The 50% rule. On the Colorado PPO and EPO plan summaries, a missed prior authorization for inpatient behavioral care cuts benefits by half of the total cost of the service. Before an admission date is agreed, get the authorization reference number and the approved start date in writing from whoever filed it.

How Much Does Rehab Cost With All Savers?

It turns on the plan design the employer bought. On the Colorado PPO P50030e plan the network deductible is $1,000 individual and $2,000 family, with an out-of-pocket limit of $3,000 and $6,000 in network and $6,000 and $12,000 out of network. The EPO E500i80LX plan runs a $500 and $1,000 deductible with a $3,000 and $6,000 limit.

High-deductible designs change the arithmetic entirely: the HSA HP6650 plan carried a $6,300 individual and $12,600 family deductible, four prescription tiers with copays applied after that deductible, and mail order charged at 2.5 times the retail copay. Those figures are from plan documents dated around 2020 and are examples, not a schedule that applies to every group.

Those are the sums a program bills without coverage, and they are what an out-of-network stay exposes a household to on an EPO design, which pays nothing outside the network. Where no plan coverage applies at all, self-pay rehab and sliding scale pricing are the usual routes, and admissions teams negotiate single-case rates more often than the published figures suggest.

Does All Savers Cover Suboxone, Methadone and Other Addiction Medication?

Pharmacy runs through OptumRx. On UnitedHealthcare’s 2026 Prescription Drug List, effective September 1, 2026, buprenorphine-naloxone film and tablets and Zubsolv are Tier 2 with a quantity limit and no prior authorization flag, while buprenorphine sublingual, oral naltrexone, acamprosate and oral disulfiram sit at Tier 1.

Naloxone and Narcan nasal spray are Tier 1 with a quantity limit. Two things are not settled by that list: which drug list a given All Savers group uses, and how methadone is handled, since no methadone or opioid treatment program policy appeared in All Savers material. Injectable naltrexone was not found in the fetched section either, so a clinic planning a long-acting injection should price it with the plan first.

Where the Prescription Is Filled

Network pharmacies and mail order are arranged through OptumRx, and All Savers runs a pharmacy services line on 855-816-6618. Tier placement, quantity limits and any step rule are checked against the drug list named in the group’s own materials rather than a general formulary, because self-funded groups do not all buy the same one.

Medication Given Inside a Program

Medication administered during an inpatient or residential stay is billed by the program under the medical benefit, so it falls inside the facility coinsurance rather than appearing as a pharmacy fill. A prescription filled after discharge moves to the OptumRx tiers above, and that handover is worth confirming with the clinical team before the discharge date.

A program is in-network when it holds a contract with the UnitedHealthcare network the specific plan uses, which may be Choice, Choice Plus, Core, Navigate or Charter, and sits in the Optum Behavioral Health network behind it. Two network details catch admissions out. Access to the Choice, Choice Plus and Core networks excludes Mayo Clinic facilities, and regional options differ: the MAPFRE network serves US Virgin Islands residents and American Health Network covers select Indiana counties. Virtual care through HealthiestYou, included with these plans, offers phone or video visits around the clock and counseling seven days a week on the California level-funded design.

Is All Savers the Same as UnitedHealthcare or Level Funded?

All Savers Insurance Company is a UnitedHealthcare company, domiciled in Indiana under NAIC number 82406. An SEC subsidiary list places it under Golden Rule Insurance Company and Golden Rule Financial Corporation, which UnitedHealth Group acquired on November 13, 2003. UnitedHealthcare Level Funded is the current product name for the same arrangement, and Surest Level Funded and Oxford Level Funded are separate siblings with their own member lines.

The underwriter also changes by state.
