Residential and Inpatient

Inpatient mental health care is covered with prior authorization, up to 90 days per benefit period on SCAN Classic and SCAN Prime as written in the 2025 San Bernardino County documents. SCAN Connections states no limit on medically necessary hospital days. A hospital psychiatric unit is the setting the plans describe, so Medicare inpatient rehab through a hospital is the likeliest route to an approved bed.

PHP, IOP and Outpatient

SCAN describes partial hospitalization as a highly structured program that stands in for full hospitalization and runs more intensively than ordinary outpatient care. Outpatient covers individual and group visits with a licensed psychiatrist, clinical psychologist or other mental health professional. Intensive outpatient is not named separately in the documents read, so ask how an IOP referral is authorized.

Which SCAN plan is on the card? SCAN Classic, Prime, Venture, MyChoice, Embrace and the Affirm, Inspired and Allied affinity plans are standard HMOs. SCAN Strive Texas for 2026 sets inpatient psychiatric care at $95 a day for days 1 to 5 and therapy at $15. On SCAN Connections, behavioral health in network is $0. Figures differ county by county, which is why a member in California and one in Texas can hold the same plan name and pay different amounts.
