How Do I Find a Rehab That Takes Florida Health Care Plans?

Start with the FHCP Find A Doctor/Facility directory, then call the behavioral health line on 386-676-7175 or Member Services on 800-352-9824 and ask for a contracted substance use program in the five-county area. For an admission, the program itself calls Utilization Review on 386-676-7187; an admissions team that will not make that call before the date is a warning sign.

Two other routes exist. ActiveHealth Management runs case management on 855-233-7401, free and voluntary, and covers a behavioral health diagnosis complicated by substance use. Doctor On Demand provides virtual visits with licensed psychologists and psychiatrists, which is the usual way telehealth is delivered on these plans, and the 24-hour nurse advice line is 877-789-2583.

Is Florida Health Care Plans the Same as Florida Blue?

No. Florida Blue acquired FHCP on January 1, 2009, and FHCP sits in the GuideWell family of companies alongside Capital Health Plan, but it remains a separate HMO with its own network, its own member numbers and its own card. A program contracted with Florida Blue is not automatically contracted with FHCP, and the opposite holds too. Nor is this a Medicaid plan: the product lines are individual, employer and Medicare, so Medicaid questions belong with the Statewide Medicaid Managed Care plans on the Florida Medicaid page.

Before you call a program: have the plan name from the card, know the county, and ask whether the program is contracted with FHCP for that product and who will file the authorization with Utilization Review. Those three answers settle the cost before anyone discusses admission dates.
