Treatment Centers in Oklahoma That Accept SoonerCare

SoonerCare, Oklahoma’s Medicaid program, is how the Oklahoma Health Care Authority covers drug and alcohol rehab. Many members belong to one of three SoonerSelect health plans, which carry behavioral health inside the plan, while others stay on fee-for-service SoonerCare. Detox, residential treatment, outpatient counseling and opioid use disorder medications are all covered benefits.

Oklahoma programs below show a direct phone line, the care they run, withdrawal management through outpatient groups, and the name of a CARF or Joint Commission accreditor if one applies. Oklahoma certification data and government treatment registers stand behind each entry; pick a town or service to cut the page down.

Does SoonerCare Cover Rehab in Oklahoma?

Yes. SoonerCare covers drug and alcohol rehab in all 77 Oklahoma counties: medical detox, residential substance use disorder treatment, outpatient counseling and opioid treatment programs. Oklahoma expanded Medicaid on July 1, 2021 after State Question 802 passed, so adults aged 19 to 64 earning up to 138 percent of the poverty level can join the Medicaid program statewide.

SoonerCare is run by the Oklahoma Health Care Authority, which since April 1, 2024 has enrolled children, parents, pregnant members and expansion adults in SoonerSelect, its managed care system. Its three SoonerSelect health plans, Aetna Better Health of Oklahoma, Oklahoma Complete Health and Humana Healthy Horizons in Oklahoma, each keep behavioral health in-house rather than using an outside vendor.

Does SoonerCare Cover Inpatient Rehab in Oklahoma?

Yes. SoonerCare covers residential treatment programs for substance use disorder from age 13, with prior authorization, in the Traditional, Choice and expansion adult packages. Oklahoma’s rule for adults aged 21 to 64, OAC 317:30-5-95.45, revised September 11, 2023, covers medically necessary residential care for members who meet the clinical criteria, and children follow the companion rules.

How does SoonerCare pay for larger residential programs?

SoonerCare pays for adult stays in larger psychiatric and addiction facilities through Oklahoma’s Institutions for Mental Disease waiver, a federal Section 1115 demonstration that took effect December 22, 2020. Medicaid.gov lists it under a temporary extension through December 31, 2026 while the Oklahoma Health Care Authority’s request to renew it through 2030 is pending, with no planned change to benefits.

Does SoonerCare need prior authorization for inpatient rehab?

Yes. SoonerCare requires prior authorization for residential treatment, inpatient detox and opioid treatment programs. Each residential admission or extension request needs a placement on the state’s chosen ASAM tool, done within seven days and scoring all six dimensions. Among the plans, Humana decides inpatient behavioral health requests within 24 hours, and Aetna decides urgent requests within 24 hours.

Does SoonerCare Cover Detox in Oklahoma?

Yes. SoonerCare covers inpatient medical detox taking Medicaid with prior authorization in the Traditional, Choice and expansion adult packages, and adults pay a small daily copay with a cap for the stay. Oklahoma Complete Health’s benefit list names withdrawal management, while the Oklahoma Department of Mental Health and Substance Abuse Services (ODMHSAS) funds urgent recovery clinics and crisis stabilization units.

Does SoonerCare Cover Outpatient Rehab in Oklahoma?

Yes. SoonerCare covers outpatient rehab programs, individual, group and family counseling, and peer recovery support. Fee-for-service outpatient care is authorized by level of care using the Client Assessment Record and the Addiction Severity Index, and electronic requests are processed within five business days. One diagnostic assessment a year and crisis intervention need no prior authorization.

SoonerSelect plans cover partial hospitalization taking Medicaid for at least three hours on each of five days a week, plus day treatment and Certified Community Behavioral Health Clinic services. Behavioral health needs no primary care referral in any of the three plans, though Humana alone lists outpatient behavioral health as needing no prior authorization.

Does SoonerCare Cover Suboxone, Methadone and Other Addiction Medications?

Yes. SoonerCare has covered methadone, naltrexone, buprenorphine and every other FDA-listed medication for opioid use disorder since October 1, 2020, with counseling, in opioid treatment programs and office-based settings. The Oklahoma Health Care Authority said Oklahoma was the first Medicaid agency to win federal sign-off for medications for addiction treatment in both settings.

Does SoonerCare cover methadone?

Yes. SoonerCare covers methadone medication clinics, and every opioid treatment program visit is prior-authorized, whether the member is fee-for-service or in any of the three SoonerSelect plans. ODMHSAS certifies opioid substitution treatment programs, so a clinic billing SoonerCare holds that state certification as an organization.

Does SoonerCare cover Suboxone and Vivitrol?

