Does Medicaid cover Sublocade?

Yes, in nearly every state. A KFF survey of state Medicaid fee-for-service programs as of July 1, 2022 found 45 states covering injectable buprenorphine and none reporting it was not covered. Federal law requires state Medicaid plans to cover FDA-approved medications for opioid use disorder.

CMS interprets that requirement to include every FDA-approved form of methadone, buprenorphine and naltrexone, with the monthly injection described as one of those forms.

Is the Medicaid requirement to cover opioid use disorder medication permanent?

Yes. The benefit took effect October 1, 2020 under the SUPPORT Act, which was signed on October 24, 2018, and a Congressional Research Service summary says the Consolidated Appropriations Act, 2024 removed the September 30, 2025 expiration date, making the requirement permanent.

New Hampshire's Medicaid agency, for example, states that effective October 1, 2025 medication-assisted treatment is a permanent benefit in its state plan.

Does Sublocade need prior authorization on Medicaid?

It varies by state and plan. CMS allows states to use preferred drug lists and prior approval for mandated drugs. KFF's 2022 survey reported prior authorization for Sublocade in Indiana and for injectable buprenorphine in Louisiana, Montana and Vermont, while other states apply none.

New York Medicaid's list revised September 14, 2026 says no prior authorization is needed for substance use disorder medications prescribed per national professional guidelines.

How fast does Medicaid answer a prior authorization request?

Within 24 hours for a covered outpatient drug billed through the pharmacy benefit, and MACPAC says a 72-hour emergency supply must be dispensed while the decision is pending. When the injection is billed under the medical benefit, managed care plans decide standard requests within 14 days and expedited ones within 72 hours.

MACPAC also notes that an approval is not binding and usually states it does not guarantee payment.

What happens if Medicaid denies my Sublocade?

You can appeal. MACPAC says Medicaid members have the right to appeal a prior authorization denial, and the process may include external medical review or a state fair hearing. A denial notice from a managed care plan such as CareSource comes with instructions on appealing and on requesting that hearing.

A drug left off a formulary must still be reachable through a prior authorization program, and a medically indicated mandated drug must be covered.

Either one, depending on the state and plan. An Oregon Medicaid plan notice says Sublocade and Brixadi are covered under the medical benefit through buy and bill or under the pharmacy benefit. Some states set one. Colorado Medicaid's January 1, 2026 list caps Sublocade at 600 mg in the first month and 300 mg a month after that, which matches the label. Yes, where the program can both bill Medicaid and have a provider give the injection. Sublocade is prepared and administered only by a healthcare provider, and the medication must come from a REMS-certified setting or pharmacy. Ask whether the program injects on site or refers you elsewhere.

A prescriber can refer you to an additional site of care, where a DEA-registered practitioner gives the injection while the prescriber keeps managing treatment.

The SUPPORT Act was signed on October 24, 2018 and the mandatory benefit began on October 1, 2020. It was due to expire on September 30, 2025, but a Congressional Research Service summary says section 201 of the Consolidated Appropriations Act, 2024 removed that expiration, making the requirement permanent. New Hampshire’s Medicaid agency, as one state example, states that effective October 1, 2025 coverage of medication-assisted treatment is a permanent benefit in its state plan.

A KFF survey of state Medicaid fee-for-service programs as of July 1, 2022 found 45 states covering injectable buprenorphine, with none reporting it was not covered. CMS’s own letter describes Sublocade as a once-monthly injection delivering buprenorphine at sustained levels and notes that long-acting injections may improve adherence and reduce accidental exposure, theft or misuse. Coverage of the drug sits alongside the rest of Medication Assisted Treatment (MAT) under the same mandate.

Covered does not mean unrestricted. CMS says states may still seek rebates and use preferred drug lists and prior approval for mandated drugs that are covered outpatient drugs. Where a mandated drug is not a covered outpatient drug, a state may apply utilization management but must still cover it where it is medically indicated for the enrollee.

How Does Sublocade Work on Medicaid?

The prescriber writes for Sublocade, the clinic obtains it through a certified route, and a healthcare provider injects it under the skin in the abdomen, thigh, buttock or back of the upper arm, rotating sites each month. The liquid forms a solid depot that releases buprenorphine across the month, with maintenance injections given at least 26 days apart.

The label indicates Sublocade for moderate to severe opioid use disorder in patients who have taken a single dose of a transmucosal buprenorphine product or are already on buprenorphine, used as part of a complete treatment program that includes counseling and psychosocial support. It is not appropriate for anyone opioid naive. The starting dose is 300 mg, the second injection is 300 mg, and maintenance is 100 mg monthly, with 300 mg monthly considered where the response is not satisfactory.

