About CHOICE Program – Indianapolis

The provider’s premises are at 1400 North Ritter Avenue, Indianapolis, Indiana. The facility is listed for detox, residential treatment and intensive outpatient. Medication-assisted treatment is part of what is listed, which for opioid dependence means buprenorphine, methadone or naltrexone alongside counseling.

The model is residential: people move in for the length of the program. Remote appointments appear on the record, useful where travel is difficult. Trauma-specific provision is recorded here. Federal listing is via SAMHSA, and the Indiana Division of Mental Health and Addiction handles licensing in this state.

CHOICE Program – Indianapolis Detox and Rehab Services

The provision listed at CHOICE Program – Indianapolis is detox, residential treatment and intensive outpatient. Medical supervision covers the withdrawal period, where alcohol and benzodiazepines carry real risk. Sessions follow a set timetable while people keep living at home. Anyone unsure which level fits should ask for an assessment rather than choosing from a list, since placement criteria exist precisely to make that judgement.

The listing records a single focus rather than combined treatment, so anyone needing both should confirm what is available. Medication and talking treatment run together under clinical oversight. Ask directly about session times and frequency, as those details decide whether someone can realistically attend.

CHOICE Program – Indianapolis Address, Location and Contact Information

The facility address is 1400 North Ritter Avenue, Indianapolis, Indiana, 46219. The city lies within Marion County. The catchment reaches beyond the city itself. How someone gets there matters more than it sounds: travel is one of the commonest reasons a course of treatment stops partway.

Phone ahead about parking, access and what the first appointment covers. Remote appointments may be possible where travel is a barrier. Admissions lines are frequently busiest early in the week, which is worth knowing if the first attempt fails.

Called in to speak with the facility about accepted insurances. When selecting a health insurance plan, it's helpful to know if your doctor will accept and honor your insurance. When calling I got 0 answers and each… person I talked to told me they 'can't tell me who they are in network with' and to refer to the insurance company. This seems ridiculous that the provider office doesn't know if they are in network with an insurance carrier or not....

Takilea C. Called me to schedule my GI appointment. She was extremely pleasant, very helpful, and very knowledgeable to all of my questions and I had a lot. I switched to community because I wasnt happy with IU over my… last few visits, even though the earliest appointment was far out, she gave me options with 2 physicians and we scheduled it and she added me to a waitlist so whoever had an appointment opening sooner, she could get me in. This was much appreciated. She went over and beyond on that call and she's the first stop, first impression, so with this upcoming appointment being my second visit, it gives me an idea of the care I will receive and that brings me comfort. Thank you Takilea😊!

I don’t recommend community health network hospital when I was 18 year old as I went to medical crisis their nurse have forced me to test and iv and change with gown without my consent on and they have their security… guard watch me all night like was prison and this wasn’t first time I have deal with community health network well they try to violate my disability rights at 21 years old at time where they have denied me medical service because they want me bring my parent when I am legal adult without guardianship I understand that happen course I have fill complaint against them for denied my appointment and remove guardship required once all as i decided switch doctor because I don’t trust commtiny health network as same year when I was 21 year they just basic kick me out their counselor service because I just was out of country like 2 month and I came back the Indy and next I know they told me I have find different counseling and gave some random number to call and over year now I feel traumatized and disappointed how they handle patient and they don’t care about your health if you are disability or have mental health issue and they need held accountable

Getting ready to transfer even more care away from Community. Doctors seem fine, but their office staff, billing, and insurance liaisons appear to have a lot of turnover, resulting in errors and communication gaps. Too… often blame patients for internal issues. When fine, it's fine; when not, seems to be blame the patient...

5 stars for Barb in Data Integrity and Megan who faxed name change for mother on my son's birth certificate to the Marion County vital records - after getting bounced around these two women finally worked the problem so… quickly and I was able to walk out with amended copy of birth certificate, same day, in order to make passport appointment in Chicago tomorrow. Not sure they'll ever see this but THANK YOU for your integrity in getting me the help I needed in absence of phone number for birth registrar.

