Does Medicaid cover clonidine for opioid withdrawal?

Yes, in practice. Generic clonidine is an inexpensive tablet that state Medicaid drug lists usually treat as an ordinary medication rather than a specialty one. Ohio's 2022 Buckeye Health Plan managed care policy lists clonidine as preferred with no prior authorization, and Michigan's drug list names clonidine tablets among preferred agents in its opioid withdrawal class.

Clonidine is labeled for high blood pressure, so a prescriber uses it off label for withdrawal. The drug cost is rarely the obstacle; the detox benefit around it is.

Does Medicaid cover detox?

In most states, yes. MACPAC reported that detoxification, inpatient, outpatient or both, was covered by 43 states and the District of Columbia as of September 2015, with 32 states covering inpatient detox, 34 covering outpatient detox and 22 covering both. Detox is an optional Medicaid benefit, not a required one.

Required services are narrower: inpatient hospital, outpatient hospital and physician services. Ask your state plan which withdrawal management levels it pays for.

Does Medicaid cover Lucemyra?

Usually with conditions. Lofexidine (Lucemyra) is the FDA-approved medication for opioid withdrawal, but state drug lists commonly place it in the non-preferred tier. North Carolina's list effective January 1, 2026 shows lofexidine and Lucemyra tablets as non-preferred, and Ohio's 2022 managed care policy lists Lucemyra as non-preferred with prior authorization.

North Carolina does not require trial and failure of preferred agents when the diagnosis is opioid withdrawal, so the rules differ sharply from one state to the next.

Does Medicaid pay for residential detox?

Often, but under a federal limit. The institution for mental diseases rule bars federal Medicaid payment for adults aged 21 to 64 in a hospital, nursing facility or other institution with more than 16 beds primarily treating mental diseases. KFF reports 38 states including D.C. had approved Section 1115 waivers for substance use treatment in those settings as of April 2026.

States can also pay for residential treatment in facilities of 16 beds or fewer, which is how many smaller withdrawal programs bill.

What is the IMD exclusion?

It is the Medicaid rule that blocks federal payment for care given to adults aged 21 to 64 inside an institution for mental diseases, meaning a hospital, nursing facility or other institution with more than 16 beds primarily engaged in diagnosing and treating mental diseases. It has shaped where Medicaid detox beds exist for decades.

CMS guidance issued in November 2017 set the terms under which a state can seek a demonstration to pay for short-term inpatient and residential withdrawal care anyway.

Can Medicaid pay for detox outside a hospital?

Yes. Medicaid.gov's ASAM resource guide describes Level 1-WM as outpatient withdrawal management in scheduled sessions and Level 2-WM as daily sessions with extended on-site monitoring, both delivered without an overnight stay. Whether your state pays depends on its plan, since detox is an optional benefit.

In one sample state crosswalk, the ambulatory levels needed only the State Plan, while residential 3.2-WM and inpatient 3.7-WM needed a Section 1115 demonstration.

Is there a copay for clonidine on Medicaid?

Only a nominal one, if any. Medicaid.gov limits prescription drug copayments to nominal amounts for members with income at or below 150% of the federal poverty level. The published base table lists $4.00 for a preferred drug and $8.00 for a non-preferred drug at that income level.

States set their own amounts within those limits, and some charge nothing at all for medications given during a covered withdrawal stay.

How much is clonidine without Medicaid?

Generic clonidine 0.1 mg tablets run about $5.40 for 30 tablets and $6.55 for 60 tablets in cash prices listed by Drugs.com. Lucemyra is far costlier, with GoodRx showing prices starting around $352.32 for 96 tablets. The medication is the cheap part of withdrawal management.

A supervised detox stay is the larger expense, usually $1,500 to $5,600 a week before insurance, which is why the detox benefit matters more than the drug tier.

Is clonidine approved for opioid withdrawal?

No. The ASAM 2020 guideline states clonidine is not FDA-approved for opioid withdrawal and has been used off label for that purpose for more than 25 years. The Catapres label lists hypertension as the indication and names no withdrawal use. Lofexidine, approved in May 2018, is the labeled option.

Off-label prescribing is lawful and routine, and Medicaid pays for clonidine as an ordinary covered drug regardless of the reason it is written.

How long does opioid withdrawal last?

Withdrawal from short-acting opioids begins within 12 hours of a missed dose, peaks at 36 to 72 hours and tapers over the next 4 to 7 days, according to a 2019 review. That timeline is why a medically supervised withdrawal stay is usually measured in days rather than weeks.

Clonidine's effect peaks at 3 to 5 hours with a half-life of 12 to 16 hours, so doses are spread through the day during that window.

What are the side effects of clonidine in detox?

The Catapres label reports dry mouth in about 40 per 100 patients, drowsiness in about 33 and dizziness in about 16. Clonidine can also slow the heart rate, particularly alongside agents affecting the sinus node, and it may add to the effects of alcohol, barbiturates and other sedating drugs.

Lofexidine shares the same pattern of orthostatic low blood pressure, fainting, slow pulse and sleepiness, which is why pulse and blood pressure checks are built into visits.

Does Medicaid cover medications after detox?

Yes, in every state. CMS says section 1006(b) of the SUPPORT Act, signed October 24, 2018, requires state Medicaid plans to cover medication-assisted treatment for opioid use disorder. The Congressional Research Service notes the Consolidated Appropriations Act, 2024 removed the September 30, 2025 end date, making the requirement permanent.

MACPAC also found medication-assisted treatment covered by all states and the District of Columbia, which is the stage that follows withdrawal management.
