Treatment Centers in Minnesota That Accept Medica

Medica, the Minnetonka-based nonprofit health plan, covers drug and alcohol rehab for Minnesotans on employer, individual, Medicare and Medical Assistance plans, and since January 1, 2026 it also runs the UCare-branded Medicaid and individual plans it acquired. Medica Behavioral Health, administered by Optum, arranges substance use care through a contracted network.

A Medica member reading these listings sees each program’s phone, its care levels, which can span withdrawal management, residential treatment, day treatment and outpatient groups, and any accreditation earned from CARF or The Joint Commission. Information comes from public registers and licenses the state of Minnesota issues. Use the filters, then ask which Medica networks the program joins.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Does Medica Cover Rehab in Minnesota?

Yes. A Medica Choice Passport certificate written for Minnesota employers lists substance use benefits as outpatient treatment and diagnostic evaluation, intensive outpatient programs, medication-assisted treatment with counseling, partial programs, and inpatient or residential treatment with medical supervision. Programs billing rehabs that accept Medica appear among the rehab centers across the state here.

Medica is a Minnesota nonprofit, and Minnesota held about 68 percent of its members before the UCare transfer. Medica agreed on November 17, 2025 to take over UCare’s 2026 Medicaid and individual plans, which kept the UCare name for 2026. Medica sells Medical Assistance plans only in Minnesota, under the Choice Care, DUAL Solution and AccessAbility Solution names.

Do UCare members go through Medica for rehab in 2026?

Yes, behind the scenes. UCare Community Health Plan issues the 2026 UCare-branded Medical Assistance and individual plans, and Medica assumed its contracts and assets from January 1, 2026. UCare members keep their UCare card and call the number on it, and their benefits and provider networks did not change for 2026.

Does Medica Cover Inpatient Rehab in Minnesota?

Yes. Medica Behavioral Health’s provider manual lists hospitalization and residential substance use programs among the services it coordinates, and the City of Duluth’s 2026 self-insured Medica plan covers residential treatment as part of inpatient addiction services. Covered behavioral care must diagnose or treat a disorder in the current DSM.

How long will Medica pay for residential rehab?

Medica publishes no residential day count and ties length to medical necessity, so each added week depends on clinical progress. Medica Behavioral Health, run by Optum, makes those decisions, and Optum uses The ASAM Criteria for substance use and publishes Minnesota-specific clinical criteria. UCare-branded Medicaid residential stays also get concurrent checks under Minnesota’s Section 1115 waiver.

Does Medica need prior authorization for inpatient rehab?

Sometimes. Medica’s administrative manual says prior authorization may be required for substance use programs and residential treatment, while in-network hospital admissions need only notification. The Minnesota Choice Passport certificate requires prior authorization for most behavioral inpatient services, and an emergency admission needs the facility to notify Medica as soon as reasonably possible.

Does Medica Cover Detox in Minnesota?

Yes, inside inpatient and residential substance use care. Optum, which administers Medica’s behavioral benefits, manages residential detox as a level of care, and an in-network hospital detox admission falls under Medica’s notification rule. Medical detox programs in Minnesota hold a withdrawal management license from the Department of Human Services.

Does Medica Cover Outpatient Rehab in Minnesota?

Yes. Medica’s Minnesota certificate defines intensive outpatient treatment as three or more hours a day, up to 19 hours a week, and covers it with outpatient therapy and diagnostic evaluation. Medica’s manual says most outpatient behavioral services need no prior authorization, and individual, family and group therapy are all covered.

Medica’s partial programs, which the certificate defines as at least four hours a day or 20 hours a week, sit between intensive outpatient and residential care. For Medical Assistance members with complex substance use disorders, Medica has run a six-month intensive program with a community partner, using both harm reduction and abstinence-based approaches.

Does Medica Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Medication-assisted treatment with counseling is a listed substance use benefit on Medica’s Minnesota HSA certificate, at 20 percent coinsurance after the deductible in network. Medica’s individual drug lists cover the prescriptions, and MNsure links Medica’s 2026 Minnesota formulary for members comparing addiction medications before choosing a plan during open enrollment.

Does Medica cover Suboxone?

Yes. Medica’s 2025 Iowa and Minnesota individual drug list puts generic buprenorphine-naloxone, in both film and tablet form, on tier 2, some strengths with a quantity limit, and shows no prior authorization flag. At Suboxone programs that take insurance, the counseling falls under Medica’s behavioral benefit, while the prescription runs through the pharmacy list.

Does Medica cover methadone?

