Treatment Centers in Kentucky That Accept Anthem

Kentucky’s Blue Cross company, Anthem Blue Cross and Blue Shield, pays for drug and alcohol rehab for people with its job-based coverage, its kynect individual plans or its Medicare plans. Anthem no longer runs a Kentucky Medicaid plan: its contract ended on January 1, 2025, and its Medicaid members moved to Humana or UnitedHealthcare.

Anthem members can open a listing to read the program’s number, learn whether it offers detox, residential beds or an intensive outpatient track, and spot any Joint Commission or CARF accreditation. Each record traces back to Kentucky licensing data or a federal treatment register. Narrowing the list by town or type of care first will save calls.

Does Anthem Cover Rehab in Kentucky?

Yes. Anthem pays for substance use care for Kentuckians on its employer, kynect and Medicare plans; Anthem’s individual-plan certificates from other states spell out inpatient facility care, residential treatment, day programs, intensive outpatient and office visits as covered substance use services. The Kentucky treatment centers listed here include rehabs that accept Anthem.

Anthem’s Kentucky plans are issued by Anthem Health Plans of Kentucky, Inc., which uses the Anthem Blue Cross and Blue Shield name. Its parent, Elevance Health, carries the Blue license in Kentucky and thirteen more states. Anthem sells 2026 individual plans through kynect, Kentucky’s official marketplace, and sells Medicare plans in Kentucky as well.

Does Anthem still offer Kentucky Medicaid?

No. Anthem Kentucky Managed Care Plan, Inc. stopped serving Kentucky Medicaid members on January 1, 2025, after a Franklin Circuit Court order, and Elevance’s latest annual report leaves Kentucky off its Medicaid list. Members kept their eligibility and were reassigned to Humana or UnitedHealthcare. Anthem’s old Medicaid line still appears on some state pages.

Former Anthem Medicaid members who are in treatment now get authorization from Humana Healthy Horizons or UnitedHealthcare Community Plan, whichever the state assigned. Kentucky now lets members switch among its five Medicaid plans at any time, through kynect at (855) 459-6328 or the other state routes, with the change starting the first day of the next month.

Does Anthem Cover Inpatient Rehab in Kentucky?

Yes. Anthem covers residential rehab programs and inpatient addiction treatment when they are medically necessary and precertified. Anthem individual certificates from 2024 in other Anthem states require precertification for inpatient and residential substance use care, and Anthem uses MCG Behavioral Health Care Guidelines for precertification and stays.

How long will Anthem pay for residential rehab?

Anthem decides residential length case by case under its utilization criteria rather than a stated number of days. Carelon Behavioral Health, Anthem’s sister company in Elevance, applies the ASAM Criteria for substance use services where it manages the benefit, and Kentucky members should look at their own plan’s certificate for any limits.

Does Anthem need prior authorization for inpatient rehab?

Yes. Anthem’s individual-plan certificates make precertification a condition for facility-based substance use care, from a residential stay or day program to intensive outpatient or an inpatient admission. A Kentucky program can request it from Anthem, or the member can call the member services number on the Anthem card, which differs by plan.

Does Anthem Cover Detox in Kentucky?

Yes. Detox falls under the inpatient and outpatient facility benefits listed in Anthem’s individual-plan certificates, so medical detox for insured members in Kentucky follows the same precertification as other facility stays. A Kentucky member’s own certificate or employer booklet sets the cost share and any network limits.

Does Anthem Cover Outpatient Rehab in Kentucky?

Yes. Anthem covers outpatient office visits for substance use, and its certificates list insurance-covered intensive outpatient care and partial hospitalization as separate covered levels that need precertification. Anthem’s Kentucky individual-plan page promotes in-person and virtual behavioral health services, and says video visits in the Sydney Health app cost nothing on most plans, though some plans apply the deductible.

Anthem members can also see therapists and psychiatrists by video through LiveHealth Online, 888-548-3432, which serves Anthem members in its Blue states. Anthem HMO individual certificates in other states do not pay for out-of-network substance use care except emergencies and urgent care, so Kentucky HMO members should stay with network programs.

Does Anthem Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Anthem employer, kynect and Medicare plans in Kentucky generally handle prescriptions through CarelonRx, Elevance’s pharmacy benefit manager (formerly IngenioRx), which manages the drug list. A January 2017 settlement in New York led Anthem to drop its approval step for buprenorphine across most of its plans nationwide. That source is older, so go by the current Kentucky drug list.

Does Anthem cover methadone?

Anthem methadone coverage depends on the plan, and none of the Kentucky Anthem documents in our sources names it. Kentucky licenses methadone clinics as narcotic treatment programs under 908 KAR 1:370, so an Anthem member considering methadone maintenance programs should ask Anthem how the plan pays such a clinic.

