Intensive Outpatient Programs (IOP) in Kentucky

Intensive outpatient programs, usually shortened to IOP, let a person with a substance use disorder attend structured treatment on several days each week while living at home and keeping up with work, school or family. In Kentucky, IOP listings run from Mount Vernon, London and Richmond to Ashland and the Lexington area, and half of the mapped programs sit in rural parts of the state.

Listings on RehabSeekers.com draw on government treatment registers, state licensing records, accreditor listings and details supplied by the providers themselves. Before you plan around a schedule, confirm hours, fees and openings with the admissions team.

Common questions about IOP

Answered from what is listed here.

Where are IOP programs in Kentucky?

Mount Vernon, Ashland, London, Richmond and Lexington-Fayette are where intensive outpatient listings in Kentucky cluster. Larger cities nearby include Lexington, Meads and Louisville. Since sessions run on set days each week, people can ask intake staff about evening or daytime group schedules before choosing a program.

Rehab Centers in Mount Vernonhttps://rehabseekers.com/locations/us/kentucky/mount-vernon-ky/
Do IOP programs in Kentucky take Kentucky Medicaid?

Kentucky Medicaid coverage is recorded by nearly all of the intensive outpatient programs in Kentucky as a payment source, and behavioral health benefits sit inside the managed care plans.

Coverage runs through the plan a member is enrolled in, such as Aetna Better Health of Kentucky, Humana Healthy Horizons in Kentucky or WellCare of Kentucky. Admissions teams can confirm which plans a program takes before the first session.

Rehab Centers in Kentucky That Accept Kentucky Medicaidhttps://rehabseekers.com/locations/us/kentucky/kentucky-medicaid/
Are there IOP programs near a river in Kentucky?

Within 25 miles of a river sit nearly all of the intensive outpatient programs in Kentucky, with a median of about 12.9 miles from listed facilities to the nearest river.

The Dix River, the Little Laurel River and the Salt River are the most-named rivers near these programs. Because people attend from home, a nearby river says only where a clinic sits, not anything about the care.

What is an intensive outpatient program for substance use in Kentucky?

An intensive outpatient program (IOP) in Kentucky is a structured, nonresidential level of care for substance use disorders that meets for several hours on multiple days each week. Sessions usually combine group counseling, individual therapy and family sessions, and some programs also manage medication while people continue living at home.

In Kentucky, nonhospital alcohol and other drug treatment programs are licensed by the Office of Inspector General within the Cabinet for Health and Family Services, under 908 Kentucky Administrative Regulations 1:370. Statewide substance use disorder services are overseen by the Division of Substance Use Disorder in the Department for Behavioral Health, Developmental and Intellectual Disabilities.

Kentucky IOPs are not all the same kind of program. Some treat mental health conditions only and do not offer substance use disorder care. If you need help with both, look at dual diagnosis programs in Kentucky, and ask intake staff which conditions their IOP treats.

Under national placement standards, adult intensive outpatient care means at least 9 hours of treatment a week, or 6 hours for adolescents, and its length depends on progress rather than a fixed calendar. Kentucky Medicaid pays for these services through its managed care plans, and many programs report evening or virtual groups for people with daytime jobs.

How many hours a week does IOP take, and how long does it last?

Under the ASAM Criteria, the national placement standard that defines this level of care as Level 2.1, adult IOP means at least 9 hours of treatment a week. For adolescents, the threshold is 6 hours. Kentucky’s licensing rule, 908 KAR 1:370, sets the standards a licensed program must follow, so ask the program how its own weekly schedule is set up. The length of IOP depends on your assessment and your progress, not on a fixed calendar. Staff review the treatment plan as goals are met and may change it.

Level of careWeekly hours (ASAM)Setting
Standard outpatient (Level 1)Fewer than 9 for adultsLive at home
IOP (Level 2.1)9 or more for adults, 6 or more for teensLive at home
PHP (Level 2.5)20 or moreDay program, home at night
Residential (Level 3)Around the clockLive on site

Is IOP the right level of care, or is PHP or residential needed?

A clinical assessment decides whether IOP fits. It tends to suit people who are medically stable, have a safe place to live and can get to sessions. Stopping alcohol, benzodiazepines or opioids can bring on risky withdrawal, so for those substances withdrawal management in Kentucky usually comes first. Some people need more structure than IOP gives. For them, a partial hospitalization program or residential treatment may be recommended. Others start with lighter standard outpatient care, or step down to it after IOP.

Does Kentucky Medicaid or private insurance cover IOP?

Kentucky Medicaid pays for intensive outpatient program services. The Department for Medicaid Services’ 2026 behavioral health fee schedule lists IOP as a daily billable service. Most members get care through one of five managed care plans:

  • Aetna Better Health of Kentucky
  • Humana Healthy Horizons in Kentucky
  • Passport Health Plan by Molina Healthcare
  • UnitedHealthcare Community Plan
  • WellCare of Kentucky

Anthem stopped being a Kentucky Medicaid plan on January 1, 2025. You can find programs that take Medicaid in the listings.

Kentucky residents without coverage have other routes. The Adult Substance Use Treatment and Recovery Services Branch oversees state and federal funds for community providers, and many of those providers appear among state-funded programs. Medicare may cover intensive outpatient care at some hospital outpatient departments and clinics; check with your plan or browse programs that take Medicare. Private insurers often ask for prior authorization, so call the number on your card before your first session.

Can I attend IOP in the evenings or online in Kentucky?

Each Kentucky IOP listing shows whether the provider reports day, evening or virtual sessions. Evening groups usually suit people who work days. Kentucky Medicaid covers telehealth under 907 KAR 3:170, including audio-only visits, and members can ask for an in-person or video visit instead. For remote options, see telehealth treatment in Kentucky.

How far you travel matters when you attend several times a week. In the RehabSeekers.com map data, 96 percent of Kentucky IOP programs sit within 25 miles of a river, most often the Dix River, at a median of 12.9 miles. Blue Grass Airport in Lexington is the most common nearest airport, and the median distance to any airport is 32.1 miles. Half of the programs are rural, 35 percent urban and 15 percent in towns.

Reading the listings and checking a license

Kentucky IOP listings show accepted insurers, languages spoken and the groups each program serves.

  • Teens: a minor may consent to substance use treatment without a parent under KRS 214.185. See teen programs.
  • Women: KY-Moms MATR offers case management for pregnant and postpartum women in all 14 regions. See women’s programs.
  • Veterans: VA care is based in Lexington and Louisville. See veterans programs.

To check a license, ask the program for its alcohol and other drug entity license number, then confirm it with the Office of Inspector General in the Cabinet for Health and Family Services. If you need to talk to someone now, call or text 988. Find Help Now Kentucky (800-854-6813) can point you to treatment openings, and in eastern Kentucky the Operation UNITE line (866-908-6483) takes referral calls.