Yes. SoonerCare plan handbooks for 2026 name Suboxone programs taking Medicaid and the Vivitrol shot for Medicaid among covered medications; Humana lists buprenorphine with naloxone. Fee-for-service adults have a monthly prescription limit, while Humana’s 2026 handbook drops the monthly count though it still allows only two brand-name fills a month.

SoonerCare Rehab Centers Near Lakes, Rivers and the Ouachita Mountains

SoonerCare pays the same way in every corner of Oklahoma, because all three SoonerSelect plans and fee-for-service SoonerCare are statewide rather than regional. A member choosing a program near a reservoir in the northeast or in the southeastern hills keeps the same plan, the same benefits and the same prior authorization rules.

SoonerCare rehab near Grand Lake and Lake Eufaula

SoonerCare members drawn to Oklahoma’s big reservoirs can browse rehabs near lakes around Grand Lake in Delaware County and Lake Eufaula in McIntosh County. Listings include treatment options in Grove on Grand Lake’s shore, where residential care is authorized on the same ASAM basis used in Oklahoma City.

SoonerCare rehab in the Ouachita Mountains

SoonerCare covers members in Le Flore and McCurtain counties, where the Ouachita National Forest covers the ridges around Talihina and Heavener. Programs near national forests and rehab services in Poteau serve this area, where an Indian Health Care Provider is open to tribal members without a referral, even outside the plan network.

SoonerCare rehab along the Illinois River

SoonerCare members in Cherokee County live along the Illinois River, which runs past Tahlequah toward Lake Tenkiller. Riverside treatment programs and recovery programs in Tahlequah serve the area, and American Indian and Alaska Native members can leave SoonerSelect at any time and return to their earlier coverage.

How Do I Find a Rehab That Accepts SoonerCare in Oklahoma?

Browse the listings on this page, which hold SoonerCare-billing programs across Oklahoma, then sort by town, level of care or substance. Every entry has its own admissions number to dial first. A separate page holds Medicaid rehab listings from across the country.

SoonerCare members need a prior authorization before a residential bed, inpatient detox or opioid treatment program can start, issued by their SoonerSelect plan or, outside a plan, by the Oklahoma Health Care Authority. Programs normally send that request themselves, and members may call the line printed on their card. Each arrangement and its line is below.

SoonerCare rehab in Oklahoma City

SoonerCare is run from Oklahoma City, where the Oklahoma Health Care Authority sits on North Lincoln Boulevard and ODMHSAS on North Classen Boulevard. Members can browse Oklahoma City rehab programs and treatment centers in Midwest City under whichever plan their card names.

SoonerCare rehab in Tulsa and Broken Arrow

SoonerCare members in Tulsa County live within the reservations of the Cherokee, Muscogee and Osage nations, and American Indian and Alaska Native members there may opt into SoonerSelect or stay on fee-for-service. Either way they can use addiction treatment in Tulsa or Broken Arrow rehab programs billed to SoonerCare.

SoonerCare rehab in Norman and Moore

SoonerCare expansion adults in Cleveland County, as everywhere in Oklahoma, face federal work requirements from January 1, 2027. Half-time study counts toward the 80 monthly hours, and people in a qualifying addiction treatment program are exempt, which matters for anyone entering Norman treatment programs.

SoonerCare rehab in Lawton, Enid and smaller cities

SoonerCare members in Comanche and Garfield counties use the same statewide plans as members in the two big metros, and telehealth by audio and video is covered for counseling and medication management. Members can browse rehab centers in Lawton or Enid recovery services and pair local visits with remote sessions.

Can I use SoonerCare for rehab in a different state?

Rarely, and only with prior authorization. SoonerCare plans pay out-of-network care only when it is authorized, and Aetna’s guide says providers more than 50 miles beyond the Oklahoma border need prior authorization. Humana and Oklahoma Complete Health arrange outside care when their networks lack a provider, and Humana lets new members finish up to 90 days with a current provider.

How much does rehab cost with SoonerCare?

SoonerCare charges no monthly premium, and copayments for non-pregnant adults are capped at five percent of household income. Fee-for-service adults pay small copays for outpatient care, inpatient days and prescriptions, with none for children under 21. Humana’s 2026 handbook lists no behavioral health copays, and Aetna’s waives them for adults aged 21 to 64.

How do I change my SoonerSelect plan?

SoonerSelect members can switch plans within their first 90 days, then at open enrollment, which ran May 1 to June 12 in 2026. Members who make no choice keep their plan, and the Oklahoma Health Care Authority assigns one to anyone who never picks. The SoonerCare Helpline, 800-987-7767, handles plan changes and applications.

SoonerCare also covers aftercare and continuing care through peer recovery support and counseling after a residential stay, and the plans report 7-day and 30-day follow-up after substance use treatment as 2026 quality measures. Oklahomans facing a mental health or drug and alcohol emergency can reach 988 by call or text, day or night.