Does Anthem cover Suboxone?

Yes, on most plans. Anthem’s 2017 change removed prior authorization for Suboxone through insurance on most affiliated plans, and the listings also show Vivitrol covered by insurance. The exact tier and any limits depend on the Kentucky plan’s own CarelonRx drug list.

Anthem Rehab Centers Near Herrington Lake, in the Beaver Creek Wilderness and in the Eastern Kentucky Mountains

Anthem’s Kentucky network stretches across the state, so for an Anthem member a rural lake or hill-country program raises one question: is it in network? Kentucky is one of the fourteen states where Elevance holds the Blue license, so the same Anthem rules apply from Paducah to Pikeville.

Anthem rehab near Herrington Lake

Anthem employer, kynect and Medicare members in Boyle, Mercer and Garrard counties live around Herrington Lake, the deep Dix River reservoir between Danville, Harrodsburg and Lancaster, south of the Kentucky River palisades. Rehab programs near lakes and Boyle County’s treatment options in Danville follow the same Anthem precertification steps as programs in Louisville.

Anthem rehab near the Beaver Creek Wilderness

Anthem members in McCreary and Pulaski counties live near the Beaver Creek Wilderness, a protected tract of the Daniel Boone National Forest north of Whitley City, where Beaver Creek drains north toward Lake Cumberland. Members there can compare wilderness-area rehab programs with downtown Somerset programs under Anthem’s network rules.

Anthem rehab in the eastern Kentucky mountains

Anthem members in Breathitt and Magoffin counties live in the hollows of the Cumberland Plateau around Jackson and Salyersville. Members there can compare rehab centers in the mountains and Salyersville treatment programs, then ask the program about its Anthem network status before a long drive to a residential bed.

How Do I Find a Rehab That Accepts Anthem in Kentucky?

Browse the listings on this page; they already show only programs that accept Anthem. Choose a town or service to trim the list; each program’s page carries its number, care levels and accreditation. Tell the program whether you have an employer, kynect or Medicare plan, since networks and benefits differ by plan.

Anthem must precertify a residential stay, hospital admission, day program or intensive outpatient course. The program usually requests it after assessing the member, using the number printed on the Anthem card. Former Anthem Medicaid members now go through Humana or UnitedHealthcare for authorization. The lines below handle authorization and benefits, not the choice of program.

Anthem rehab in Louisville

Anthem’s Kentucky individual-plan page names Louisville first among the cities its kynect coverage reaches, alongside Lexington, Frankfort, Bowling Green and Owensboro, and Jefferson County members can buy those plans through kynect with subsidies when their income qualifies. Drug rehab programs in Louisville and Highlands treatment services are listed.

Anthem rehab in Lexington

Anthem names Lexington among its Kentucky individual coverage areas, so Fayette County residents can shop Anthem plans on kynect. Fayette County families who once had Anthem Medicaid now hold Humana or UnitedHealthcare cards unless they changed plans. Lexington addiction treatment and programs near Chevy Chase appear in the listings.

Anthem rehab in Frankfort

Anthem lists Frankfort among its Kentucky coverage areas, and the Franklin Circuit Court in the capital issued the order that ended Anthem’s Medicaid contract. Anthem members in Franklin County use the same precertification as members elsewhere for Frankfort treatment centers and nearby programs.

Anthem rehab in Bowling Green and Owensboro

Anthem names Bowling Green and Owensboro among its Kentucky individual coverage areas, and members in Warren and Daviess counties can use no-cost virtual visits on most individual plans before stepping up to in-person care. Fairview area programs and rehab centers in Owensboro are listed.

Can I use Anthem for rehab in a different state?

Sometimes. Out-of-state payment depends on the plan type: Anthem HMO certificates elsewhere limit out-of-network substance use care to emergencies and urgent care, and employer plans write their own terms. Elevance also holds the Blue license in Indiana, Ohio, Missouri and most of Virginia, but Tennessee, West Virginia and Illinois have separate Blue companies, so ask Anthem before a planned stay.

How much does rehab cost with Anthem?

Anthem costs depend on the plan. Anthem tells members to look at their benefit materials or call the number on the ID card for copays, and kynect subsidies apply only to Anthem plans bought through the Kentucky Health Benefit Exchange. Anthem says plans with a no-premium option are not available in all areas and depend on federal income guidelines.

Anthem is one of several ways Kentuckians pay for treatment, alongside the five Kentucky Medicaid plans, Medicare, state funds and self-pay. A member who loses job-based Anthem coverage can look at an Anthem kynect plan or, if eligible, Kentucky Medicaid. Rehabs accepting private insurance and Medicaid-covered rehab are on separate pages.
