Sublocade Rehab That Takes Insurance

Insurance status sits in its own SAMHSA field, and almost every site offering extended-release injectable buprenorphine also records private coverage. Filtering on it produces a page that overlaps heavily with the main Sublocade listing instead of surfacing something new. Read it that way.

The injection is billed differently from a prescription filled at a pharmacy, often under the medical benefit, so ask the program how that works before your first appointment. The main Sublocade page sets out the same providers with more on how the treatment runs.

Why Trust RehabSeekers.com?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

Does insurance cover Sublocade?

Yes, private health plans commonly cover Sublocade for moderate to severe opioid use disorder when their criteria are met. Aetna and UnitedHealthcare both publish coverage policies for the monthly buprenorphine injection, and every Marketplace plan must cover substance use disorder treatment as an essential health benefit.

Coverage rules still differ plan by plan, so the diagnosis, the dose and whether prior authorization is needed all get checked before the first injection.

Sublocadehttps://rehabseekers.com/medications/mat/buprenorphine/sublocade/
Does Sublocade need prior authorization with private insurance?

Often yes. Indivior tells practices that some insurers require prior authorization for Sublocade, and both Aetna and UnitedHealthcare publish criteria a request must meet. A 2025 review found 22 states had some prohibition on prior authorization for opioid use disorder medications in private plans by 2023.

If prior authorization is denied the injection can still be given, but the member is then responsible for the full cost of the medication.

Suboxone and Buprenorphine Rehab That Takes Insurancehttps://rehabseekers.com/medications/mat/suboxone-and-buprenorphine-insurance/
Does UnitedHealthcare cover Sublocade?

Yes. UnitedHealthcare's commercial policy effective October 1, 2026 covers Sublocade or Brixadi when the patient tolerated a buprenorphine test dose or is already taking buprenorphine and the dosing follows the FDA label. Initial authorization runs up to 12 months at a time.

For continuation the plan asks for a documented positive clinical response and label dosing, again with authorization of no more than 12 months.

A Sublocade rehab that takes insurance is a treatment program that gives the once-monthly buprenorphine extended-release injection and bills a private health plan for it rather than asking for cash up front. The injection is prepared and given by a healthcare provider, so the program needs both a clinician who can inject it and a route to obtain the drug.

The coverage question splits into two parts that are easy to confuse. One is whether the plan pays for the medication, which usually runs through prior authorization and a published policy. The other is whether the plan pays for the appointment where the injection is given, which is billed separately when a specialty pharmacy supplies the drug.

This guide covers what Sublocade is and who it is labeled for, what happens at a monthly injection visit, how long treatment runs, what it costs and what private plans pay, how Aetna and UnitedHealthcare write their criteria, what to do with a denial, and how to find a program near you that injects it and bills insurance.
