What states does UniCare operate in?

UniCare is a legacy health plan brand of Elevance Health, and the two plan lines that carried the name now trade as Wellpoint. For anyone holding a card or a benefit book that still says UniCare, the first task before calling an admissions team is working out which plan it is, because the addiction treatment rules differ sharply between the Massachusetts public-employee plans and the West Virginia Medicaid plan.

The name goes back to 1996, when WellPoint Health Networks bought MassMutual’s Life and Health Benefits Management Division for $402.2 million and renamed it UNICARE Life & Health Insurance Company. The Massachusetts Group Insurance Commission plans became Wellpoint on July 1, 2024, with new ID cards mailed that June and no change to plan design or doctors. UniCare Health Plan of West Virginia, Inc. became Wellpoint West Virginia, Inc. around January 1, 2025, keeping the same network, benefits, member ID numbers and customer service number. unicare.com still runs for employer group, vision, life and disability and employee assistance products, with coverage underwritten by Wellpoint Life and Health Insurance Company.

Does UniCare Cover Drug and Alcohol Rehab?

Yes. The Massachusetts GIC plans cover mental health and substance use disorder services through the Carelon Behavioral Health network, from medically managed inpatient withdrawal through residential care to outpatient therapy. In West Virginia the Medicaid and WVCHIP plan covers inpatient and outpatient behavioral health and substance use treatment plus case management, with no referral needed for most behavioral health services.

The parent is Elevance Health, which was called Anthem, Inc. until June 2022, and the successor brand is Wellpoint. Employer group products sold on unicare.com publish no behavioral health benefit detail and list no behavioral health phone line, so anyone on one of those plans should ask the employer’s benefits office for the summary of benefits and the name of the behavioral health administrator before an assessment is booked.

Which UniCare is on the card? The GIC plans are Wellpoint Total Choice (indemnity), Wellpoint PLUS (tiered), Wellpoint Community Choice (Massachusetts only, excluding Nantucket and Martha’s Vineyard) and Wellpoint Medicare Extension (a Medicare supplement, not Medicare Advantage). Total Choice and PLUS are open to GIC members whose primary residence is in Massachusetts, New Hampshire, Rhode Island, Maine, Vermont or Connecticut. The West Virginia plan is Medicaid and WVCHIP.

What Does the Plan Pay For at Each Level of Care?

The Wellpoint PLUS handbook lists the full ladder and maps it to ASAM levels of care: partial hospitalization at 2.5, clinical stabilization services and dual diagnosis acute treatment at 3.5, acute residential withdrawal management at 3.7 and medically managed inpatient detox at level 4. Medical necessity decides which rung applies at assessment.

Medical Detox

Acute residential withdrawal management and medically managed inpatient substance use services are both named as covered on PLUS, which is the plan language for medical detox. West Virginia Medicaid added withdrawal management under the state’s 1115 substance use waiver on July 1, 2018. Alcohol and benzodiazepine withdrawal can produce seizures, and an emergency department is the right destination during an acute episode whatever the network answer turns out to be.

Residential and Inpatient

Acute residential treatment, clinical stabilization services, dual diagnosis acute treatment, inpatient psychiatric care, crisis stabilization units and transitional care units all appear on the PLUS list, so residential rehab sits squarely inside the benefit. The same waiver brought adult residential treatment under ASAM criteria into West Virginia Medicaid, alongside peer recovery support.

PHP, IOP and Outpatient

Partial hospitalization, intensive outpatient, structured outpatient addiction programs, medication-assisted treatment, community support programs, outpatient therapy, acupuncture withdrawal management and substance use assessment and referral are all covered outpatient services on PLUS, and the handbook states there are no visit limits for most behavioral health services. The plan also covers DBT, TMS, in-home behavioral services and mobile crisis intervention, the last at no member cost.

Does UniCare Need Prior Authorization for Rehab?

In Massachusetts, no for inpatient substance use treatment at contracted facilities in the state and for outpatient substance use programs, but the plan must be notified within 48 hours. Inpatient substance use care outside Massachusetts, or at a non-contracted facility, does need preapproval. The behavioral health preapproval line is 800-442-9300.

Mental health services on the preapproval list are different and worth reading closely: acute residential treatment, transitional care units, inpatient psychiatric care outside Massachusetts, crisis stabilization stays longer than five days, ABA, DBT, family stabilization teams, partial hospitalization and TMS. Where preapproval is required and not obtained, benefits may be reduced or not paid at all, which is the single most expensive administrative mistake available on these plans.

What Happens With Out-of-Network Rehab?

It depends on the plan. Total Choice is an indemnity plan: any doctor and any hospital are covered on the same terms, in or out of network. On PLUS, a non-preferred behavioral health provider leaves the member paying 20% coinsurance with balance billing allowed, while preferred providers accept the allowed amount as payment in full. The summary of benefits also states that preauthorization is not required for outpatient services from providers licensed by the Massachusetts Department of Public Health.

