Utah Medicaid covers substance use disorder treatment across the full ASAM continuum under a federal 1115 waiver granted on November 9, 2017, from early intervention and outpatient counseling to residential care, medically managed inpatient treatment and opioid treatment programs. Programs among the rehab programs across Utah show which of those levels each one offers.

Utah Medicaid splits each member’s care between two plans. Physical health sits with one of four accountable care organizations (Molina Healthcare, SelectHealth Community Care, Healthy U or Health Choice Utah), and behavioral health for nearly everyone else goes to the home county’s Prepaid Mental Health Plan. Rehabs that bill it appear under Medicaid rehab coverage.

Which Utah counties use an integrated Medicaid plan?

Five counties do, for Adult Expansion members only. Since January 1, 2020, Utah Medicaid Integrated Care has combined physical health, mental health and substance use benefits in a single plan in Weber, Davis, Salt Lake, Utah and Washington counties. The plans are Integrated Health Choice, Integrated Healthy U, Integrated Molina and Integrated SelectHealth; Healthy U skips Washington County.

Does Utah Medicaid Cover Inpatient Rehab in Utah?

Yes. Utah Medicaid covers clinically managed residential treatment at ASAM levels 3.1, 3.3 and 3.5, medically monitored intensive inpatient care at 3.7 and medically managed inpatient addiction treatment at 4.0. Residential rehab programs with 16 or fewer beds qualify, and larger licensed substance use residential programs are covered through the waiver’s exception for institutions for mental disease.

How long will Utah Medicaid pay for residential rehab?

Utah Medicaid sets residential stay length by clinical need, not by a set number of days. The state’s 2017 substance use implementation protocol sets no fixed statewide cap and uses ASAM criteria for placement and continued stay. Utah’s request to renew its waiver for 2027 to 2032 asks to keep residential substance use treatment in larger facilities.

Does Utah Medicaid need prior authorization for inpatient rehab?

Yes. Under the Utah Medicaid provider manual, the Prepaid Mental Health Plan or integrated plan must authorize a residential substance use stay first, and the 2026 Member Guide adds that hospital medical detoxification may need it too. The provider, not the member, sends the request. Wasatch County has no Prepaid Mental Health Plan, so no county authorization step applies there.

Does Utah Medicaid Cover Detox in Utah?

Yes. Utah Medicaid covers withdrawal management at ambulatory levels 1-WM and 2-WM and at clinically managed residential detox programs, level 3.2-WM. Medical detoxification in a hospital falls to the physical health plan, an integrated plan or the Fee for Service Network rather than the county plan, so medical detox taking Medicaid follows that plan’s rules.

Does Utah Medicaid Cover Outpatient Rehab in Utah?

Yes. Utah Medicaid covers level 1.0 outpatient counseling, level 2.1 intensive outpatient and level 2.5 day treatment, which Utah also calls partial hospitalization, and the 2026 Member Guide lists individual, group and family therapy, medication management and peer support. Utah Medicaid charges no copay for outpatient mental health or substance use treatment. Behavioral health visits can be held by telehealth when clinically appropriate, and Utah Medicaid pays for rides to outpatient substance use services through its transportation program, booked with ModivCare at 1-855-563-4403 three business days ahead. The county plans themselves do not cover those rides.

Does Utah Medicaid Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Opioid treatment programs give Utah Medicaid members methadone, buprenorphine or naltrexone as part of the state’s substance use continuum. Under the 2017 protocol, people with alcohol or opioid use disorders must be told about these medications, and local authorities must assess for them within 10 days. Does Utah Medicaid cover methadone?

Yes, outside the county plan. Instead of the Prepaid Mental Health Plan, methadone is billed to whichever physical health or integrated plan the member has, or to fee-for-service, and Optum Salt Lake County’s May 2025 member handbook says opioid treatment programs bill Utah Medicaid directly. Does Utah Medicaid cover Suboxone?

