Treatment Centers in Kentucky That Accept Molina

Molina covers drug and alcohol rehab in Kentucky under the name Passport by Molina Healthcare, which runs one of the five Kentucky Medicaid plans, Passport Advantage dual plans for people with Medicare and Medicaid, and a reduced set of marketplace plans. Passport was a Kentucky plan before Molina bought it in 2020.

Every Passport listing below carries the program’s phone line, along with the program’s services, say withdrawal management, a residential bed or an opioid treatment clinic, and any Joint Commission or CARF seal. Federal treatment registers and Kentucky’s licensing records feed the listings. A town or service filter shortens the page.

Does Molina Cover Rehab in Kentucky?

Yes. Passport by Molina Healthcare lists substance use disorder services for its Kentucky Medicaid members, including outpatient opioid treatment, outpatient and non-hospital medical withdrawal management, crisis stabilization and peer support. Programs that bill rehabs that accept Molina appear among the recovery programs in Kentucky listed on this page.

Passport by Molina Healthcare, formerly Passport Health Plan, holds a 2026-2027 renewal contract under Kentucky Medicaid’s managed care program and enrolls members in every county. The Passport name covers Molina’s Kentucky Medicaid, Medicare and marketplace plans, and Kentucky pharmacy questions go to MedImpact, the state’s Medicaid pharmacy manager.

Does Molina still sell marketplace plans in Kentucky?

Yes, for 2026, on a smaller scale. Molina’s annual report lists Kentucky among the fourteen states where it sold 2026 marketplace plans, but a June 2026 Robert Wood Johnson Foundation report describes a major Molina pullback in Kentucky. Molina has said it will narrow marketplace sales to about six states for 2027 without naming them.

Does Molina Cover Inpatient Rehab in Kentucky?

Yes. Passport by Molina pays for 30-day residential rehab for Medicaid and inpatient addiction programs, using the Kentucky Medicaid waiver that funds certified residential providers at each of the state’s four ASAM residential levels. Passport Advantage, Molina’s Kentucky dual plan, covers inpatient hospital stays and lists psychiatric hospital care with Medicare’s 190-day lifetime limit.

How long will Molina pay for residential rehab?

Passport by Molina authorizes residential days by assessment against the ASAM Criteria, which Kentucky adopted for every Medicaid plan. The state’s waiver tracks a 30-day statewide average across all residential stays, not a cap on any one member’s stay, so a Passport member can be authorized for more days when the assessment supports it.

Does Molina need prior authorization for inpatient rehab?

Yes. Passport by Molina requires prior authorization for residential substance use treatment and inpatient admissions, as all five Kentucky Medicaid plans have since utilization management resumed on June 25, 2025. Providers request it through Passport’s behavioral health authorization line, 1-800-578-0775, and out-of-network providers need authorization before any non-emergency care.

Does Molina Cover Detox in Kentucky?

Yes. Passport by Molina covers outpatient withdrawal management, non-hospital medical withdrawal management and withdrawal management crisis stabilization at alcohol and drug treatment centers. Kentucky members can start at ambulatory detox programs, and Passport does not require prior authorization for crisis stabilization units, according to the state’s 2025 comparison of plan rules.

Does Molina Cover Outpatient Rehab in Kentucky?

Yes. Passport by Molina members need no referral to see a behavioral health provider and can pick or change their provider or case manager at any time, as long as the provider is in Passport’s network. Intensive outpatient treatment and partial hospitalization need Passport’s authorization, and Passport lists 16 intensive outpatient units or days a year before further authorization.

Passport by Molina also requires authorization for peer support, with an annual threshold of 200 units, about 50 hours. Passport adds Teladoc visits from home, PsychHub’s library of mental health videos, and community connectors who link members to transport, housing, schooling and work after residential care.

Does Molina Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Passport by Molina lists outpatient opioid treatment as a covered Medicaid service and does not require prior authorization for Kentucky Medicaid’s medication treatment service codes. Prescriptions for addiction treatment medications run through MedImpact under the Kentucky Medicaid preferred drug list, the same list every Kentucky Medicaid plan uses.

