Do rehabs allow smoking?

Some do. A smoking friendly program permits tobacco, usually in a designated outdoor space, while keeping indoor areas smoke-free. Policies differ program by program: some set scheduled breaks, others limit smoking to supervised periods, and staff may hold cigarettes for residents between those times.

Is smoking allowed inside a rehab facility?

Rarely. State clean indoor air laws govern indoor smoking, and smoking friendly programs almost always restrict tobacco to outdoor areas. Why do some rehabs let people smoke?

Staff and patients often believe that quitting tobacco during substance use treatment will trigger a return to other substances, a belief described in research on residential programs. Enforcement is also difficult, particularly where staff smoke themselves, so a designated area becomes the practical compromise.

Are there laws requiring rehabs to go tobacco-free?

Only in some states. The Public Health Law Center records a number of states as having no law or regulation on tobacco-free grounds for mental health and substance use programs, so the policy is set by the facility rather than by statute in those places.

Does ventilation make an indoor smoking room safe?

No. The Surgeon General concluded that separating individuals who smoke from those who do not, cleaning the air and ventilating buildings are not effective protections against secondhand smoke. The CDC adds that no engineering approach can fully eliminate the risk.

What health effects does secondhand smoke cause?

Secondhand smoke can cause coronary heart disease, stroke and lung cancer in adults, and respiratory infections, ear infections and asthma attacks in children, according to the CDC. Exposure is widespread: an estimated 58 million nonsmoking Americans were exposed during 2013 to 2014.

Does smoking affect clozapine?

Yes. The FDA clozapine label states that tobacco smoke is a moderate inducer of the CYP1A2 enzyme, and that drugs inducing CYP1A2 can lower clozapine plasma concentrations, reducing effectiveness. The label advises monitoring and considering a dose increase in that situation.

A meta-analysis estimated that about 100 and 200 mg a day in nonsmokers corresponds to roughly 200 and 400 mg a day in smokers.

Yes. The FDA olanzapine label states that clearance of olanzapine is about 40 percent higher in smokers than in nonsmokers, though dosage modifications are not routinely recommended. One study of 354 adult psychiatric patients found smoking increased olanzapine elimination 1.23-fold.

Which medicines are affected by smoking?

Medicines broken down by the CYP1A2 enzyme are the main group: caffeine, clozapine, fluvoxamine, olanzapine, tacrine and theophylline. A systematic review also found lower plasma levels in smokers for quetiapine, risperidone, escitalopram, amitriptyline, doxepin, mirtazapine, verapamil and metronidazole.

Not everything changes. The same review found no significant difference between smokers and nonsmokers for lamotrigine and loxapine.