Yes. Utah Medicaid covers buprenorphine through opioid treatment programs and the pharmacy benefit. Fee-for-service members follow the Utah Medicaid Preferred Drug List, reissued monthly with prior authorization and quantity limits marked and last dated October 1, 2026, while ACO members use their own plan’s drug list. Utah Medicaid Rehab Centers Near Ski Resorts, Red Rock Canyons and National Forests

Utah Medicaid follows the member’s county of residence, not the setting of the program, so a stay in the mountains or canyon country is authorized the same way as one in Salt Lake City. What changes from region to region is the county plan that authorizes behavioral care and whether joining an ACO is required or optional.

Summit County Medicaid members, from Park City to Kamas and Coalville, get behavioral health through Healthy U Behavioral, which University of Utah Health Plans has run since September 1, 2019 for every Medicaid-eligible resident whatever their medical plan. Utah Medicaid rehab in red rock canyon country

Grand, Carbon and Emery counties, which take in Moab, Price, Helper and the canyons of the San Rafael Swell, share one county Prepaid Mental Health Plan reached at 435-637-7200 or 855-489-9102. Joining an ACO is optional there. Rehabs near canyons and Price drug and alcohol programs are authorized through that same county line.

Utah Medicaid rehab near the Uinta Basin national forests

Daggett, Duchesne and Uintah counties, between the Ashley National Forest and the High Uintas, use a single county plan reached at 844-824-6776, with local offices in Vernal at 435-789-6300 and Roosevelt at 435-725-6300. Salt Lake County Medicaid members outside the integrated plans get mental health and substance use care through Optum Salt Lake County, which works under contract to the county’s Division of Behavioral Health Services. Its member line, 1-877-370-8953, is open weekdays from 8 a.m. to 5 p.m. for Salt Lake City treatment programs and West Valley City programs.

Utah Medicaid rehab in Provo, Orem and Utah County

Utah County is both an ACO-mandatory county and an integrated county, so Adult Expansion members there pick an integrated plan that pays for addiction treatment directly. Other Utah County members use the county local authority, listed at 801-373-4760 or 866-366-7987, for behavioral health at Orem addiction treatment programs and treatment centers in Provo.

Utah Medicaid rehab in Ogden, Layton and Davis County

Weber and Morgan county members outside Adult Expansion are served by the Weber plan, on 801-625-3700, with hospital authorizations at 801-625-3757. Davis County, covering Layton, Kaysville and Bountiful, runs its own plan at 801-773-7060. Both Weber and Davis are integrated counties for Adult Expansion, and listings include drug rehabs in Ogden and Layton treatment options.

Utah Medicaid rehab in St. George and Washington County

Washington County Adult Expansion members choose Integrated Health Choice, Integrated Molina or Integrated SelectHealth, because Integrated Healthy U is not offered there. Other members in Washington, Iron, Beaver, Garfield and Kane counties use the southwest county plan, whose hospital line is 435-705-1388, for St. George rehab programs and Cedar City treatment centers.

Can I use Utah Medicaid for rehab in a different state?

Usually not. Routine care in another state is not covered, and out-of-state urgent or emergency care is paid only when the provider enrolls as a Utah Medicaid provider and accepts the Medicaid payment in full. Inside Utah, plan members must use their plan’s network, and the county of residence, which a member cannot change, sets the county plan.

How much does rehab cost with Utah Medicaid?

Very little. Outpatient mental health and substance use treatment carries no copay for anyone, and no copay of any kind applies to Targeted Adult Medicaid members, anyone under 18, pregnant members or American Indian and Alaska Native members. Other non-pregnant adults owe a small copay for each inpatient hospital stay, and the 2026 Member Guide lists no premium or deductible.

Will the 2027 work requirement affect someone in rehab?

No, treatment counts as an exemption. Starting January 1, 2027, Adult Expansion and Targeted Adult Medicaid members aged 19 to 64 need 80 monthly hours of work, job training, half-time school or community service, and Utah exempts anyone enrolled in drug or alcohol treatment. The same adults move to eligibility renewals every six months from that date.

Utah Medicaid is not the only public route into care. Targeted Adult Medicaid covers very low income adults who are chronically homeless, justice-involved or need treatment, though the state proposes folding it into Adult Expansion on June 30, 2027.
