Common questions about Opioid Rehab

Answered from what is listed here.

Where are opioid rehabs in Kentucky?

Opioid treatment listings in Kentucky cluster in Mount Vernon, Ashland, Elizabethtown, London and Morehead. Residents around Lexington and Louisville can widen a search outward from those cities as well. Comparing drive times matters, since medication programs can involve frequent, sometimes daily, visits to the clinic.

Rehab Centers in Mount Vernonhttps://rehabseekers.com/locations/us/kentucky/mount-vernon-ky/
Can I use Kentucky Medicaid for opioid rehab?

Nearly all of the opioid rehabs in Kentucky record accepting Kentucky Medicaid. Coverage flows through the managed care plan a member is enrolled in, such as Passport Health Plan by Molina Healthcare, UnitedHealthcare Community Plan or Aetna Better Health of Kentucky.

Anthem stopped being a Kentucky Medicaid plan on January 1, 2025, so anyone still holding an Anthem card can confirm their current plan before calling a program's admissions team.

Rehab Centers in Kentucky That Accept Kentucky Medicaidhttps://rehabseekers.com/locations/us/kentucky/kentucky-medicaid/
Are there opioid rehabs near a river in Kentucky?

Nearly all of the opioid rehabs in Kentucky have a river within 25 miles, at a median of about 12.2 miles from each listed facility. The Dix River, Salt River and Blue River Branch come up by name more than others, though how close any one program sits to the water varies by address.

How do I start methadone or buprenorphine treatment in Kentucky?

To start methadone in Kentucky, you enroll at a certified opioid treatment program; to start buprenorphine, you can go to one of those programs or see an office-based or telehealth prescriber. Under federal rules in Title 42 of the Code of Federal Regulations, Part 8, methadone for opioid use disorder is dispensed only through a certified program.

Kentucky’s Department for Behavioral Health, Developmental and Intellectual Disabilities serves as the State Opioid Treatment Authority that oversees these programs. Buprenorphine no longer needs a separate federal waiver, so a clinician with a standard Drug Enforcement Administration registration can prescribe it from an ordinary office.

Methadone begins with a first dose at the clinic after a medical assessment, while buprenorphine usually starts once withdrawal has begun, and people who have used fentanyl sometimes need adjusted timing. Extended-release naltrexone is a monthly injection given only after withdrawal is complete.

Kentucky Medicaid pays for medication treatment through five managed care plans, each with its own prior authorization rules, and Medicare Part B covers methadone at enrolled programs. Uninsured residents can call Find Help Now Kentucky at 800-854-6813, and anyone leaving treatment should keep naloxone nearby, since tolerance drops after a break from use.

MedicationWhere you get itWhen it starts
MethadoneCertified OTP onlyFirst dose at the clinic after a medical assessment
BuprenorphineOTP, office-based prescriber or telehealthUsually once withdrawal has begun, though prescribers use different start methods
Extended-release naltrexonePrescriber’s office or clinic, as a monthly injectionOnly after withdrawal is complete, because an early dose can bring on sudden withdrawal

Kentucky Medicaid covers telehealth visits under 907 KAR 3:170, including audio-only visits, and members can ask for an in-person or video visit instead. We found no Kentucky-specific rule on starting buprenorphine by telehealth. Federal telemedicine rules for controlled medications have changed several times, so ask the prescriber how a first visit works. Programs that dispense methadone are on our methadone clinics in Kentucky page. Office-based prescribers appear under Suboxone clinics in Kentucky, and remote options are on the telehealth listings.

What kinds of opioid treatment programs are available in Kentucky?

Kentucky listings for opioid use disorder include OTPs, withdrawal management, residential programs, partial hospitalization, intensive outpatient, standard outpatient and telehealth care. Fentanyl is now common in the illicit drug supply. That is one reason many people choose medically managed withdrawal or a supervised medication start rather than stopping alone. People who have been using fentanyl sometimes find a standard buprenorphine start harder, and prescribers may adjust timing or dosing.

Withdrawal management is a first step, not treatment on its own. Before discharge, ask intake staff about the next level of care, such as an intensive outpatient program in Kentucky or ongoing medication visits.

Do residential programs accept people taking methadone or buprenorphine?

Some Kentucky residential programs accept people who are already taking methadone or buprenorphine, and some do not, so ask the admissions team directly. Useful questions include whether a prescriber works on site and how methadone doses are arranged with your OTP. Ask too whether buprenorphine can continue through discharge. Residential rehab listings in Kentucky show the medication services each program reports.

Does Kentucky Medicaid pay for opioid treatment medications?

Kentucky Medicaid pays for behavioral health care, including substance use treatment, through five managed care plans. Each plan sets its own rules for medications such as buprenorphine and naltrexone. The Department for Medicaid Services contracts with Aetna Better Health of Kentucky, Humana Healthy Horizons in Kentucky, Passport Health Plan by Molina Healthcare, UnitedHealthcare Community Plan and WellCare of Kentucky. Anthem left Kentucky Medicaid at the start of 2025. Ask your plan about prior authorization and which OTPs are in network. Providers that take the program are under Medicaid-accepting rehab.

Medicare Part B covers services at enrolled opioid treatment programs, including methadone. If you have no insurance, the Division of Substance Use Disorder funds community providers with state and federal money, and the Kentucky Overdose Response Effort contracts with more than 70 organizations for treatment and recovery. Find Help Now Kentucky, 800-854-6813, lists treatment openings. Programs that use public funding appear on our state-funded rehab in Kentucky page.

Pregnancy and opioid treatment

Pregnant people get preference for admission to treatment funded by the federal block grant (42 U.S.C. 300x-27), and many OTPs prioritize them. The state’s KY-Moms MATR case management serves pregnant and postpartum women in all 14 regions. Rehab for pregnant women in Kentucky lists providers that report prenatal coordination.

Where can I get naloxone in Kentucky?

You can get naloxone at Kentucky pharmacies or free through harm reduction programs listed by the state Office of Drug Control Policy. Under KRS 217.186, pharmacists with Kentucky Board of Pharmacy certification dispense naloxone under a physician-approved protocol. Naloxone nasal spray is also sold over the counter. The Stop Overdoses pages and the Find Naloxone Now Kentucky locator show free naloxone and syringe exchange sites.

Tolerance drops after withdrawal, a residential stay, jail or any break from using. A dose that was once familiar can then cause an overdose, so keep naloxone nearby when leaving treatment. In an overdose, call 911. Kentucky’s Good Samaritan law, KRS 218A.133, protects a person who seeks medical help in good faith from prosecution for possessing drugs or paraphernalia.

What do the Kentucky opioid listings show?

Each Kentucky opioid listing shows the accreditation, languages, populations served and address that the provider reports. Nearly all listed opioid programs, 97 percent, sit within 25 miles of a river, most often the Dix River. The median distance to the nearest river is 12.2 miles. Blue Grass Airport in Lexington is the most common nearest airport, and the median trip to the nearest airport is 33.7 miles.
