AultCare is a Canton, Ohio health plan and third-party administrator owned by the nonprofit Aultman Health System, and in northeast Ohio it is the organization that authorizes an addiction treatment admission and settles what the stay costs. It has operated under that name since 1985 and still trades under it in 2026.

What AultCare writes now is employer group coverage, self-funded plans it administers, MEWA plans, Medigap and Medicare Advantage under the PrimeTime Health Plan brand. Individual and family marketplace coverage ended with a last day of coverage of December 31, 2025, and small group insured ACA plans were discontinued for the 2026 plan year, a withdrawal that affected roughly 6,000 enrollees. An old marketplace card no longer describes a live benefit.

Does AultCare Cover Drug and Alcohol Rehab?

Yes. AultCare’s behavioral health material describes five levels of care: inpatient hospital, residential crisis stabilization, partial hospitalization, intensive outpatient and outpatient treatment. Sample plan summaries state that mental health, behavioral health and substance use services are payable on the same basis as any other illness, subject to the rules of the specific plan.

The qualifier is whose plan it is. A large share of AultCare business is self-funded employer coverage, where covered services and cost sharing are set by each employer’s Plan Document rather than by AultCare, and the prior authorization guideline tells a member to read that document. PrimeTime Medicare Advantage runs on its own benefit schedule instead.

Which AultCare is on the card? Commercial group numbers starting A, D, I, 4, 6, 7, 8 or 9 are insured or MEWA plans; those starting 1, 2 or H are self-funded. Cigna, Quality Care Partners, Aultra and The Timken Company group 157 plans carry extra requirements, and PrimeTime Health Plan cards follow the Medicare Advantage schedule. What Does AultCare Pay For at Each Level of Care?

All five levels appear as recognized service types on the commercial prior authorization list, which names behavioral health and substance use inpatient admissions, inpatient detox, observation beyond 48 hours, residential, PHP and IOP. The level is matched to clinical need at assessment, and a member with no behavioral health provider is told to start with the primary physician.

Medical Detox

Inpatient detox is a listed service and needs prior authorization on commercial plans. Emergency and urgent care is covered in or out of network with no pre-certification, so acute alcohol or benzodiazepine withdrawal belongs in an emergency department rather than on hold with a plan line; notify AultCare the next business day if the visit becomes an admission. Medical detox is the usual starting point where physical dependence is established.

Residential and Inpatient

AultCare describes residential care as 24-hour support in a community-based setting with daily clinical check-ins and therapy at least twice weekly, over stays of days to weeks, and inpatient hospital care as the highest level, staffed around the clock by psychiatrists, nurses and therapists. Both need authorization before a planned admission.

PHP, IOP and Outpatient

Partial hospitalization is described as day treatment of three to six hours a day, five days a week, living at home, with individual, group and family therapy and weekly medication checks. Intensive outpatient runs about nine hours a week across several days. Outpatient care, the least intensive level, is sessions once or twice weekly. AultCare’s access standards ask behavioral health providers to see a new patient within 10 business days, handle urgent requests in 24 to 48 hours, and follow up within seven days of a hospital discharge.

Does AultCare Need Prior Authorization for Rehab?

Yes for planned admissions, no for emergencies. The commercial prior authorization list effective September 1, 2026 states that all behavioral health and substance use inpatient admissions, inpatient detox, observation beyond 48 hours, residential treatment, PHP and IOP require prior authorization. Emergency care needs no pre-certification, and a referral for specialists was not required on the Silver 3000 PPO.

Requests run through AultCare’s own utilization management department rather than an outside behavioral health vendor. The admissions team submits through the provider portal using TriZetto Touchless Authorization Processing or an electronic form; the member-facing number is 330-363-6360 or 1-800-344-8858, Monday to Friday 8 a.m. to 4:30 p.m., with pre-certification fax 330-454-9635 and mail to P.O. Box 6910, Canton, OH 44706. Aultra plan questions go to 1-855-270-8497 or 330-363-2050. eviCore stopped processing AultCare requests on April 1, 2026, so an older instruction naming that vendor is out of date.

Which Criteria Does AultCare Use to Review a Rehab Admission?

AultCare applies InterQual guidelines to inpatient care and specific services, and publishes its utilization management internal coverage criteria through an InterQual transparency portal. No ASAM, MCG or LOCUS reference appears in the published material, so the admissions clinician should ask which criteria set the reviewer is reading from for a residential request, and what the review timeframe is. Behavioral health authorization rules for PrimeTime are not published alongside the commercial list; a Medicare Advantage member should ask the plan directly before an admission date is set.

Authorization is not payment. AultCare’s member guide states plainly that obtaining pre-certification does not mean the service will be covered and paid at the highest benefit level, or at all, and the September 2026 list repeats that claims are reviewed when they arrive. Ask the admissions team for the authorization number and the benefit quote in writing.

How Much Does Rehab Cost With AultCare?

It turns on the plan. On the AultCare Silver 3000 PPO summary dated 2025, inpatient and outpatient mental health and substance use care ran at 20% coinsurance in network and 40% out of network, after a $3,000 individual or $6,000 family network deductible and a $9,000 or $18,000 out-of-network deductible.