Yes, for Medica Medical Assistance members at an Opioid Treatment Program, a service the state’s health care programs manual covers, though the state does not pay for guest dosing at a second clinic. On the Choice Passport HSA certificate, medication-assisted treatment with counseling is a behavioral benefit, so in-network methadone clinic programs fall under the plan’s coinsurance.

Medica Rehab Centers Near Brainerd’s Lakes, the Cannon River and Buck Hill

Medica’s individual plans are organized by network region, so the county a program sits in decides which Medica network includes it. Applause covers the whole state, while Engage covers 19 southern counties and other networks, several co-branded with regional health systems, cover the metro area, the northwest or the north.

Medica rehab in the Brainerd lakes area

Medica members in Crow Wing County live around Brainerd and Baxter and the Gull and Whitefish lakes. One of Medica’s northern individual networks covers twelve counties, Crow Wing among them, and Applause covers it too, so rehabs in lake country and Brainerd treatment programs can fall in either network.

Medica rehab along the Cannon River

Medica members in Rice County live along the Cannon River, which runs through Faribault and Northfield toward the Mississippi. Medica began offering Medical Assistance there on January 1, 2025, and Rice is also in the Engage network, so riverside rehab programs and Faribault rehab centers fit both lines.

Medica rehab near Buck Hill

Medica members in Dakota County live near Buck Hill, the ski and snowboard hill on the south edge of Burnsville. Dakota was one of nine counties where Medica added Medical Assistance on January 1, 2025, so rehabs near ski areas and downtown Burnsville programs are open to Choice Care members as well as employer and individual plan members.

How Do I Find a Rehab That Accepts Medica in Minnesota?

Browse the listings on this page. Entries carry phone numbers, services and accreditation, and you can sort them by town, insurer or level of care. Tell the program which Medica product you hold, such as Applause, Engage, Choice Care or a UCare-branded plan, because the network differs between them.

Medica admission to residential or substance use program care may need prior authorization, which the program requests from Medica Behavioral Health at 1-800-848-8327 through Optum. Medical Assistance members start with a comprehensive assessment at the program they pick, under Minnesota’s Direct Access rules. The table lists Medica’s 2026 Minnesota lines.

Medica rehab in Minneapolis and Minnetonka

Medica is headquartered in Minnetonka, and its behavioral claims and provider searches run through Optum’s liveandworkwell.com site. Hennepin County members can buy Applause or a nine-county metro network on MNsure, and Medica Advantage Solution PPO serves Hennepin for Medicare, reaching Minneapolis rehab programs and Minnetonka treatment centers.

Medica rehab in Saint Paul and Ramsey County

Medica added Medical Assistance Families and Children and MinnesotaCare in Ramsey County on January 1, 2025. When UCare left Ramsey on September 1, 2025, Medica was already one of the plans those members could choose, and it now runs UCare’s remaining plans too, covering care at Saint Paul rehab centers.

Medica rehab in Rochester and Olmsted County

Medica began Medical Assistance and MinnesotaCare in Olmsted County on January 1, 2025, and Olmsted is part of the 19-county Engage individual network. Medica’s behavioral manual notes special rules for City of Rochester and Olmsted County employee plans, so those members ask their benefits office before choosing treatment programs in Rochester.

Medica rehab in Duluth and Saint Cloud

Medica’s twelve-county northern network includes St. Louis County, and the City of Duluth’s 2026 self-insured Medica plan treats residential care as inpatient. Further south, Medica added Medical Assistance in Stearns County in January 2025 and Benton County that September, covering Duluth recovery programs and Waite Park rehab centers.

Can I use Medica for rehab in a different state?

Yes, on many Medica employer plans. Medica Choice Passport PPO plans pay out-of-network behavioral care at a higher member share, 50 percent after the deductible on the Minnesota HSA plan, and out-of-network admissions typically need authorization. Individual plans tie to their Minnesota network region, and Medical Assistance pays providers enrolled with Minnesota Health Care Programs.

How much does rehab cost with Medica?

Medica costs follow the plan. The Minnesota Choice Passport HSA certificate charges 20 percent coinsurance after the deductible in network for substance use office visits, intensive outpatient, medication-assisted treatment and inpatient care, and 50 percent out of network, where balance billing may also apply. Medical Assistance had no copays in 2025.

Medica also ended one product for 2026: its integrated Special Needs BasicCare plan, AccessAbility Solution Enhanced, closed in all 38 Minnesota counties on January 1, 2026, and those members moved to other coverage. Members who need help paying can also look at Medicaid rehab programs statewide.
