Federally Qualified Health Center (FQHC)

Federally Qualified Health Centers and Rehab

Primary care and behavioral health share a building at a federally qualified health center. Federal funding comes with a condition: serve everyone, insured or not. Addiction treatment, counseling and prescribing often sit alongside the family doctor, dentist and pharmacy, which keeps referrals short.

Sliding fee discounts are a condition of FQHC funding, calculated from household income and size, so bring documentation to a first visit. Buprenorphine prescribing is common at these sites. Listings record what each center accepts for payment. Compare the health centers serving your area.

Why Trust RehabSeekers.com?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

What is a federally qualified health center?

A federally qualified health center is a community health center that meets HRSA Health Center Program requirements, serves a medically underserved area or population, and charges on a sliding fee scale. HRSA describes these clinics as treating medical, dental, mental health, substance use and other health care needs in one place.

HRSA also says health centers are community based and led in part by patients, with a governing board where a majority of members are patients of the center.

Community Treatment Centershttps://rehabseekers.com/type/community/
What is the difference between an FQHC and a regular clinic?

An FQHC must serve a designated medically underserved area or population, keep a governing board with a patient majority, offer a sliding fee scale and serve individuals regardless of ability to pay. A private clinic carries none of those federal obligations and sets its own fees.

HRSA status also brings FQHC-rate reimbursement, 340B drug pricing, Vaccines for Children access and National Health Service Corps staffing help.

Nonprofit Rehabhttps://rehabseekers.com/type/nonprofit/
Do FQHCs treat mental health conditions?

Yes. HRSA defines health center mental health services as prevention, assessment, diagnosis, treatment and follow-up for conditions such as depression, anxiety and attention deficit disorders, including care of stabilized patients with severe mental illness. Services may include individual or group counseling, psychotherapy, cognitive-behavioral therapy, 24-hour crisis services and case management.

What any one center offers depends on its HRSA-approved scope of project, so ask before booking.

A federally qualified health center, usually shortened to FQHC, is a community health center that meets the federal Health Center Program requirements, or holds look-alike designation, or operates as a tribal or urban Indian outpatient program. It delivers primary care with behavioral health built in, serves a designated medically underserved area or population, discounts its fees by income and family size, and may not turn a patient away for inability to pay.

That last obligation is the thing that distinguishes an FQHC from an ordinary clinic, and it is not a courtesy — it is a condition of the designation, together with a governing board where a majority of members are patients of the clinic. An FQHC is also a distinct Medicare provider type with its own payment system, which is why a visit there is priced differently from the same appointment in a private office. It is not a rural health clinic and cannot be approved as one.

Below: what the designation requires, the difference between a funded center and a look-alike, who can walk in with no coverage, how the sliding fee discount is calculated, what behavioral health and substance use care falls inside an approved service list, what a visit costs under each payer, what to ask at the front desk, and how to find a health center near you.
