What is Recovery Dharma?

Recovery Dharma is a peer-led, Buddhist-inspired program for freedom from addiction of all kinds, based on the Four Noble Truths and the Eightfold Path and using meditation, inquiry, community and ethical living. For substance use it is structured around a commitment to the intention of abstinence.

Is a non-12-step program the same as drug-free treatment?

No. Avoiding the Twelve Steps says nothing about medication. NIAAA's Navigator advises steering clear of any program, physician or therapist who rejects medications out of hand, for example by saying 'We only offer drug-free treatment'. Naltrexone, acamprosate and disulfiram are approved for alcohol use disorder.

It suits anyone who wants a secular frame, or who prefers skills work and medication to a spiritual program, and it is delivered at the same levels of care as any other treatment.

NIAAA describes specialty alcohol treatment at four levels defined by ASAM: outpatient, intensive outpatient or partial hospitalization, residential, and intensive inpatient. Most alcohol use disorder treatment is provided in outpatient settings, and NIAAA advises seeking the least intensive type of care first, moving up only if needed. Residential care can give stability and a structured routine to anyone without stable housing or supportive family or friends.

Non-12-step does not mean drug-free. NIAAA’s Alcohol Treatment Navigator advises steering clear of any program, physician or therapist who rejects medications out of hand, for example by saying “We only offer drug-free treatment”. Some programs cannot prescribe, but they should be willing to work with a doctor who can.

What Happens in a Non-12-Step Program?

A comprehensive assessment comes first, then a treatment plan built from behavioral therapies and, where indicated, medication. NIDA lists cognitive-behavioral therapy, contingency management, motivational enhancement therapy and family therapy among the behavioral therapies for drug treatment; a non-twelve-step program draws on these rather than on twelve-step facilitation.

How Long Does Non-12-Step Rehab Take?

NIAAA states that a course of alcohol use disorder treatment is likely to be measured in months, not days or weeks. NIDA’s Principles of Drug Addiction Treatment states that for residential or outpatient treatment, treatment lasting significantly longer than 90 days is recommended for maintaining positive outcomes. No source sets a separate length for non-twelve-step programs.

Detox is the start rather than the course. NIAAA states that detox alone is not treatment for alcohol use disorder and that continued care in residential or outpatient settings, or both, is often needed, which is why a medical detox admission is normally followed by a step down to PHP or IOP. Peer meetings run on no schedule at all: NIDA notes that mutual help groups, unlike most treatment, are free and not time-limited.

Price tracks the level of care, not the philosophy. Outpatient treatment runs roughly $1,200 to $5,500 a month, IOP $2,500 to $10,000, PHP $7,000 to $15,000 and residential $6,000 to $30,000 a month before coverage. Medicare Part B covers intensive outpatient services when a care plan states at least nine hours of therapeutic services a week are needed, with 20 percent coinsurance after the Part B deductible, in hospitals, community mental health centers, FQHCs, rural health clinics and opioid treatment programs. Medicaid coverage varies greatly by state; MACPAC’s 2018 report found 38 states and the District of Columbia covered at least one residential level, with the IMD exclusion limiting federal payment for adult facilities of more than 16 beds unless a Section 1115 demonstration applies. What Medications Are Used Instead of the Steps?

Three medications are FDA-approved for alcohol use disorder: naltrexone, taken orally each day or by monthly injection, acamprosate, two pills three times a day, and disulfiram, one pill daily. NIAAA states that no specialized training or licensing is needed to prescribe them and that none of them is addictive.

For opioid use disorder, NIDA names methadone, buprenorphine and naltrexone as standard of care, with lofexidine approved for withdrawal symptoms. Only certified opioid treatment programs can dispense methadone for opioid use disorder, while buprenorphine is prescribed in office-based practice. No medications are currently available for stimulant or cannabis use, so treatment there consists of behavioral therapies. SMART Recovery states that it supports the scientifically informed use of legally prescribed psychiatric and addiction medication, and LifeRing treats medically assisted treatment as a decision made with treatment providers.

NIAAA reports that at least one-third of individuals enrolled in alcohol treatment have a co-occurring mental health disorder, most often anxiety or depression, and NIDA says both are usually better treated at the same time.
