Rehabs That Take Ascension

Ascension appears in the insurance lists of treatment centers in Illinois, Indiana and elsewhere. Ascension is a large Catholic health system, and the listings do not say which Ascension-linked plan or network they mean. Where an Ascension plan applies, coverage for drug and alcohol treatment can vary by plan, network and level of care.Because the exact plan is unclear, start with the name printed on your insurance card and read it to the facility. Ask the center whether it works with that plan, then ask the insurer about network status, prior authorization, any referral needed and your likely out-of-pocket cost before admission begins.

Why Trust RehabSeekers.com?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

What is Ascension health insurance?

Ascension is a nonprofit Catholic health system based in St. Louis, not primarily an insurer. The only coverage sold under its own name today is SmartHealth, a self-funded medical plan for Ascension associates and their dependents, sponsored by Ascension Technologies and administered by Automated Benefit Services.

Its marketplace plan, Ascension Personalized Care, ended December 31, 2024, and its Medicare Advantage plan, Ascension Complete, became Wellcare Complete in January 2024.

Private Insurance Acceptedhttps://rehabseekers.com/insurance-accepted/private-insurance-accepted/
Does Ascension SmartHealth cover rehab?

Yes. Substance use disorder and addiction treatment is a listed SmartHealth benefit, built on the same structure as mental health coverage. The schedule of benefits names individual and group therapy, e-visits, partial day treatment, intensive outpatient therapy and inpatient admission, and residential treatment sits on the prior authorization list.

What you pay depends on which SmartHealth option you hold and whether the center is in the Ascension Network, the Anthem-based National Network or out of network.

Private Insurance Acceptedhttps://rehabseekers.com/insurance-accepted/private-insurance-accepted/
Does Ascension SmartHealth cover residential rehab?

Yes, with prior authorization. Residential treatment center admissions for chemical dependency and psychiatric care appear on the SmartHealth prior authorization list, so authorization must be in place before admission. On the 2026 PPO the member pays 20% after deductible in network; the EPO and HDHP apply 15%.

Authorization requests go to 844-217-8191, Monday to Friday 8:00 a.m. to 6:00 p.m. EST, or by fax to 586-693-4768.

Ascension is a nonprofit Catholic health system based in St. Louis, and the one health plan it writes under its own name today is SmartHealth, a self-funded medical plan for Ascension associates and their dependents. If the card reads Ascension SmartHealth, the plan sponsor is Ascension Technologies and the claims administrator is Automated Benefit Services in Detroit, which is who an admissions team will bill.

The other Ascension-branded plans that still appear on treatment center websites have ended or changed name. Ascension Personalized Care, the marketplace plan underwritten by US Health and Life Insurance Company, left the exchange in every state, with the last day of coverage on December 31, 2024. Ascension Complete, the Medicare Advantage venture with Centene, became Wellcare Complete on January 1, 2024, and the old Ascension Complete member ID cards stopped working that day. In Texas, Seton Health Plan, Inc. trades as Dell Children’s Health Plan for Medicaid STAR and CHIP in the Travis Service Area.

This guide covers what SmartHealth pays for at each level of addiction treatment, which admissions need prior authorization and who decides, what the 2026 PPO, EPO and HDHP figures look like, where addiction medication sits now that the pharmacy benefit has moved, and how to work out whether a program sits in Tier 1, Tier 2 or out of network.
