A listing there shows licensing, not network status, so members still ask Florida Blue and the program whether the program takes their plan.

Luxury residential rehab in Florida typically costs $23,000 to $70,000 a month, or about $770 to $2,330 a day, based on Florida price ranges. A Florida Blue plan may pay toward the clinical part of that bill. The setting, the chef and the spa sessions that make a program luxury are usually billed to the client.

Florida Blue is the Blue Cross and Blue Shield licensee for the state, a health insurer owned through GuideWell, a not-for-profit mutual holding company. Its research row spans all 67 counties and lists employer, marketplace, Medicare Advantage, Medicare Supplement and federal employee plans.

The brand has regional branches. Florida Health Care Plans covers Volusia, Flagler, Seminole, Brevard and St. Johns counties, Capital Health Plan covers nine Big Bend counties, and HMO coverage is issued by Florida Blue HMO. Truli for Health sells HMO plans to employers in Central and select South Florida counties.

Luxury programs in Florida often run as small residential programs with private or shared suites, more one-to-one therapy and a home or estate setting. Many add chef-prepared meals, fitness, pools, massage and other wellness sessions, and some arrange airport transfers. Which of these come with the stay is a question for the program’s admissions team.

Does Florida Blue pay for private rooms, chefs and spa treatments?

No, Florida Blue plans pay for covered behavioral health care, not for amenities. A plan that pays for residential treatment pays for the clinical care, and a private room, a chef or a spa session is a charge the client normally covers. Ask for the program’s rate and the plan’s expected payment in writing.

Luxury listings in Florida that accept Florida Blue cluster around Mangonia Park, Juno Beach, West Palm Beach, Gulf Stream and Boca Raton. The nearest airport for the largest share is the Palm Beach airport, followed by Fort Lauderdale-Hollywood, and Orlando International and Miami International also serve the state. A Florida Blue plan may pay for the clinical care at a private residential program, such as detox, residential treatment, therapy and medication, when the stay meets medical necessity criteria. The member pays the deductible, copays or coinsurance, and the client pays for amenities and any charge above what the plan allows.

Does Florida Blue cover luxury rehab in Florida?

Yes, in part. Florida Blue describes behavioral health as covering mental health and substance use disorders, and its member article names behavioral therapy, intensive outpatient or partial hospital care, hospital-based care, residential or inpatient treatment, medications and recovery support. The plan pays for treatment, not the luxury setting.

Who manages Florida Blue’s behavioral health benefit?

Lucet, formerly New Directions Behavioral Health, manages authorizations and case management for Florida Blue members, and it runs the behavioral network. Lucet uses the ASAM Criteria for substance use disorder requests and tells members that their plan may require the provider to get authorization before intensive outpatient, partial hospitalization, residential or inpatient services.

Does Florida Blue require prior authorization for residential rehab?

Yes, usually. Florida Blue’s August 2023 provider bulletin says authorizations through Lucet are required for inpatient services, partial hospitalization and intensive outpatient programs, and Lucet tells members that residential care may need it as well. Routine outpatient care does not. Find out from the program which side submits the request.

Florida Blue states a decision time of 72 hours for urgent pre-service requests and 15 calendar days for non-urgent ones. Without a required authorization, services may not be covered and the member may owe the entire cost or a benefit penalty.

What do copays and deductibles look like on Florida Blue plans?

Plan documents show wide differences. One 2026 myBlue Gold plan lists a $0 deductible, a $60 copay for a physician office visit and $600 per day for hospital inpatient care up to $1,800. A 2026 employer BlueOptions PPO lists a $2,500 individual in-network deductible and a $20 office copay.

An employer BlueCare HMO example charges $1,500 per inpatient day up to $7,500. Copay-style plans charge flat amounts per visit and per day, while deductible plans charge a deductible plus coinsurance for hospital-based behavioral health care. These are plan examples, not a promise for every member.

Lucet’s level-of-care definitions

Lucet describes intensive outpatient treatment as nine to fifteen hours a week while the member lives at home, partial hospitalization as five to seven days a week for six or more hours a day, and residential treatment as temporary facility-based care six to seven days a week. Inpatient hospitalization is round-the-clock care in a hospital.

Who licenses and funds rehab in Florida?

The Florida Department of Children and Families licenses substance use service providers, and its state mental health and substance use program is the single state authority for these services. Florida Medicaid is run by the Agency for Health Care Administration, and most recipients are enrolled in Statewide Medicaid Managed Care plans.

Those plans include Aetna Better Health, Community Care Plan, Florida Community Care, Humana Medical Plan, Molina Healthcare, Simply Healthcare, Sunshine Health and UnitedHealthcare. Florida Blue was not among the plans named in the December 2024 announcement of Medicaid contracts, and Luxury listings do not list Medicaid.

Can Florida Blue pay for an out-of-network luxury rehab?

Yes, sometimes. A BlueOptions PPO plan covers out-of-network care at higher cost sharing, one employer example charging the deductible plus coinsurance, while myBlue and BlueCare HMO plans do not cover out-of-network behavioral health apart from emergencies. Out-of-network providers are not contracted to request authorization, so the member must obtain it.

A program that is out of network can ask for a single case agreement, which lets one stay be paid at in-network benefit levels. The member can ask Florida Blue for a behavioral health case manager and ask whether an agreement is possible.
