, but the fact that you can get everything done there! They take your blood right then and there so you don't have to schedule at quest. They have every dr you need, including vision and dental!
I have been a patient at this facility for over six years. My primary concern, which I have consistently observed, pertains to the operational efficiency of the pharmacy department. Each month, or whenever I attempt to… refill prescriptions, I encounter recurring issues. Specifically, when requesting refills for my standard medications, particularly those requiring syringes due to my hormone replacement therapy, I consistently face difficulties in obtaining the necessary supplies. A similar challenge arises with my estrogen liquid medication. I am typically provided with a supply intended to last a specific duration. However, when this supply proves insufficient, leading to withdrawal symptoms, I am consistently met with a lack of explanation. The recurring response suggests that the issue stems from my own actions, despite my consistent adherence to the prescribed medication regimen for several years. While I value my relationship with Evergreen, I strongly believe that improvements are necessary within their pharmaceutical department.