Who Can Prescribe and Inject It

Any practitioner with a DEA registration that includes Schedule III authority may prescribe buprenorphine for opioid use disorder, with no federal waiver or patient cap, where state law permits. Indivior says a prescriber can refer a patient to an additional site of care, where a DEA-registered practitioner gives the injection while the prescriber continues overall treatment. That split is common in outpatient MAT clinics and is worth asking about before the first appointment.

Pharmacy Benefit or Medical Benefit

An Oregon Medicaid plan notice says Sublocade and Brixadi are covered either under the medical benefit through buy and bill or under the pharmacy benefit. Colorado Medicaid’s January 2026 preferred drug list says long-acting injectables for mental health or substance use disorders given by a healthcare professional need no place-of-service prior authorization and may be billed to either benefit. Which route a clinic uses changes who files the authorization, so the admissions team should say which applies.

Does Sublocade Need Prior Authorization on Medicaid?

It depends on the state and the plan. KFF’s 2022 survey reported prior authorization for Sublocade in Indiana and for injectable buprenorphine in Louisiana, Montana and Vermont. New York Medicaid’s preferred drug list revised September 14, 2026 lists Sublocade and says prior authorization is not required for substance use disorder medications prescribed per national professional guidelines, and an Oregon plan notice dated January 29, 2025 says Sublocade is available without it.

Dose rules are the other common control. Colorado Medicaid’s January 1, 2026 list sets a maximum of 600 mg in the first month and 300 mg a month after that. The 2022 survey noted Florida capped Sublocade at 300 mg every 30 days, Virginia required service authorization, and the District of Columbia required prior authorization above the FDA maximum, though MACPAC reports that plans in the District cannot require prior authorization for medications to treat opioid use disorder generally. A University of Washington guide says Washington State Medicaid typically covers injectable buprenorphine without prior authorization or limits, while Montana requires it. KFF says that as of state fiscal year 2024 most states no longer require prior authorization for at least one standard buprenorphine formulation under fee-for-service, though they often keep it for higher doses.

How Fast Must Medicaid Answer?

MACPAC says fee-for-service programs and managed care plans must answer a prior authorization request for a covered outpatient drug within 24 hours and dispense a 72-hour supply in an emergency. Where the injection is billed under the medical benefit, managed care plans must decide standard requests within 14 days and expedited ones within 72 hours; CMS had set no federal timeline for fee-for-service medical requests as of 2024.

CareSource’s Medicaid page says a 72-hour override may be used in an emergency so a member can get a medicine while authorization is pending. MACPAC also notes that an approval is not binding and usually states that it does not guarantee payment, which is why clinics confirm again at the appointment.

If it is denied. MACPAC says Medicaid enrollees have the right to appeal a prior authorization denial, and the process may include external medical review or a state fair hearing. CareSource says that where an authorization is not approved it sends information on how to appeal and on the right to a state hearing. Under the drug rebate rule, MACPAC adds, any drug left off a formulary must still be reachable through a prior authorization program.

How Long Does Medicaid Authorize Sublocade For?

Where a plan sets a term, twelve months is the common one. UnitedHealthcare’s Community Plan medical benefit drug policy effective October 1, 2026 covers Sublocade or Brixadi for opioid use disorder where the patient had a tolerated test dose or is on buprenorphine and dosing follows the label, with initial authorization up to 12 months.

Renewal under that policy requires a documented positive clinical response and label dosing, again for no more than 12 months at a time. The policy does not apply in nine listed states, and for Arizona, Indiana and Pennsylvania it points to the state’s own Medicaid clinical policy instead. The label itself sets no maximum duration of maintenance treatment and says that for some patients treatment may continue indefinitely, so the renewal cycle is a plan rule rather than a clinical endpoint. Ongoing buprenorphine maintenance and continuing care usually run alongside it.

How Much Does Sublocade Cost With Medicaid?

Very little. Medicaid.gov says most Medicaid out-of-pocket charges are limited to nominal amounts, with children, terminally ill individuals and individuals residing in an institution exempt, and no charges for emergency, family planning or pregnancy-related services. KFF’s 2022 state table lists copayments for injectable buprenorphine of $3 in Colorado and $4 in North Carolina.

Medicaid.gov’s table of base amounts lists a maximum $4.00 copay for preferred drugs and $8.00 for non-preferred drugs for individuals at or below 150 percent of the poverty level. Indivior reports that almost 90 percent of individuals with Medicaid pay $0 for Sublocade and most of the rest pay $1 to $4 a month, drawn from claims between September 2022 and August 2023. In the 2022 KFF survey, 13 states reported a copayment for injectable buprenorphine and 15 reported some limit or utilization control.