Does Molina cover methadone?

Yes. Passport by Molina Medicaid members in every county get methadone treatment programs through Kentucky Medicaid, which added methadone on July 1, 2019. Passport Advantage covers opioid treatment program services with no copay, though its 2026 summary of benefits says the medication itself needs prior authorization.

Does Molina cover Suboxone?

Yes. Kentucky Medicaid stopped requiring prior authorization for buprenorphine-naloxone film and generic buprenorphine tablets in July 2021, so Passport members can fill buprenorphine maintenance prescriptions without that step. Sublocade for Medicaid members was preferred with prior authorization on the 2024 list.

Molina Rehab Centers Near Dale Hollow Lake, Along the Green River and in the Knott County Hills

Passport by Molina’s Medicaid plan covers all 120 Kentucky counties, while Passport Advantage is sold only in named counties, so the setting matters more for dual members. Outside an emergency, Passport pays network providers only, so a Passport member weighing a rural program should first find out if it contracts with Passport.

Molina rehab near Dale Hollow Lake

Passport members in Clinton and Cumberland counties live along Dale Hollow Lake, which crosses the Tennessee line south of Albany and Burkesville. Programs close to lakes near treatment options in Albany must be in Passport’s network, and a Tennessee program across the lake would need Passport’s authorization before any planned stay.

Molina rehab along the Green River

Passport Medicaid and marketplace members along the Green River live in a corridor that runs from Green River Lake past Munfordville and Mammoth Cave to Calhoun and the Ohio near Henderson. Rehab near rivers and programs in Munfordville use the same Passport authorization line.

Molina rehab in the Knott County hills

Passport members in Knott and Floyd counties live among the steep Cumberland Plateau ridges around Hindman. Mountain treatment programs and rehab programs in Hindman are listed; a Medicaid ride to a program farther off is booked with the state’s regional broker, and leaving the member’s own medical service area takes a primary care referral.

How Do I Find a Rehab That Accepts Molina in Kentucky?

Browse the listings on this page. Molina is accepted by each Kentucky program shown. Use the city and service filters, then open a program for its phone number, what it treats and its accreditation. Say whether you have Passport Medicaid, Passport Advantage or a marketplace plan, since each has its own network and member line.

Passport’s behavioral health authorization line decides Molina admissions to residential care, a hospital, a day program or intensive outpatient, and the program normally files after its assessment. Passport member services, (800) 578-0603, can explain benefits. The lines below are for authorization, benefits and crisis help, not for choosing a program.

Audubon Park treatment programs and Germantown recovery services are among the Louisville listings.

Molina rehab in Covington and Northern Kentucky

Molina rehab in Lexington and Winchester

Molina rehab in Paducah and Murray

Passport by Molina members in McCracken and Calloway counties, in the Jackson Purchase at the far western end of the state, can use Teladoc for a first visit from home before traveling for treatment, and the plan’s Nurse Advice Line, (800) 606-9880, runs 24 hours. Downtown Paducah programs are listed.

Can I use Molina for rehab in a different state?

Rarely. Passport by Molina members must use a Passport network provider unless it is an emergency, and Kentucky Medicaid plans cover planned care outside the network only with authorization. Passport Advantage and Passport marketplace plans are built on Kentucky service areas, so members of those plans should ask Passport before any planned out-of-state stay.

How much does rehab cost with Molina?

Nothing, for Passport Medicaid members: Kentucky regulation bars copayments anywhere in Medicaid. Passport Advantage lists no copay for inpatient stays, outpatient substance use therapy or opioid treatment program services in its 2026 summary. Marketplace members pay their plan’s deductible and copays, and Molina’s marketplace rates rose for 2026 across its states.

Molina is one of several payers listed on Kentucky programs. A program that does not take Passport may still bill another Kentucky Medicaid plan, Medicare, private insurance or state funds, or accept self-pay, and its facility page says which. Medicaid detox programs and Medicaid rehab options are gathered on separate pages.
