Treatment Centers in Minnesota That Accept HealthPartners

HealthPartners, a nonprofit plan based in Bloomington, covers drug and alcohol rehab through job-based plans, individual plans sold directly and on MNsure, Medicare plans and Medical Assistance and MinnesotaCare plans in eleven Twin Cities and central Minnesota counties. Its own Utilization Management team handles substance use authorizations, with no outside behavioral health company in between.

Listings give a phone number for each program, the kinds of care it runs, for example detox, residential treatment, day treatment or outpatient counseling, and any CARF or Joint Commission credential. Data come from government treatment registers and Minnesota license files. Pick a city or service, then ask about your HealthPartners network.

Does HealthPartners Cover Rehab in Minnesota?

Yes. A HealthPartners Minnesota small employer benefits chart covers outpatient substance use assessment and therapy, day treatment, intensive outpatient care, opiate replacement therapy, inpatient hospital care, residential treatment and detoxification in licensed programs. Rehabs that accept HealthPartners appear among the Minnesota addiction treatment listings here.

HealthPartners is a Minnesota company, headquartered on 33rd Avenue South in Bloomington, and it also runs clinics and hospitals in Minnesota and western Wisconsin. Its plans include employer coverage for organizations in five states, Minnesota and Wisconsin individual plans, Medicare Advantage, Cost and Supplement plans, and four Minnesota public program plans. Its materials often call substance use care chemical health.

Does HealthPartners cover rehab for Medical Assistance members?

Yes. HealthPartners Families and Children covers people under 65 and families with children on Medical Assistance, and HealthPartners MinnesotaCare covers lower-income Minnesotans who do not qualify for it. Under Minnesota’s Direct Access rules the member picks a program for a comprehensive assessment, and plan-authorized residential room and board is billed to the state.

Does HealthPartners Cover Inpatient Rehab in Minnesota?

Yes. The HealthPartners small employer chart covers inpatient hospital substance use services and primary residential treatment in licensed centers. HealthPartners’ authorization form lists high, medium and low intensity residential addiction treatment, matching Minnesota’s ASAM residential levels, plus hospital-based addiction care as separate levels it reviews.

How long will HealthPartners pay for residential rehab?

HealthPartners’ small employer chart describes primary residential treatment as typically 30 days or less, but the actual length follows clinical need under the plan’s medical coverage criteria. Continued stays go back through HealthPartners Utilization Management, which asks for current clinical records, so a Minnesota program sends updates as treatment goes on and the plan decides each extension.

Does HealthPartners need prior authorization for inpatient rehab?

Yes, for the codes on its list. HealthPartners’ Authorization for Substance Use Disorder Services form covers detox, residential, partial hospitalization and intensive outpatient care, and tells providers to include only codes that need authorization. Requests need a comprehensive substance use disorder assessment, collateral information, medication records and withdrawal scores, faxed to (952) 853-8830.

Does HealthPartners Cover Detox in Minnesota?

Yes. HealthPartners covers detoxification in licensed facilities on its small employer chart, and its authorization form separates residential detox from hospital detox. State law licenses residential detox programs under chapter 245F, and the HealthPartners form asks for withdrawal scores and medication administration records so the plan can match the level of care.

Does HealthPartners Cover Outpatient Rehab in Minnesota?

Yes. The HealthPartners small employer chart covers outpatient assessment, individual, group and family therapy, and day treatment in licensed programs, plus intensive outpatient programs. Video, e-visit and telephone services are covered at the matching benefit level, and two 2024 HealthPartners employer summaries charged the same outpatient behavioral copay as a primary care visit.

HealthPartners lists partial hospitalization programs on its substance use authorization form, so a program asks Utilization Management before the first day. For questions about whether a counselor or program is in a member’s network, HealthPartners Behavioral Health Navigators answer at 952-883-5811 or 888-638-8787, Monday to Friday.

Does HealthPartners Cover Suboxone, Methadone and Other Addiction Medications?

Yes. The HealthPartners small employer chart lists opiate replacement therapy, meaning methadone or buprenorphine, as a covered outpatient substance use service. HealthPartners publishes 2026 and 2027 drug lists for group, individual and Medical Assistance plans, which set the cost of medications for addiction treatment filled at a pharmacy.

Does HealthPartners cover Suboxone?

Yes, as buprenorphine opiate replacement therapy. Each plan’s formulary sets the tier, and HealthPartners says formulary exception requests generally get an answer within one business day. Suboxone treatment programs serving Medical Assistance members follow HealthPartners’ separate Medical Assistance drug list and its prior authorization criteria.

Does HealthPartners cover methadone?

Yes. Methadone is named on the HealthPartners small employer chart as opiate replacement therapy, given through licensed programs. For HealthPartners Medical Assistance members, the state route to methadone dosing clinics is a licensed Opioid Treatment Program, and the state’s Minnesota Health Care Programs manual does not cover guest dosing at another clinic.