How Much Does Rehab Cost With UniCare?

On Wellpoint PLUS for the 2026–2027 plan year, the deductible is $500 individual and $1,000 family, the in-network out-of-pocket maximum is $5,000 and $10,000, inpatient behavioral health carries a $275 facility copay per calendar quarter with no charge for professional services, and outpatient behavioral health is $10 per visit with the deductible waived. Out-of-network inpatient care runs the deductible plus 20% coinsurance.

Medication-assisted treatment, substance use assessment and referral and emergency service programs carry no member cost on PLUS, and acupuncture withdrawal management is $20. Virtual behavioral health visits are $10 with the first three free. Outpatient behavioral health is $20 on Total Choice and Community Choice. On Medicare Extension, contracted visits one to four are free and then $5–$10, with a $1,000 out-of-pocket maximum; non-contracted care is 20% coinsurance for visits one to fifteen and 50% after, capped at $3,000. West Virginia Medicaid charges no copays for behavioral health services; other services run $0–$75 per visit by income tier with quarterly household maximums of $8–$143. Those are the sums a program bills without coverage. With a GIC plan the member share is the copay and coinsurance above, bounded by the annual out-of-pocket maximum, which is why the notice and preapproval steps matter more than the sticker price.

Does UniCare Cover Suboxone, Methadone and Other Addiction Medication?

Yes. Medication-assisted treatment is covered at no member cost on Wellpoint PLUS and on Medicare Extension, and the handbook summary places methadone treatment under that heading. West Virginia Medicaid has covered methadone treatment with counseling, SBIRT and naloxone since January 14, 2018 under the state’s substance use waiver.

Where the Prescription Is Filled

CVS Caremark runs the pharmacy benefit for Total Choice, PLUS and Community Choice on 877-876-7214, using the Performance Drug List and the Advanced Control Specialty list; SilverScript covers Medicare Extension. The PLUS prescription deductible is $100 per person and $200 per family, with retail and mail copays of $10/$25 at tier 1, $30/$75 at tier 2 and $65/$165 at tier 3. Tier placement and any authorization rules for buprenorphine and naltrexone are not stated in the plan handbook, so check the drug list before discharge rather than at the pharmacy counter.

Medication on West Virginia Medicaid

Prescriptions are carved out to the state’s pharmacy vendor, Gainwell Technologies, on 888-483-0797, rather than handled by the plan. The plan itself covers medicines given during a hospital stay and injectables administered in an office, which is the route a long-acting formulation usually takes.

Which Rehab Centers Are In-Network With UniCare?

A program is in-network when it holds a contract for the specific product on the card, which for the Massachusetts plans means a Carelon Behavioral Health contract and, for outpatient substance use care, a license from the Massachusetts Department of Public Health. Status is settled in Carelon’s behavioral health directory, described as more than 50,000 contracted providers, or in the West Virginia plan’s Find a Doctor tool.

Each listing shows the payments the program says it accepts, its licensing and accreditation, and how far it sits from the nearest airport, so a shortlist can be drawn up before the call that settles contract status.

How Do I Find a Rehab That Takes UniCare?

GIC members call 833-663-4176 for Total Choice, PLUS and Community Choice, covering both medical and behavioral health, or 800-442-9300 for Medicare Extension; lines run Monday to Thursday 7:30 a.m. to 6 p.m. and Friday to 5 p.m. West Virginia Medicaid and WVCHIP members call 800-782-0095, with a 24-hour nurse line on 888-850-1108 and the 988 Suicide and Crisis Lifeline for a behavioral health emergency.

Active GIC-eligible employees and dependents also have Mass4YOU, the Employee Assistance Program run by Optum on 844-263-1982, which includes addiction recovery and crisis support and sits separate from the health plan. Programs across Massachusetts and West Virginia can be compared first, then taken to member services for the contract answer.

Is UniCare the Same as Wellpoint, Anthem or Blue Cross Blue Shield?

UniCare and Wellpoint are the same plans under a new name in Massachusetts and West Virginia, and both sit under Elevance Health. Anthem Blue Cross and Blue Shield is a sister brand holding the Blue license; UniCare is the non-Blue brand and no Blue Cross Blue Shield license for it was found. HealthLink, whose operations were combined with UniCare’s in December 2005, is a separate brand again.

Before admission. Have the plan name, not just the brand, ready; ask the admissions team whether the program is contracted with Carelon for that plan; ask who files the 48-hour notice or the preapproval request, and get the reference number. UniCare in Illinois and Texas ended with the exit announced in late October 2009, so a card from those states is no longer live coverage.