I have grown to absolutely hate the community network. It’s always a reschedule, cancellation because they can’t get things straight with the insurance company. Most of all I’m sick of being judged because I look… younger than I am and am heavily tattooed. I got denied from all three specialists I’ve seen because I asked for a pain killer at least three days with to get some refills, they’re all gonna act like they don’t prescribe narcotics whereas an upper class white man I know has a whole pharmacy. I have a hernia, a pinched nerve in my lumbar region and my injections for cervical dystonia after wearing off. I’m in so much pain and work with autistic kids for a living so I’m obviously not a drug addict. I have to live a child, run after, and everything else and all I asked for was for a few days worth of pain killers so I’m not suffering. I can’t take ibuprofen. I’m in an ridiculous amount of pain. I’m switching to eskinazi. Just because I have a lot of tattoos and don’t look like a Caucasian soccer mom doesn’t mean I’m a junky. I am am a person who in pain.

I was seen at community health October 09, of 2019 because I had been sick for two weeks by that time. A week prior I went to urgent care. I had a fever that I had for 3 or 4 days at that time. I was told I had strep… throat. I was told that the bacterial test for it came back negative. I was told I should feel better in a few days and to contact my pcp, which I didn't have so they set me up with one. I didn't get better, I got worse so I took myself, while very ill, to the hospital emergency room. My heart rate was elevated but my fever had lowered from being outside for an extended amount of time in the cold. I notified the nurse that I couldn't swallow food and had intense pain and difficulty swallowing water. I was very dehydrated. They looked at the notes from urgent care and said my bacterial strep test was negative but prescribed me antibiotics anyway, to take, while there in their presence, with ice water. Though they knew I had difficulty swallowing. I managed it and they sent me on my way. No other care was given. My dehydration wasn't addressed. Neither was my elevates heart rate. I went home and received a very high bill that I had been paying ever since. I was notified in January that community health hospital was charging me a separate fee, of a sum above 1,600 dollars, for having been in their facility after I had been paying for a 3,000 dollar bill since 2019. As of October of 2021 it was now a total due of 466.87. I hadn't been seeing a doctor during that time because I couldn't afford to due to paying that bill. During the January call I told then I could not begin to pay for having been at the community health hospital because I was already paying for the bill directly to the doctor I didn't even see, I only saw two nurses. And thought that I'd just be paying off the last 400 dollars. I received a payment reminder today that had the sum of 1,633.87, and called about that number which I hadn't agreed to pay and shouldn't be paying. I was unaware that the initial bill for specifically for the doctors only and that the new total was for the hospital separately. Mind you I am insured and community health is supposed to be in network. I haven't been able to see a doctor because my fsa is used to pay for any bills not covered and co pays. The money going in twice a month is only slightly more than the money coming being removed to pay my bill which doesn't give me enough wiggle room to pay co-pays and any other new bills. That would mean that I haven't been seen by a doctor since my ER visit 3 years ago that cost me 3000 in doctor fees and over 1600 in facility fees. That should be criminal that a medical emergency, which it was, would cost so much. I wouldn't recommend going to this hospital as it is akin to highway robbery. I would rather die at home, as that would be cheaper.

Very professional while being kind. I felt comfortable with all my caregivers. Doctors, nurses, Techs and the restaurant. Food was always hot and delicious. After I was released and come back to wound care. Great… experiences. I wish I knew all the names of all the caregivers. I was on third floor/orthopedic.

My husband had a fall and went to ER, scared for the injury (fortunately good out come) and as a nurse, I tried not to be hypercritical but I was apprehensive of the facility, The experience was professional from point… of check in, escorting to room, registration, nursing and the PA. Each one was thorough and I was very impressed with every player's level of communication. There were times it felt like we were waiting, but the reality was we didn't, we were just nervous and wanted answers 'immediately'. Walked in with not fair judgement and walked out with a very positive attitude. Great experience.