That plan’s out-of-pocket maximum was $7,600 individual and $15,200 family in network, against $27,600 and $55,200 out of network. An older group plan shows how different an employer product can be: Group Purchasing Plan III in 2022 carried a $200 individual network deductible and paid inpatient mental health and substance use at 10% coinsurance in network, 30% out of network, with preauthorization required.

PrimeTime charges fixed daily amounts rather than coinsurance. For 2026, the Basic MA-Only plan has a $0 premium and $0 deductible, charges $275 per day for days 1 to 6 in an inpatient psychiatric hospital and $0 for days 7 to 90, and caps in-network spending at $3,900. Classic carries a $60 monthly premium, $375 per day for days 1 to 6 and $0 thereafter, a $4,500 in-network maximum and Part D with a $200 deductible. Both list a $35 copay for an individual or group therapy session. Announced 2027 premiums are $0 for Basic MA-Only and Aultimate, $69 for Classic and $115 for Plus.

Those are the sums a program bills without coverage. One extra line matters out of network: charges above the plan’s Reference Based Pricing or usual, customary and reasonable allowance are not a patient’s responsibility with a network provider, but can be with a non-network one, on top of the separate deductible and coinsurance.

Does AultCare Cover Suboxone, Methadone and Other Addiction Medication?

On the Medicare side, yes, and the tiers are published. The PrimeTime 2027 comprehensive formulary places buprenorphine and buprenorphine/naloxone sublingual tablets on Tier 2 and the film on Tier 4, naltrexone 50 mg tablets on Tier 2 and Vivitrol 380 mg injection on Tier 5 specialty, across a five-tier design.

That formulary also lists methadone tablets on Tier 2 and oral solutions on Tiers 2 to 4; Part D pays for methadone written for pain, while dosing inside an opioid treatment program is a Part B service. Acamprosate sits on Tier 4, disulfiram on Tier 3 and naloxone injection on Tiers 2 to 3, with lofexidine not listed. Coverage of Suboxone and buprenorphine or the Vivitrol shot under a commercial AultCare plan is not published in the same way, so ask the plan for the current tier and any step therapy before discharge.

Where the Prescription Is Filled

The pharmacy benefit for commercial plans runs through Optum Rx, with 2026 abridged, comprehensive and Premium Managed formularies and separate step therapy programs. Mail order is 1-866-868-2372, Optum Specialty Pharmacy is 1-855-427-4682 and Gentry Health Services handles specialty fills on 1-844-443-6879.

If a Drug Is Not on the List

Medications needing preauthorization are listed on the AultCare provider site, and a non-formulary drug can be requested through a coverage determination, with a 72-hour standard turnaround and 24 hours for an expedited request. A change of buprenorphine formulation at the counter is a poor moment to find that out, so settle the formulation while the prescriber is still involved.

Which Rehab Centers Are In-Network With AultCare?

A program is in network when it holds an AultCare contract for the particular network printed on the card, which may be the AultCare PPO, the AultCare HMO, the Aultman Employee Network or the PrimeTime network, and when it is licensed by the state to deliver that level of care. Network status is settled in AultCare’s provider directory and on the plan line, not in advertising.

Each listing shows the payments the program says it accepts, its licensing and accreditation, and how far it sits from the nearest airport, so a shortlist can be drawn up before the call that settles contract status.

How Do I Find a Rehab That Takes AultCare?

Match the network named on the ID card, then search the matching AultCare directory, PPO, HMO, Aultman Employee Network or PrimeTime, or call customer service on 330-363-6360 or 1-800-344-8858, Monday to Friday 7:30 a.m. to 5 p.m. Eastern. PrimeTime members call 330-363-7407 or 1-800-577-5084.

Outside the northeast Ohio service area, a leased network named on the card applies instead, among them Cigna PPO, First Health on 800-226-5116, FrontPath, MedCost Preferred, Ohio PPO Connect, Quality Care Partners and MultiPlan and PHCS on 866-972-7427. AultCare tells members to confirm the arrangement with the provider before services begin. A 24-hour nurse line is available on 866-422-9603 or 330-363-7620, and PrimeTime runs its own on 330-363-7600 or 1-855-409-6448; 988 is the route for a mental health emergency and 911 for a medical one.

Is AultCare the Same as PrimeTime Health Plan or Aultman?

Not quite. PrimeTime Health Plan is AultCare’s Medicare Advantage brand, an HMO-POS under CMS contract H3664 that has held a Medicare contract since 1997, rated 4.5 stars overall for the 2026 contract year and offered across 11 counties: Carroll, Columbiana, Harrison, Holmes, Mahoning, Medina, Portage, Stark, Summit, Tuscarawas and Wayne. Aultman Health System, known as Aultman Health Foundation until June 2024, is the nonprofit parent of both, and Aultra is a related employer plan brand with its own network and phone line. University Hospitals has been in network for AultCare commercial and PrimeTime members since December 1, 2020.