HealthPartners Rehab Centers Near the St. Croix River, the Chisago Lakes and the Southwest Prairie

HealthPartners plans are tied closely to counties. Its individual networks each cover a set list, its Medical Assistance plans run in eleven counties, and its Medicare Advantage plans serve the Twin Cities and central Minnesota. A program’s location therefore decides which HealthPartners product can pay for it for a given member, more than with a national carrier.

HealthPartners rehab along the St. Croix River

HealthPartners members in Washington County live along the St. Croix River from Stillwater to Afton, across from Wisconsin’s St. Croix and Polk counties, where HealthPartners also sells a Wisconsin state employee plan. Rehabs near rivers and Lake Elmo treatment programs sit in the Select ACO and Peak individual network areas.

HealthPartners rehab in the Chisago Lakes area

HealthPartners Medical Assistance members in Chisago County, around Chisago City, Center City and Lindstrom, live among the Chisago Lakes. Chisago is one of the eleven counties where HealthPartners offers Families and Children and MinnesotaCare, so treatment centers near lakes and Center City rehab programs sit inside that service area.

HealthPartners rehab near Pipestone’s falls

HealthPartners’ Cornerstone individual network covers ten southwestern Minnesota counties, including Pipestone, Rock, Nobles and Lyon, where Winnewissa Falls drops through the quartzite at Pipestone. Rehabs near waterfalls and Luverne addiction programs fall inside that network’s area for Cornerstone members, while Medical Assistance members in those counties are with another plan.

How Do I Find a Rehab That Accepts HealthPartners in Minnesota?

Browse the listings on this page. Minnesota entries carry a phone number, services and accreditation, and filters narrow by city, level of care and insurer. When calling, name your HealthPartners network, such as Select ACO, Peak, Cornerstone, Alpine or a Medical Assistance plan, since each network covers a different set of programs.

HealthPartners admission to detox, residential, partial hospitalization or intensive outpatient care goes through HealthPartners Utilization Management at (952) 883-7501; the program phones in with the comprehensive assessment. Medical Assistance members’ requests go to State Public Programs Medical Management at 952-883-6333. The table shows HealthPartners’ 2026 Minnesota plans by area.

HealthPartners rehab in Minneapolis and Bloomington

HealthPartners’ Select ACO individual network, its narrowest, is built around Hennepin, Ramsey and four other metro counties, and Hennepin is also in its Medical Assistance service area. Members in the network can choose South Minneapolis treatment programs or rehab programs in Edina, near the company’s Bloomington headquarters.

HealthPartners rehab in Saint Paul and Ramsey County

HealthPartners closed its Medical Assistance plans to new members on December 1, 2024, then reopened Families and Children and MinnesotaCare enrollment for 2026 in the same counties, Ramsey included. New members in Ramsey County can again choose HealthPartners, and HealthPartners also offers Minnesota Senior Care Plus and Senior Health Options there for care at Saint Paul addiction treatment programs.

HealthPartners rehab in Saint Cloud and Stearns County

HealthPartners extends its Peak individual network and its Medical Assistance plans to Stearns and Benton counties, and adds Sherburne for Minnesota Senior Care Plus. Members there can use rehab centers in Saint Cloud or Sauk Rapids programs within those plans, and HealthPartners Medicare Advantage also reaches central Minnesota, so seniors in the St. Cloud area have a Journey plan option.

HealthPartners rehab in Worthington and Marshall

HealthPartners’ Cornerstone network reaches Nobles and Lyon counties, home to Worthington and Marshall, along with Chippewa, Cottonwood, Jackson, Lincoln, Murray and Yellow Medicine. Cornerstone individual members in southwest Minnesota can look at Worthington recovery programs; HealthPartners does not offer Medical Assistance in those counties for 2026.

Can I use HealthPartners for rehab in a different state?

Yes, on employer plans. A December 2024 Cigna guide says HealthPartners cardholders use the Cigna network outside the HealthPartners service area, except in Iowa, Nebraska and South Dakota. Individual and Medical Assistance members stay within their own network counties, and the small employer chart pays non-network behavioral care at 50 percent of charges.

How much does rehab cost with HealthPartners?

HealthPartners costs follow the plan’s deductible, copay and coinsurance. On its HSA Gold MN small employer plan, network substance use care is paid in full after the deductible and non-network care at 50 percent. HealthPartners says its Medical Assistance plans are low or no cost, and Minnesota Senior Care Plus has no premium and no cost sharing.

HealthPartners is one of several Minnesota plans that pay for rehab, and its Behavioral Health Navigators can explain a plan’s benefits before admission. For gold, silver and bronze individual plans, HealthPartners notes that gold pays about 80 percent of costs. Medicaid-covered rehab programs are another route for eligible members.
